Abstract: OBJECTIVE To assess the level and predictors of physical activity at discharge among children recovering from severe acute malnutrition (SAM). METHODS We conducted a prospective study among 69 children 6-59 months of age admitted with SAM for nutritional rehabilitation at Mulago National Referral Hospital, Uganda. Using hip-mounted triaxial accelerometers, we measured physical activity expressed as counts per minute (cpm) during the last three days of hospital treatment. As potential predictors, we assessed clinical and background characteristics, duration to transition phase and duration of hospitalisation, serum C-reactive protein and whole-blood docosahexaenoic acid (DHA). Multiple linear regression analyses were used to identify predictors of physical activity. RESULTS The median (IQR) age was 15.5 (12.6; 20.5) months. At discharge, the mean (SD) movement was 285 (126) cpm. Physical activity was 43 (19; 67) cpm higher for each unit increase in weight-for-height z-score (WHZ) and 72 (36; 108) cpm higher for each centimetre increase in MUAC. Whole-blood DHA on admission was also a positive predictor of physical activity, whereas duration to transition phase and duration of hospitalisation were both negative predictors. CONCLUSION The level of physical activity at discharge among children treated for SAM was low. WHZ, MUAC and DHA on admission were positive predictors of physical activity, whereas duration of stabilisation and hospitalisation was negative predictors of physical activity. These results suggest that assessment of physical activity may be used as a marker of recovery.

Abstract: Background. Good postoperative analgesic management improves maternal satisfaction and care of the neonate. Postoperative pain management is a challenge in Mulago Hospital, yet ketamine is accessible and has proven benefit. We determined ketamine’s postoperative analgesic effects. Materials and Methods. We did an RCT among consenting parturients that were randomized to receive either intravenous ketamine (0.25 mg/kg) or placebo after spinal anesthetic. Pain was assessed every 30 mins up to 24 hours postoperatively using the numerical rating scale. The first complaint of pain requiring treatment was noted as “time to first breakthrough pain.” Results. We screened 100 patients and recruited 88 that were randomized into two arms of 44 patients that received either ketamine or placebo. Ketamine group had 30-minute longer time to first breakthrough pain and lower 24-hour pain scores. Postoperative diclofenac consumption was lesser in the ketamine group compared to placebo and Kaplan-Meier graphs showed a higher probability of experiencing breakthrough pain earlier in the placebo group. Conclusion. Preincision intravenous ketamine (0.25 mg/kg) offered 30-minute prolongation to postoperative analgesia requirement with reduced 24-hour pain scores. We recommend larger studies to explore this benefit. This trial is registered with Pan African Clinical Trial Registry number PACTR201404000807178 .

Abstract: INTRODUCTON: Breast cancer is one of the leading cancers amongst women world-wide. Although mortality has been reduced and survival rates increased in developed countries, mortality rates from breast cancer are still a major health challenge for many developing countries. In Uganda, there are no screening programmes and in many cases mammography is used for diagnostic purposes. The purpose of this study was to describe the clinical presentations and mammographic breast density patterns amongst women that presented to the radiology department for mammography at a national referral hospital. METHODS: This was a retrospective study carried out at Mulago Hospital in Uganda between January 2011 and January 2012. Records for patients who had mammography during this period were reviewed. RESULTS: The total number of patients was 382 with a mean age of 46 years. Majority presented with breast pain and masses. Mammograms done were normal in majority of the women with fatty breast density dominating. In Uganda, mammography was mainly performed for diagnostic purposes. CONCLUSION: There is no mammography screening programme in Uganda and many women cannot access the service due to its limited availability and costs despite its significance in breast cancer management. There is therefore need for governments in Uganda, but in other areas as well to support regular mammography screening as a way of reducing mortality from breast cancer.
