Abstract: Rationale: In studies that address health-related quality of life (QoL) and survival, subjects who die are usually censored from QoL assessments. This practice tends to inflate the apparent benefits of interventions with a high risk of mortality. Assessing a composite QoL-death outcome is a potential solution to this problem. Objectives:Todeterminetheeffectoflungvolumereductionsurgery (LVRS) on a composite endpoint consisting of the occurrence of deathoraclinicallymeaningfuldeclineinQoLdefinedasanincrease of at leasteight points in the St. George’s Respiratory Questionnaire total score from the National Emphysema Treatment Trial. Methods:Inpatientswithchronicobstructivepulmonarydiseaseand emphysema randomized to receive medical treatment (n 5 610) or LVRS(n 5608),weanalyzedthesurvivaltothecompositeendpoint, thehazardfunctionsandconstructedpredictionmodelsoftheslope of QoL decline. Measurements and Main Results: The time to the composite endpoint was longer in the LVRS group (2 years) than the medical treatment group (1 year) (P , 0.0001). It was even longer in the subsets of patients undergoing LVRS without a high risk for perioperative death and with upper-lobe-predominant emphysema. The hazard for the composite event significantly favored the LVRS group, although it was most significant in patients with predominantly upper-lobe emphysema. The beneficial impact of LVRS on QoL decline was most significant during the 2 years after LVRS. Conclusions: LVRS has a significant effect on the composite QoLsurvival endpoint tested, indicating its meaningful palliative role, particularly in patients with upper-lobe‐predominant emphysema.

Abstract: Background Commercial and residential buildings can become contaminated with molds, which may trigger allergic disorders. Mold remediation efforts may require costly replacement of mold-contaminated building materials. Disinfectants that contain dilute sodium hypochlorite can kill mold and are practical to use. Whether they also inhibit mold allergy symptoms is unknown. Objective We tested the hypothesis that sodium hypochlorite–containing spray products kill Aspergillus fumigatus and inhibit A fumigatus allergens. Methods A fumigatus was grown on 3 common building construction materials, as well as in solution by conventional laboratory methods. Two sodium hypochlorite–containing household products (diluted bleach and Tilex) were sprayed on the mold-contaminated materials or added to mold in solution and compared with untreated controls. Surface mold and associated debris were mechanically removed from treated and untreated boards. Conidia in the extracted board materials were quantified by light microscopy, examined for morphologic changes by scanning electron microscopy, and cultured for viable mold. Extracts were tested for A fumigatus antigen by ELISA, and for A fumigatus allergen by skin prick testing using extracts prepared from both the boards and the cultured solutions. Results Both sodium hypochlorite disinfectants killed A fumigatus in solution and on mold-contaminated building materials. Light microscopy and scanning electron microscopy demonstrated changes to the conidial surface. Both dilute bleach and Tilex inhibited A fumigatus recognition by ELISA. Skin testing supported the results of the ELISAs and demonstrated loss of skin test reactivity to the sodium hypochlorite–treated mold solutions in most of the subjects. Of the 4 individuals who had a positive skin test result to mold grown on oriented strand board building material, 3 no longer reacted to extracts from bleach-treated boards. Conclusion Spray application of sodium hypochlorite–containing disinfectants onto mold-contaminated building material kills A fumigatus , modifies the surface characteristics of A fumigatus conidia, reduces recognition of A fumigatus mold by ELISA, and results in loss of skin test reactivity to the treated mold in individuals allergic to A fumigatus .

Abstract: We have investigated the induction of competence (IL-2 responsiveness) and progression in human T lymphocyte proliferation triggered by phorbol ester and calcium ionophore. The degree of proliferation induced with the phorbol ester, phorbol 12,13-dibutyrate (PDB) and the calcium ionophore ionomycin was dependent on the duration of exposure to these agents, with more than 6 h required for obtaining maximum proliferation. Following brief exposure to both agents for 30 min, which did not cause significant proliferation, T cells became competent to proliferate in response to exogenous interleukin 2 (IL-2). These competent T cells also progressed to DNA synthesis following incubation with PDB in the absence of ionomycin. Induction of competence to proliferate in response to either PDB or IL-2 was blocked by EGTA, suggesting that transmembrane Ca2+ flux was obligatory at this stage. Since other phorbol esters and synthetic diacylglycerols also stimulated DNA synthesis in competent cells, it is likely that progression was triggered by activation of protein kinase C. Following a brief exposure to PDB and ionomycin, subsequent incubation with PDB induced gene expression and secretion of IL-2 and augmented the expression of IL-2 receptors in the competent cells. Thus, we have demonstrated that Ca2+ mobilization is required for rendering T cells competent to express functional IL-2 receptors, to produce IL-2 in response to subsequent incubation with PDB, and that sustained activation of protein kinase C seems necessary for IL-2 production and subsequent progression of competent T cells to DNA synthesis.

Abstract: Atopic dermatitis (AD) is a chronic, pruritic, inflammatory skin disease affecting more than 10% of all children. Sensitization to foods triggers isolated skin symptoms in about 30% of children. These symptoms include immediate reactions within minutes after ingesting food without exacerbation of AD and early and late exacerbations of AD. It is important to identify clinically relevant sensitizations to foods using skin prick tests, a specific IgE blood test (ImmunoCAP; Phadia, Portage, MI, USA), and double-blind, placebo-controlled food challenges to initiate appropriate dietary interventions and avoid unnecessary dietary restrictions. Children with AD triggered by food allergens demonstrate a distinct immune response upon stimulation of their peripheral blood mononuclear cells with food allergen. A defective skin barrier and increased intestinal permeability appear to facilitate allergen sensitization. Appropriate skin care to maintain skin barrier function and dietary avoidance of highly allergenic foods during infancy may help to prevent allergen sensitization, thereby reducing the severity of AD and food allergies.
