Presented before the 14th Annual Meeting, American College of Angiology, San Juan, Puerto Rico, Dec. 3 to 8, 1968. This paper reports a phenomenon which can occur as a result of the use of protamine sulfate to nullify the effects of heparin after completion of a vascular reconstructive procedure. In our earliest vascular reconstructive procedures of the lower extremities we usually used a light general anesthetic and invariably followed the case with intravenous protamine sulfate to counteract the effect of the heparin

Abstract: For many years now it has been our supposition that the living homologous saphenous vein should be almost as useful as the autogenous vein in arterial reconstruction. As can be appreciated, the true antigenicity of human tissue for other human tissue cannot really be tested in experimental animals; so that a human situation was necessary to validate our theories. Two years ago we were presented with an opportunity to put our theories into practical application. A 73-year-old man with a gangrenous extremity and no saphenous veins was treated by Dacron bypassing into an isolated popliteal segment vessel. This healed his gangrene; but the graft closed within four months. A subsequent lumbar sympathectomy worked for eight more months before gangrene of the foot again supervened. The patient refused amputation and was not a suitable candidate (because of the vessel’s calcified state) for superficial femoral endarterectomy. Subsequently a living homologous saphenous vein was transplanted from a female donor of the same blood type into the patient as a common femoral-distal isolated popliteal segment bypass graft. This was over two years ago-the graft healed the gangrenous foot and continues to function well without dilatation exactly similar to an autogenous vein graft. There are several reasons why we believed this should work well. The disappointing results with arterial homografts can be traced to the fact that anatomically arterial walls are considerably thicker than the saphenous vein walls. Abstract: T b e g i n with, let me point out that these comments are altogether tentative. The increasing intensity of the problem of black poverty, in all its dimensions, has provoked a veritable flash flood of information of all kinds, and at all levels, about every facet of the \"black problem.\" Thus, I am perfectly willing to entertain the notion that what I have to say will be rendered entirely wrong by some new study, some evidence of a changing consciousness on the part of blacks or whites, some increase in political repression, or even some new poem I might read that, like all good poems, will alter my own perception of what goes on out there. 1 I should like to define with some precision what I mean by \"economics\" and thus \"economists\" in this paper. \"Modern economics,\" as used here, refers to the mainstream of the ideas of American academic economists. 2 This definition excludes a variety of analyses, polemics, and descriptions coming from the fringe of the mainstream, a fringe that we will take up later. It should also be emphasized at this point that a real change is beginning to take place all over the country in the attitudes that economists have about the efficacy of the orthodox approach to solve this nation's problems. Abstract: The Bassett cuffed tracheostomy tube 1 with the Nosworthy connector is manufactured in polyvinyl chloride (PVC) and designed for coupling to standard anaesthetic fittings without reduction of lumen. Although the basic design has proved satisfactory, two disadvantages have become apparent. Firstly, stretching of the female Nosworthy connector by repeated removal and re-insertion of the male component makes for an insecure join and, secondly, the risks of infection in the tracheostomy wound and bronchial tree are increased by frequent handling of the tube. The use of a male Nosworthy suction component which is available avoids these problems, but the aperture for passage of a suction catheter is rather small. An alternative approach has been to remove the female Nosworthy connector or ‘chimney’ and use a Cobb‘s Suction union*, but unfortunately this reduces the lumen of the tube.
