Abstract: ContextThe hypothesis that a low-fat dietary pattern can reduce breast cancer risk has existed for decades but has never been tested in a controlled intervention trial.ObjectiveTo assess the effects of undertaking a low-fat dietary pattern on breast cancer incidence.Design and SettingA randomized, controlled, primary prevention trial conducted at 40 US clinical centers from 1993 to 2005.ParticipantsA total of 48 835 postmenopausal women, aged 50 to 79 years, without prior breast cancer, including 18.6% of minority race/ethnicity, were enrolled.InterventionsWomen were randomly assigned to the dietary modification intervention group (40% [n = 19 541]) or the comparison group (60% [n = 29 294]). The intervention was designed to promote dietary change with the goals of reducing intake of total fat to 20% of energy and increasing consumption of vegetables and fruit to at least 5 servings daily and grains to at least 6 servings daily. Comparison group participants were not asked to make dietary changes.Main Outcome MeasureInvasive breast cancer incidence.ResultsDietary fat intake was significantly lower in the dietary modification intervention group compared with the comparison group. The difference between groups in change from baseline for percentage of energy from fat varied from 10.7% at year 1 to 8.1% at year 6. Vegetable and fruit consumption was higher in the intervention group by at least 1 serving per day and a smaller, more transient difference was found for grain consumption. The number of women who developed invasive breast cancer (annualized incidence rate) over the 8.1-year average follow-up period was 655 (0.42%) in the intervention group and 1072 (0.45%) in the comparison group (hazard ratio, 0.91; 95% confidence interval, 0.83-1.01 for the comparison between the 2 groups). Secondary analyses suggest a lower hazard ratio among adherent women, provide greater evidence of risk reduction among women having a high-fat diet at baseline, and suggest a dietary effect that varies by hormone receptor characteristics of the tumor.ConclusionsAmong postmenopausal women, a low-fat dietary pattern did not result in a statistically significant reduction in invasive breast cancer risk over an 8.1-year average follow-up period. Abstract: Distortions of the electronic density of states (DOS) are a potent mechanism to increase the thermopower of thermoelectric semiconductors, thereby increasing their power factor. We review band-structure engineering approaches that have been used to achieve this, resonant impurity levels, dilute Kondo effects, and hybridization effects in strongly correlated electron systems. These can increase the thermoelectric power of metals and semiconductors through two mechanisms: (1) the added density of states increases the thermopower in a nearly temperature-independent way; (2) resonant scattering results in a strong electron energy filtering effect that increases the thermopower at cryogenic temperatures where the electron–phonon interactions are weaker. Electronic structure calculation results for Tl:PbTe and Ti:PbTe are contrasted and identify the origin of the thermopower enhancement in Tl:PbTe. This leads to a discussion of the conditions for DOS distortions to produce thermopower enhancements and illustrates the existence of an optimal degree of delocalization of the impurity states. The experimentally observed resonant levels in several III–V, II–VI, IV–VI and V2-VI3 compound semiconductor systems are reviewed.

Abstract: relapse-free rate (79.7% and 78.9%, respectively, vs. 60.9%; P<0.001 for both comparisons), a lower risk of 3-month sustained progression of disability (hazard ratio for the 3.5-mg group, 0.67; 95% confidence interval [CI], 0.48 to 0.93; P = 0.02; and hazard ratio for the 5.25-mg group, 0.69; 95% CI, 0.49 to 0.96; P = 0.03), and significant reductions in the brain lesion count on magnetic resonance imaging (MRI) (P<0.001 for all comparisons). Adverse events that were more frequent in the cladribine groups included lymphocytopenia (21.6% in the 3.5-mg group and 31.5% in the 5.25-mg group, vs. 1.8%) and herpes zoster (8 patients and 12 patients, respectively, vs. no patients). Conclusions Treatment with cladribine tablets significantly reduced relapse rates, the risk of disability progression, and MRI measures of disease activity at 96 weeks. Abstract: Inter-Vehicle Communication Systems rely on multi-hop broadcast to disseminate information to locations beyond the transmission range of individual nodes. Message dissemination is especially difficult in urban areas crowded with tall buildings because of the line-of-sight problem. In this paper, we propose a new efficient IEEE 802.11 based multi-hop broadcast protocol (UMB) which is designed to address the broadcast storm, hidden node, and reliability problems of multi-hop broadcast in urban areas. Thisprotocol assigns the duty of forwarding and acknowledging broadcast packet to only one vehicle by dividing the road portion inside the transmission range into segments and choosing the vehicle in the furthest non-empty segment without apriori topology information. When there is an intersection in the path of the message dissemination, new directional broadcasts are initiated by the repeaters located at the intersections. We have shown through simulations that our protocol has a very high success rate and efficient channel utilization when compared with other flooding based protocols.

Abstract: Background In patients with human immunodeficiency virus (HIV) infection, combined treatment with several agents may increase the effectiveness of antiviral therapy. We studied the safety and efficacy of saquinavir, an HIV-protease inhibitor, given with one or two nucleoside antiretroviral agents, as compared with the safety and efficacy of a combination of two nucleosides alone. Methods In this double-blind trial, patients with HIV infection were randomly assigned to receive either saquinavir (1800 mg per day) plus both zidovudine (600 mg per day) and zalcitabine (2.25 mg per day) or zidovudine plus either saquinavir or zalcitabine. The 302 patients enrolled had CD4+ counts of 50 to 300 cells per cubic millimeter and had previously received zidovudine for a median of 27 months. The study lasted 24 weeks, with an optional double-blind extension period of an additional 12 to 32 weeks. Results Ninety-six percent of the patients completed the 24-week study.
