Validating two survey methods for identifying cases of autism spectrum disorder among adults in the community

Abstract: BackgroundThere are no tested methods for conducting epidemiological studies of autism spectrum disorders (ASDs) in adult general population samples. We tested the validity of the Autism Diagnostic Observation Schedule module-4 (ADOS-4) and the 20-item Autism-Spectrum Quotient (AQ-20).MethodRandomly sampled adults aged ⩾16 years were interviewed throughout England in a general population multi-phase survey. The AQ-20 was self-completed by 7353 adults in phase 1. A random subset completed phase 2, ADOS-4 assessments (n=618); the probability of selection increased with AQ-20 score. In phase 3, informant-based Diagnostic Interview Schedule for Social and Communication Disorders (DISCO) and Autism Diagnostic Interview – Revised (ADI-R) developmental assessments were completed (n=56). Phase 1 and 2 data were presented as vignettes to six experienced clinicians (working in pairs). The probability of respondents having an ASD was compared across the three survey phases.ResultsThere was moderate agreement between clinical consensus diagnoses and ADOS-4. A range of ADOS-4 caseness thresholds was identified by clinicians: 5+ to 13+ with greatest area under the curve (AUC) at 5+ (0.88). Modelling of the presence of ASD using 56 DISCO assessments suggested an ADOS-4 threshold in the range of 10+ to 13+ with the highest AUC at ADOS 10+ to 11+ (0.93–0.94). At ADOS 10+, the sensitivity was 1 [95% confidence interval (CI) 0.59–1.0] and the specificity 0.86 (95% CI 0.72–0.94). The AQ-20 was only a weak predictor of ADOS-4 cases.ConclusionsClinically recommended ADOS-4 thresholds are also recommended for community cases: 7+ for subthreshold and 10+ for definite cases. Further work on adult population screening methods is needed.

Grain-Size Variation on Dunes in the Southwest Kalahari, Southern Africa

Abstract: Grain-size parameters (mean, standard deviation, skewness, and kurtosis) were determined for 83 sediment samples from six dune cross-profiles and one 28 km long-profile on dunes in the southwest Kalahari Desert in southern Africa. Although some studies elsewhere and previous studies in the Kalahari have reported significant grain-size variation over dune cross-profiles, data presented here show no systematic variation of grain-size parameters except that sediments from the crest of a dune are almost invariably better sorted (indicated by the standard deviation of the sample) than samples from the base, or plinth, of the dune. The proportion of fines (< 63 µm) in plinth sediments is also greater than in crest samples. There is no significant trend in grain-size parameters along the length of a linear dune. This lack of significant pattern corresponds with similar findings for central Australian dunes.

Choosing the Best Evidence to Guide Clinical Practice: Application of AACN Levels of Evidence

Abstract: Evidence-based nursing care is informed by research findings, clinical expertise, and patients' values, and its use can improve patients' outcomes. Use of research evidence in clinical practice is an expected standard of practice for nurses and health care organizations, but numerous barriers exist that create a gap between new knowledge and implementation of that knowledge to improve patient care. To help close that gap, the American Association of Critical-Care Nurses has developed many resources for clinicians, including practice alerts and a hierarchal rating system for levels of evidence. Using the levels of evidence, nurses can determine the strength of research studies, assess the findings, and evaluate the evidence for potential implementation into best practice. Evidence-based nursing care is a lifelong approach to clinical decision making and excellence in practice.

Sex Steroids and Human Behavior: Prenatal Androgen Exposure and Sex‐Typical Play Behavior in Children

Abstract: Gonadal hormones, particularly androgens, direct certain aspects of brain development and exert permanent influences on sex-typical behavior in nonhuman mammals. Androgens also influence human behavioral development, with the most convincing evidence coming from studies of sex-typical play. Girls exposed to unusually high levels of androgens prenatally, because they have the genetic disorder, congenital adrenal hyperplasia (CAH), show increased preferences for toys and activities usually preferred by boys, and for male playmates, and decreased preferences for toys and activities usually preferred by girls. Normal variability in androgen prenatally also has been related to subsequent sex-typed play behavior in girls, and nonhuman primates have been observed to show sex-typed preferences for human toys. These findings suggest that androgen during early development influences childhood play behavior in humans at least in part by altering brain development.

Characteristics of laser diodes for interferometric use.

Abstract: Low-coherence-length light from laser diode sources has applications in extending the useful range of interferometric fiber optic sensors. The characteristics of two commercial low-coherence laser diodes were investigated and compared with theoretical models to determine the operational characteristics of the devices. Reasonable trends in the comparison were seen.
