Abstract: Disorders of calcium metabolism are not generally considered important either clinically or pathophysiologically in essential hypertension Recent reports, though, suggest that increased parathyroid gland function may be one of the more common endocrine disturbances associated with hypertension We measured serum parathyroid hormone (PTH) concentrations as well as routine blood and urine chemistries in 34 hypertensives Their mean PTH, 791 +/- 31 muliter Eq/muliter, was significantly higher (p less than 0025) than the mean PTH, 669 +/- 33, of an age- and sex-matched normotensive control population The mean serum calcium, 95 +/- 01 mg%, was identical in the two populations Compared to a second age- and sex-matched normotensive population, the hypertensives demonstrated a significant (p less than 0005) relative hypercalciuria For any level of urinary sodium, hypertensives excreted more calcium These preliminary data suggest that parathyroid gland function may be enhanced in essential hypertension This increased gland activity appears, in part, to be an appropriate, physiologic response to a previously unrecognized relative hypercalciuria, or renal calcium leak, associated with essential hypertension We conclude that the increased prevalence of hypertension in subjects with hyperparathyroidism probably represents the final event in a continuum that begins with obligatory urinary calcium losses in hypertensives, but whose pathological presentation is hyperparathyroidism The results of this pilot study indicate a need for derivative experiments directed at defining the importance of our preliminary findings in the pathogenesis of human and experimental hypertension

Abstract: Fundus color photographs and retinal fluorescein angiograms were obtained from 48 nonhuman primates of three macaque species. Yellow pigmentation of the macula was present in monkeys fed a standard laboratory diet containing xanthophylls but was absent in animals maintained on semipurified or liquid formula diets with no xanthophyll content. Plasma levels of xanthophylls ranged from 0.5 to 2.4 microliters/ml in monkeys receiving the standard diet but were undetectable in animals raised on semipurified or liquid formula diets. Fluorescein angiograms revealed foveal areas of hyperfluorescence in almost all monkeys; however, the degree of hyperfluorescence was significantly greater in monkeys maintained on the semipurified or liquid formula diets.

Abstract: In 1978 the authors established a weekly psychiatric clinic for Indochinese refugees. During the first 20 months, 50 patients were evaluated and treated at the clinic; a Vietnamese psychiatric resident and several native Indochinese mental health counselors bridged the language and cultural barriers between patients and clinic personnel. Most of the patients seen at the beginning of the program were psychotic and severely impaired. However, patients seen later suffered from a wider variety of problems. A flexible approach to treatment was adopted that would be compatible with the cultural expectations of the refugees. This resulted in the use of different forms of therapy and special emphasis on the medical approach of the physician, a role familiar to Indochinese patients. Gradually the clinic gained acceptance by members of the local refugee community.

Abstract: The rate of neutralization of sulfuric acid aerosol by ammonia gas has been measured in a laboratory flow reactor for particle diameters between 0.3 and 1.4 micrometers and for relative humidities between 8 and 80%. The rates were between 21 and 70% of the rates expected if diffusion of NH/sub 3/ to the aerosol droplets was the rate-determining factor, and the reaction coefficient, which is the fraction of gas-particle collisions resulting in a chemical reaction, increased with increasing particle size. A simple model of H/sub 2/SO/sub 4/ aerosol generation and neutralization in the atmosphere showed that sulfate aerosol more acidic than NH/sub 4/HSO/sub 4/ should only be present during periods of rapid oxidation of the precursor SO/sub 2/ and high ratios of (SO/sub 2/) to (NH/sub 3/) or when the equilibrium vapor pressure of NH/sub 3/ over the partially neutralized H/sub 2/SO/sub 4/ droplet exceeds the ambient NH/sub 3/ partial pressure.

Abstract: We studied the effects of afterload reduction in chronic severe aortic insufficiency by measuring the hemodynamic response to oral hydralazine in 10 consecutive patients. Hemodynamics were also measured during maximal exercise in eight of these patients. At rest, hydralazine reduced pulmonary artery wedge pressure from 14 to 9 mm Hg (p less than 0.01), and increased cardiac index by 70% and stroke volume index by 35% (both p less than 0.001). Before hydralazine, pulmonary artery wedge pressure exceeded 20 mm Hg in five patients during maximal exercise; with hydralazine, at identical levels of exercise, pulmonary artery wedge pressure remained below 20 mm Hg in all patients. For the group, hydralazine reduced pulmonary artery wedge pressure from 21 to 12 mm Hg (p less than 0.05) and increased cardiac index by 31% (p less than 0.05) during exercise; changes in stroke volume index were more variable and there was no significant increase for the group, although several patients increased stroke volume substantially and the overall increase was 34%. These data show that afterload reduction has beneficial effects on cardiac performance in chronic severe aortic insufficiency both at rest and during exercise. Hydralazine may be of use in such patients either in preparation for valve replacement or as interim therapy.

Nephrotoxicity or adverse effects in patients with renal disease are noted and adjustments for dialysis suggested.

Abstract: • We report here a case of an endotracheal tube fire occurring during carbon dioxide (CO 2 ) laser surgery in the path of gases that support combustion. The tube was thought to be ignited by flaming tissue in close proximity to the tip and not directly by the laser. Tubes 1 cm away from an object repeatedly hit by the laser can easily be ignited indirectly. Aluminum-tape wrapping does not prevent this complication. We recommend caution when using the CO 2 laser in the path of combustible gases in the presence of flammable objects. ( Arch Otolaryngol 106:639-641, 1980)

Abstract: The management of chronic pain is a universal and vexing problem for physicians. Literature indicates that health care providers have a poor understanding of basic concepts relating to pain, which leads to frustration for the physician and inadequate relief for the patient. This paper addresses misconceptions about organic versus functional pain, discusses placebos and administration of narcotic analgesics, and outlines therapeutic alternatives. Emphasis is on distinguishing chronic pain of benign origin from that secondary to malignancy as an individual treatment plan is formulated.

Abstract: The clinical, electrophysiologic, and pathologic features of the peripheral neuropathy in abetalipoproteinemia (Bassen-Kornzweig disease) have been described incompletely. On the basis of findings in threee patients, we believe that large muscle fibers in peripheral nerves are selectively affected and that weakness may be due to chronic partial denervation.

Abstract: Rauscher spleen focus-forming virus (SFFV) was cloned free of its helper virus into normal rat kidney and mouse fibroblasts, and the resulting nonproducer fibroblast clones were analyzed. Our results suggested that Rauscher SFFV encodes a glycoprotein with an apparent Mr of 54,000 (gp54) that reacts with antisera made to the envelope glycoprotein (gp70) of ecotropic murine leukemia viruses, as well as with a rat antiserum that reacts with the gp709s of dual-tropic mink cell focus-inducing and HIX viruses but not with the gp709s of ecotropic viruses. In these respects and in its tryptic peptide map, Rauscher SFFV-encoded gp54 is nearly identical to the gp55 glycoprotein which we previously reported to be encoded by Friend SFFV (Dresler et al., J. Virol. 30:564--575, 1979). However, gp54 is slightly smaller, and it lacks one methionine-containing tryptic peptide that occurs in gp55. Studies with cytotoxic antiserum in the presence of complement and with a rosetting technique which employed sheep erythrocytes coupled to protein A suggested that the gp54 and gp55 glycoproteins are weakly expressed on the surface membranes of SFFV-infected cells. In addition, the Rauscher SFFV genome also encodes gag polyproteins which appear to be identical to the gag polyproteins encoded by helper Rauscher murine leukemia virus, but differ from the antigenically related polyproteins encoded by some but not all clones of Friend SFFV. Furthermore, the glycosylated gag polyproteins encoded by Rauscher SFFV and by some Friend SFFVs also appear to be expressed on the surface membranes of infected cells. Abstract: • Five patients with juvenile chronic granulocytic leukemia are described. Two patients had multiple cafe-au-lait spots compatible with von Recklinghausen's neurofibromatosis, and three had cutaneous xanthomata. Recurrent cutaneous leukemic infiltrates were noted in two patients. The clinical course of all five patients was characterized by recurrent respiratory symptoms and pulmonary infiltrates that responded to antileukemic therapy in three. Abstract: A study was conducted to determine the validity of the MMPI for 142 American Indian patients from Pacific Northwest tribes. All the MMPI profiles were similar and had significant elevations in the Sc, Pd, and Pa scales. There was no significant difference between diagnostic groups on many clinical scales. Nonpsychotic, depressed Indian patients could not be distinguished from schizophrenic patients on any clinical scale, and there were no significant differences between antisocial-alcoholic patients and those with situational reactions. There were minimal differences between the Northwest Coast, Plateau, and Plains cultural groups. The authors believe that the similarity of all subgroup profiles demonstrates a significant cultural influence on the results of the MMPI in this population of American Indians.

Abstract: Herpes simplex is a rare but usually fatal cause of acute hepatitis in adults. Most previously reported cases have occurred in debilitated or immunosuppressed patients. We report two additional fatal cases that occurred in renal transplant recipients. In case 1, there is evidence that the allograft may have been the initial nidus of infection. In case 2, dissemination from a genital infection occurred and failed to respond to vidarabine therapy that was started early in the clinical course. We also review the literature concerning previously reported cases of herpes hepatitis. (Arch Intern Med140:1656-1660, 1980)

Abstract: Nitrosoguanidine mutagenesis of Pseudomonas aeruginosa strain PAO-1 yielded a mutant strain, PAO-PR1, which produced a protein that was immunologically indistinguishable from native toxin A and was nontoxic for cultured Chinese hamster ovary cells. In contrast to native toxin, the cell-associated and extracellular crossreactive material (CRM), designated "CRM protein," possessed no adenosine diphosphate-ribosylating activity. This CRM protein comigrated with native toxin A on sodium dodecyl sulfate/polyacrylamide gels, could be immunoprecipitated with antitoxin from culture supernatants of strain PAO-PR1, and gave a reaction of identity in immunological assays. Equivalent amounts of toxin A antigen and CRM protein antigen were produced in liquid culture by their respective strains as quantitated in a radioimmunoassay for toxin A. These data suggest that mutant strain PAO-PR1 possesses one or more missense mutations within the structural gene for toxin A that adversely affect enzymatic activity, thereby rendering the molecule nontoxic.

Abstract: We have described a heterogeneously processed glycoprotein with an apparent molecular weight of 55,000 (gp55) that is encoded by the Friend spleen focus-forming virus, an acute erythroleukemia virus [Dresler, S., Ruta, M., Murray, M.J. & Kabat, D. (1976) J. Virol. 30, 564-573]. Several lines of evidence suggest that a small proportion of the gp55 in infected cells is located on the surface membranes. First, different nonproducer cell lines infected with cloned Friend spleen focus-forming virus were efficiently killed in the presence of complement by cytotoxic antisera that react with gp55. Furthermore, a clone of cells selected for resistance to the cytotoxic antibody synthesized an altered intracellular form of gp55. This immunoselection procedure appears to also be useful for isolating glycoprotein mutants of other RNA tumor viruses. Analysis of cell surface proteins labeled with [125I]iodine supported the idea that gp55 occurs on the plasma membranes. However, the cell surface gp55 had more highly processed oligosaccharides than the majority of the gp55, which occurs within the infected cells. In addition, we have found that leukemia cells from mice infected with the erythroleukemic Rauscher virus complex contain a membrane glycoprotein that appears similar to the gp55 encoded by Friend spleen focus-forming virus. The encoding of similar glycoproteins by independently isolated acute erythroleukemia viruses suggests that these glycoproteins may be important in leukemogenesis.

Abstract: A 53-year-old male being treated for a metastatic squamous cell carcinoma had fatal pulmonary toxicity as a result of bleomycin therapy. The toxic reaction was observed when the conventional dose regimen of bleomycin was given to the patient who had unrecognized cisplatin-induced acute renal failure. Bleomycin dosage should be reduced in patients with established acute renal failure and the combination of cisplatin and bleomycin should be carefully followed.

Abstract: To begin to define the nature of the biosynthesis and processing of ACTH and beta-endorphin in the human, anterior pituitary tissue (fresh normal and adenomatous, and autopsy) was extracted in acetic acid in the presence of protease inhibitors and subjected to sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The gel slice eluates were assayed for ACTH and beta-endorphin immunoactivity. Human anterior pituitary tissue contained four major size classes of ACTH and three major size classes of beta-endorphin. We found that in all tissue sources examined there was a virtual absence of 13-15K ACTH, which is a major form in the rat and mouse. When comparing extracts obtained from fresh normal or adenomatous anterior pituitary tissue, we also found a drastic decrease in beta-lipotropin and beta-endorphin in extracts of autopsy human anterior pituitaries. These results suggest that the biosynthesis and processing of pituitary ACTH and beta-endorphin in the human may be different than in the mouse, and because of apparent postmortem proteolysis of beta-endorphin, human pituitary obtained at autopsy is probably not a good source of material for biochemical studies of pituitary tissue.

Abstract: Spleen cells from mice primed with the thymus dependent (TD) antigen trinitrophenyl keyhole limpet hemocyanin several months earlier can be stimulated in vitro to produce an IgG anti-hapten response to TD as well as thymus independent (TI) forms of the hapten. Selective killing of TD or TI-2 responding B cells can be accomplished with the corresponding antigen by bromouridine deoxyribose (BUdR) and light treatment without affecting the other population. In contrast, we show here that selective killing with TI-1 antigens does not occur. Rather, the TI-1 antigens, TNP-Brucella abortus or TNP-lipopolysaccharide, eliminate both TD and TI-2 responding IgG memory B cells. All TNP-responding B cells are similarly eliminated if cultures are challenged simultaneously with TD and TI-2 antigens before BUdR and light but not when they are challenged with either a TD or TI-2 antigen separately. We conclude that IgG memory B cell precursors stimulated to produce anti-TNP by TD or TI-2 forms of the hapten are defined by only two functionally distinct subpopulations and that TI-1 antigens can stimulate both of these populations at least to divide.

Abstract: Knowledge in history-taking has increased rapidly over the last twenty years. Currently the principles to be taught include "conduct," "content," and "diagnostic reasoning." However, inattentiveness of medical schools, reluctance of busy faculty to be involved, and increasing enrollments have resulted in difficulties in teaching these skills. Studies have shown a beneficial short-term effect of teaching these materials on interview performance but it is unknown whether this effect is long-lasting. The methods for instruction include the bedside and videotape models utilizing the concept of the fifteen-minute interview technique, programmed instruction, patient instructors, and direct student feedback. Future research should focus on identifying strategies in diagnostic reasoning, developing graduated competency criteria for trainees at different levels of their education, refining methods to evaluate large numbers of students, measuring outcomes of effective training such as compliance, and comparing costs and effectiveness of various methods. In addition, there remains the need to establish an association of course directors.

Abstract: This report describes three sisters, including monozygotic (MZ) twins, with clinical, ultrastructural, and histochemical features typical of yellow mutant albinism; This form of albinism is clinically similar to the tyrosinase-positive type, but hair bulbs showed (1) organelles similar to red hair pheomelanosomes and (2) absence of tyrosinase activity. Classical tyrosinase-negative albinism was found in a maternal cousin of the probands. Pedigree analysis of this family suggests multiple alleles occupying a single locus.

In six inbred Labrador dogs with neonatally-induced thoracic aortic coarctation and in seven littermate controls, serial measurements of forelimb BP, plasma renin activity (PRA), extracellular (ECV), and plasma (PV) volumes (\"Na and '\"RISA spaces), and renal function (clearances of inulin and PAH) were performed during varied steady-state sodium intake over 1-12 months post-aortic-banding (PAB). In coarcted dogs, significant forelimb hypertension by indirect technique [p < 0.001) was confirmed by aorticarch pressures at 1 year PAB (182 ± 8/111 ± 8 mm Hg vs 131 4/78 l, p < 0.001/ < 0.01). Significant increases in ECV (386 ± 4 cc/kg in coarcted, n = 80, vs 371 3cc/kg in controls, n 89, p < 0.01), in PV (49 ± 1 cc/kg, n = 76, vs 47 n = 87, In controls, p < 0.025), and calculated blood volume (p < 0.025) in coarcted dogs averaged 4.1%, 4.1%, and 5.1% over littermate controls and were of similar magnitude during normal, low, and high sodium intake. Since PRA in coarcted animals was comparable to control values at each sodium intake (and thus in excess with reference to volume status), the renin/volume curve was displaced upward relative to that of normotensive controls. The maintenance of normal effective renal plasma flow and glomerular filtration rate in coarcted dogs over the first year PAB also fulfills the steady-state predictions for renin-mediated hypertension and suggests that the renal circulation is a major regulated variable in renin-angiotensin-volume homeostasls. We conclude that, during the first year PAB, neonatally-induced coarctation hypertension is characterized by an abnormal renin/volume relationship demonstrable during varied sodium intake and despite an apparently \"normal\" PRA. While available evidence supports the potential hypertensive impact of this quantitatively small abnormality, its renln-dependence remains to be documented.
