Abstract: The definition of molecular cooperativity is discussed. The characteristic length of the glass transition describes the size of this cooperativity. Differential scanning calorimetry (DSC) and heat capacity spectroscopy (HCS) results of a series of poly(n-alkyl methacrylates) (alkyl = methyl, ethyl, propyl, butyl, pentyl, hexyl, and octyl) and a series of statistical copolymers poly(n-butylmethacrylate-stat-styrene) are discussed in terms of molecular cooperativity in the αβ splitting region, where a high-frequency dispersion zone a splits off into the main transition zone α and a Goldstein Johari process β at lower frequencies. The characteristic length tends to small values of order one monomer diameter in the splitting region for scenarios with an α relaxation onset. The statements about the size scale of cooperativity are conditional upon certain assumptions leading to the equation used for calculation of this size from HCS and DSC data. The step height of heat capacity (Δcp) and, with less certainty, the square root of the cooperativity volume or number (V1/2α or 1/2α) are proportional to the temperature distance from the cooperativity onset, T = Tons. © 1996 John Wiley & Sons, Inc.

Abstract: Patients with infective endocarditis (IE) form a heterogeneous group, ranging from those who are successfully treated with no adverse events, to those with severe complications and a high mortality. In this Review, we highlight pathogen-host interactions and the mechanisms underlying various risk factors for patients with IE. A temporal trend in the pattern of IE has been observed in high-income countries within the past 5 decades, with patients contracting IE at an increasingly old age, and a growing incidence of health-care-associated staphylococcal IE. Consequently, prevention strategies should no longer focus on prophylaxis of streptococcal bacteraemia during dental procedures, but instead encourage a more-general approach to reduce the incidence of health-care-associated IE. Much knowledge has been gained about the mechanisms of vegetation formation, growth, and embolization on damaged or inflamed cardiac valves, and on cardiac devices. Improved understanding of these mechanisms will help to combat the increasing problem of antimicrobial resistance. Two mechanisms of IE should increasingly be the focus of future research: the role of immunosenescence in elderly patients with IE, particularly after transcatheter aortic valve implantation, and the mechanisms that trigger septic shock, a condition that leads to a substantial increase in the risk of death in patients with IE.

Abstract: Context: Pheochromocytomas are rare tumors of predominantly adrenal origin that often produce and secrete catecholamines. Malignancy occursinavariablepercentageofcasesdependingongeneticbackground andtumorlocation.Definitivediagnosisreliesonthedetectionofdistant metastases. Treatments for malignant pheochromocytoma include surgical debulking, pharmacological control of hormone-mediated symptoms, targeted methods such as external irradiation, and systemic antineoplastic therapy. Different agents and protocols for this purpose are reviewed, and their therapeutic potential is discussed. Evidence Acquisition: Literature on antineoplastic therapies for malignant pheochromocytoma was identified by searching the PubMed database with restriction to articles published in English during the past 30 yr. Evidence Synthesis: Because of the rarity of the condition, no randomized clinical trials concerning the treatment of malignant pheochromocytoma have been performed. The strategy established best is [ 131 I]meta-iodobenzylguanidine (MIBG) therapy, which is well tolerated. Similar to cytotoxic chemotherapy with cyclophosphamide, vincristine, and dacarbazine, MIBG can induce remission for a limited period in a significant proportion of patients. Octreotide as a single agent seems to be largely ineffective. Conclusions: MIBG radiotherapy and cyclophosphamide, vincristine, and dacarbazine chemotherapy are comparable with respect to response rate and toxicity. It is unclear whether combining both can improve the outcome. Future developments may include new multimodal concepts with focus on inhibition of angiogenetic factors and heat shock protein 90. Any present or new therapeutic approach must take into account the highly variable natural course of the disease. (J Clin Endocrinol Metab 92: 1217–1225, 2007)
