Abstract: We examined the relationships between the serum levels of chromium and cobalt ions and the inclination angle of the acetabular component and the level of activity in 214 patients implanted with a metal-on-metal resurfacing hip replacement. Each patient had a single resurfacing and no other metal in their body. All serum measurements were performed at a minimum of one year after operation. The inclination of the acetabular component was considered to be steep if the abduction angle was greater than 55°. There were significantly higher levels of metal ions in patients with steeply-inclined components (p = 0.002 for chromium, p = 0.003 for cobalt), but no correlation was found between the level of activity and the concentration of metal ions. A highly significant (p < 0.001) correlation with the arc of cover was found. Arcs of cover of less than 10 mm were correlated with a greater risk of high concentrations of serum metal ions. The arc of coverage was also related to the design of the component and to size as well as to the abduction angle of the acetabular component. Steeply-inclined acetabular components, with abduction angles greater than 55°, combined with a small size of component are likely to give rise to higher serum levels of cobalt and chromium ions. This is probably due to a greater risk of edge-loading.

Abstract: 1. The results of a study of the characteristics of the vessels found in forty-six human femoral heads during the growth period are described. 2. Of the three different sources of blood entering the human adult femoral head it was found that from birth to about three to four years the vessels of the ligamentum teres do not contribute to the nourishment of the head. 3. After the fourth year the metaphysial vessels decrease in importance until they finally disappear, leaving the head with only one source of blood through the lateral epiphysial vessels; the ligamentum teres is not yet contributing to the circulation of the head. 4. After about eight or nine years it was found that the vessels of the ligamentum teres contribute to the blood supply of the head while the metaphysial blood flow is still arrested. 5. Finally, at puberty, after a period of activity of the metaphysial vessels, epiphysial fusion takes place, bringing together the three sources of blood characteristic of the adult.

Abstract: Objective: To describe a practical method of setting personalized but specific goals in rehabilitation that also facilitates the use of goal attainment scaling. Background: Rehabilitation is a complex intervention requiring coordinated actions by a team, a process that depends upon setting interdisciplinary goals that are specific, clear and personal to the patient. Goal setting can take much time and still be vague. A practical and standardized method is needed for being specific. Method: A novel approach to writing specific, measurable, achievable, realistic/ relevant and timed (SMART) goals is developed here. Each goal can be built up by using up to four parts: the target activity, the support needed, quantification of performance and the time period to achieve the desired state. This method can be employed as part of goal attainment scaling and the other levels can be easily and quickly formulated by adding, deleting and/or changing one or more of the (sub)parts. Discussion: The success of goal setting and goal attainment scaling depends on the formulation of the goals. The method described here is a useful tool to standardize the writing of goals in rehabilitation. It saves time and simplifies the construction of goals that are sufficiently specific to be measurable.

Abstract: Animal locomotion typically employs several distinct periodic patterns of leg movements, known as gaits. It has long been observed that most gaits possess a degree of symmetry. Our aim is to draw attention to some remarkable parallels between the generalities of coupled nonlinear oscillators and the observed symmetries of gaits, and to describe how this observation might impose constraints on the general structure of the neural circuits, i.e. central pattern generators, that control locomotion. We compare the symmetries of gaits with the symmetry-breaking oscillation patterns that should be expected in various networks of symmetrically coupled nonlinear oscillators. We discuss the possibility that transitions between gaits may be modeled as symmetry-breaking bifurcations of such oscillator networks. The emphasis is on general model-independent features of such networks, rather than on specific models. Each type of network generates a characteristic set of gait symmetries, so our results may be interpreted as an analysis of the general structure required of a central pattern generator in order to produce the types of gait observed in the natural world. The approach leads to natural hierarchies of gaits, ordered by symmetry, and to natural sequences of gait bifurcations. We briefly discuss how the ideas could be extended to hexapodal gaits.
