Abstract: Surgical site infections (SSIs) are difficult to treat and are associated with substantially longer hospital stay, higher treatment cost, morbidity and mortality, particularly when the etiological agent is multidrug-resistant (MDR). To address the limited data in Uganda on SSIs, we present the spectrum of bacteria isolated from hospitalized patients, the magnitude and impact of MDR bacterial isolates among patients with SSIs. A descriptive cross sectional study was conducted from September 2011 through April 2012 involving 314 patients with SSIs in the obstetrics & gynecology, general surgery and orthopedic wards at Mulago National Hospital in Kampala, Uganda. Wound swabs were taken and processed using standard microbiological methods. Clinico-demographic characteristics of patients were obtained using structured questionnaires and patients’ files. Of the 314 enrolled patients with SSIs (mean age 29.7 ±13.14 years), 239 (76.1%) were female. More than half of the patients were from obstetrics and gynecology (62.1%, 195/314). Of 314 wound swabs taken, 68.8% (216/314) were culture positive aerobically, yielding 304 bacterial isolates; of which 23.7% (72/304) were Escherichia coli and 21.1% (64/304) were Staphylococcus aureus. More than three quarters of Enterobacteriaceae were found to be extended spectrum beta lactamase (ESBL) producers and 37.5% of S. aureus were Methicillin resistant S. aureus (MRSA). MDR occurred in 78.3% (238/304) of the isolates; these were more among Gram-negative bacteria (78.6%, 187/238) compared to Gram-positive bacteria (21.4%, 51/238), (p-value < 0.0001, χ2 = 49.219). Amikacin and imepenem for ESBL-producing Enterobacteriacea and vancomycin for MRSA showed excellent performance except that they remain expensive drugs in Uganda. Most SSIs at Mulago National Hospital are due to MDR bacteria. Isolation of MRSA and ESBL-producing Enterobacteriaceae in higher proportions than previously reported calls for laboratory guided SSIs- therapy and strengthening of infection control surveillance in this setting.

Objective To describe how the SUPPORT collaboration developed a short summary format for presenting the results of systematic reviews to policy-makers in low- and middle-income countries (LMICs).Methods We carried out 21 user tests in six countries to explore users’ experiences with the summary format. We modified the summaries based on the results and checked our conclusions through 13 follow-up interviews. To solve the problems uncovered by the user testing, we also obtained advisory group feedback and conducted working group workshops.Findings Policy-makers liked a graded entry format (i.e. short summary with key messages up front). They particularly valued the section on the relevance of the summaries for LMICs, which compensated for the lack of locally-relevant detail in the original review. Some struggled to understand the text and numbers. Three issues made redesigning the summaries particularly challenging: (i) participants had a poor understanding of what a systematic review was; (ii) they expected information not found in the systematic reviews and (iii) they wanted shorter, clearer summaries. Solutions included adding information to help understand the nature of a systematic review, adding more references and making the content clearer and the document quicker to scan.Conclusion Presenting evidence from systematic reviews to policy-makers in LMICs in the form of short summaries can render the information easier to assimilate and more useful, but summaries must be clear and easy to read or scan quickly. They should also explain the nature of the information provided by systematic reviews and its relevance for policy decisions.

Abstract: The impact of waste disposal on trace metal contamination was investigated in eleven wetlands in the Lake Victoria Basin. Samples of soil, water and plants were analysed for total Zn, Cu, Pb and Ni concentrations using flame atomic absorption spectrophotometry. The trace metal concentrations in soil were the highest in Katanga wetland with the highest mean concentrations of 387.5±86.5 mg/kg Zn, 171.5±36.2 mg/kg Pb, 51.20±6.69 mg/kg Cu and 21.33±2.23 mg/kg Ni compared to the lowest levels observed at Butabika (30.7±3.2 mg/kg Zn, 15.3±1.7 mg/kg Pb, 12.77±1.35 mg/kg Cu and 6.97±1.49 mg/kg Ni). Katanga receives waste from multiple industrial sources including a major referral city hospital while Butabika is a former solid waste dumpsite. Wetland soil near a copper smelter had a Cu concentration of 5936.3±56.2 mg/kg. Trace metal concentrations in industrial effluents were above international limits for irrigation water with the highest concentrations of 357,000 µg/L Cu and 1480 µg/L Zn at a Cu smelter and 5600 µg/L Pb at a battery assembling facility compared to the lowest of 50 µg/L Cu and 50 µg/L Zn in water discharged from Wakaliga dumpsite. Uptake of trace metals from soil differed from plant to plant and site to site. Higher levels of trace metals accumulated in the root rather than in the rhizome and the least amount was in the leaf. The study identifies industry as a potential source of trace metal contamination of water and the environment pent-up need for policy intervention in industrial waste management .

IPTst had no significant effect on the incidence of symptomatic malaria or the prevalence of anemia, but reduced the prevalence of asymptomatic parasitemia by 54% (95% CI, 47%–60%, P< .0001). Conclusions. Monthly IPT with DPoffered remarkable protection against clinical malaria, parasitemia, and anemia in schoolchildren living in a high-malaria-transmission setting.
