Abstract: We have generated a high-resolution (<20 yr) 2 3 0 Th-dated stalagmite oxygen isotope record from Timta Cave in the western Himalaya in India that documents Southwest Indian summer monsoon (ISM) precipitation variations during the Bolling-Allerod interstadial from 15.2 to 11.7 ka. Compared with the glacial and Younger Dryas, ISM precipitation was enhanced during the Bolling-Allerod. ISM precipitation was apparently coupled to variations in the East Asian monsoon and North Atlantic climate on millennial and multicentennial time scales during the deglaciation. Analyses of a high growth rate interval (<2.5 yr resolution) encompassing the late Bolling-early Alterod suggest that multidecadal monsoon variability was an important aspect of ISM behavior at that time. The frequency spectrum of ISM precipitation during this time interval is similar to that of the Δ 1 4 C record and other ISM precipitation records during'the latest Holocene. This raises the hypothesis that multidecadal climate dynamics during the late Boiling-early Allerod may have been similar to those that operated during the last several millennia, even though the boundary conditions of these two time intervals were very different.

Abstract: Universities play an important role in the development of a country in this age of the knowledge economy. As government subsidies to universities have been reducing in recent years, the more efficient use of resources becomes an important issue for university administrators. This paper applies data envelopment analysis (DEA) to assess the relative efficiency of the academic departments at National Cheng Kung University in Taiwan. The outputs considered are total credit-hours, publications, and external grants; and the inputs utilized by the departments are personnel, operating expenses, and floor space. An assurance region is constructed by the top administrators of the university to confine the flexibility in selecting the virtual multipliers in DEA. Four groups of departments of similar characteristics are categorized via an efficiency decomposition and cluster analysis. The aggregate efficiency indicates whether the resources have been utilized efficiently by a department and the efficiency decomposition helps identify the weak areas where more effort should be devoted so that the efficiency of the department can be improved.

Abstract: In this paper, a novel high step-up dc-dc converter for distributed generation systems is proposed. The concept is to utilize two capacitors and one coupled inductor. The two capacitors are charged in parallel during the switch-off period and are discharged in series during the switch-on period by the energy stored in the coupled inductor to achieve a high step-up voltage gain. In addition, the leakage-inductor energy of the coupled inductor is recycled with a passive clamp circuit. Thus, the voltage stress on the main switch is reduced. The switch with low resistance RDS(ON) can be adopted to reduce the conduction loss. In addition, the reverse-recovery problem of the diodes is alleviated, and thus, the efficiency can be further improved. The operating principle and steady-state analyses are discussed in detail. Finally, a prototype circuit with 24-V input voltage, 400-V output voltage, and 200-W output power is implemented in the laboratory to verify the performance of the proposed converter.

Abstract: Background and Aims: This study assessed the efficacy and safety of up to 4 years of lamivudine treatment and the clinical relevance of the emergence of YMDD-variant hepatitis B virus (HBV). Methods: Fifty-eight Chinese adult patients with chronic hepatitis B (CHB) were randomized to lamivudine 100 mg/day for up to 5 years and were monitored for YMDD-variant HBV, hepatitis B e antigen (HBeAg) seroconversion (loss of HBeAg and detectable antibody to HBeAg) and serum alanine aminotransferase (ALT) concentrations. Four-year data are reported here. Results: The rate of HBeAg seroconversion increased with extended therapy and also with higher baseline ALT concentrations. YMDD-variant HBV was detected in 67% (39/58) of patients at some point during treatment. After 4 years, a total of 47% (27/58) of patients achieved HBeAg seroconversion. Thirty-three per cent (13/39) of patients with YMDD-variant HBV achieved HBeAg seroconversion; this increased to 57% (8/14) in patients with moderately elevated (>2–5 × upper limit of normal) pre-treatment ALT concentrations. The proportion of patients that achieved normal serum ALT increased from 29% (17/58) at baseline to 69% (31/45) following 4 years of treatment. That included 68% (23/34) of patients with YMDD-variant HBV and 73% (8/11) of those without the variant. All patients receiving lamivudine had reduced serum concentrations of HBV-DNA compared with baseline, despite the emergence of YMDD-variant HBV in 39 patients. Lamivudine was generally well tolerated; there was little change in the number or type of drug-related adverse events in the fourth year of the study. Conclusions: Despite the emergence of YMDD-variant HBV, Chinese patients showed increased HBeAg seroconversion and improvement in ALT levels with an increased duration of treatment with lamivudine.

Abstract: Background—Stroke is a leading cause of death around the world. Improving the quality of stroke care is a global priority, despite the diverse healthcare economies across nations. The American Heart Association/American Stroke Association Get With the Guidelines-Stroke program (GWTG-Stroke) has improved the quality of stroke care in 790 US academic and community hospitals, with broad implications for the rest of the country. The generalizability of GWTG-Stroke across national and economic boundaries remains to be tested. The Taiwan Stroke Registry, with 30 599 stroke admissions between 2006 and 2008, was used to assess the applicability of GWTG-Stroke in Taiwan, which spends ≈1/10 of what the United States does in medical costs per new or recurrent stroke. Methods and Results—Taiwan Stroke Registry, sponsored by the Taiwan Department of Health, engages 39 academic and community hospitals and covers the entire country with 4 steps of quality control to ensure the reliability of entered data.
