Abstract: The success of military operations depends on soldiers’ execution of the operation as well as resources used for the operation. However, this does not mean that more men and firepower will ensure victory. Military units, just like any other organization, are collections of distributed elements, and improving the organization or command and control (C2) structure of such elements will ultimately show the true power of more men and resources. This paper presents a case study comparing two C2 structures in a counterfire operation, which is a very realistic scenario in some parts of the world. We modeled each structure with meta-networks and agent-based simulations, and then determined why one structure has a better outcome in the simulation. In particular, we jointly analyze the virtual experiment and network metrics, i.e., centralities, to identify the important resources and human factors. This research provides critical insight and suggestions to reform the C2 structure based on quantitative findings. In terms of the C2 structure, assigning detection units to the decentralized echelon brings about the reduction of the time for the targeting process, while the strengthened gun power for multiple targets is proved to have strong influence from the operational perspective.

Abstract: We report the case of a 16-year-old male patient who was involved in a traffic accident and transferred to the emergency department with mild chest pain. We initially did not find evidence of tracheal injury on computed tomography (CT). Within an hour after presentation, the patient developed severe dyspnea and newly developed subcutaneous emphysema and pneumoperitoneum were discovered. Abdominal CT showed no intra-abdominal injury. However, destruction of the right main bronchus was identified on coronal images of the initially performed CT scan. Emergency exploratory surgery was performed. The amputated right main bronchus was identified. End-to-end tracheobronchial anastomosis was performed, and the patient recovered without any complications.

Abstract: Background and objective This study verified the physiological and psychological effects of plant fragrance therapy to analyze whether the therapy has a forest healing effect on maladjusted soldiers. Methods This study was conducted from March 14 to May 31, 2020 according to the Green Camp curriculum for 2 weeks per recruit class. Thus, plant fragrance therapy was implemented twice each for three recruit classes. Each session of the program was carried out for 4 hours from 1 p.m. to 5 p.m. The venue was the garden within the camp and the forest nearby. Forest healing activities using plant fragrances were led by 1 forest education specialist and 1 assistant. The effects of plant fragrance therapy for maladjusted soldiers were analzed analyzed by quantifying physiological and psychological changes through measuring brain waves and stress levels before and after the therapy. Results As a result of analyzing BRQ and SQ to determine the physiological and psychological healing effect of plant fragrance therapy, both the left and right brain showed higher BRQ and SQ after the therapy with statistical significance. This indicates that plant fragrance therapy on maladjusted soldiers helped positively stabilize their negative psychological and behavioral dispositions, and stabilization of brain waves lowered physical and mental stress and improved self-regulation and immunity. On the other hand, there was no significant difference in ATO. This is the result of analyzing the stress-coping mechanism. Troubleshooting (t = −2.702, df = 61, p = .009), emotional remission (t = −2.173, df = 61, p = .034), pursuit of help (t = −3.286), df = 61, p = .002), and wishful thinking (t = −3.714, df = 61, p < .001) showed statistically significant results. Conclusion Applying plant fragrance therapy to maladjusted soldiers positively stabilized their negative psychological and behavioral dispositions and helped their self-regulation. Thus, plant fragrance therapy has psychological and physical forest healing effect on maladjusted soldiers.

All rights reserved. c Purpose: The purpose of this study was to analyze the frequency and patterns of intra-articular lesions detected during ankle fracture surgery using ankle arthroscopy. Materials and Methods: Thirty patients (31 ankles) who underwent open reduction and internal fixation combined with ankle arthroscopy for acute ankle fracture at Inje University Busan Paik Hospital from June 2011 to September 2013 were evaluated. The ankle fractures were classified according to the AO/OTA (AO Foundation and Orthopaedic Trauma Association) classification and the intraarticular injuries were identified by ankle arthroscopy. Osteochondral lesions of the talus were divided into nine subtypes based on their locations, and the ligament injuries were classified according to avulsion fracture and rupture. Results: Using arthroscopy, abnormality in the distal tibiofibular ligament was found in 21 cases and osteochondral lesions and defects of the talus larger than 5 mm were detected in 26 cases. Among ligament injuries, anterior inferior tibio-fibular ligament injury was found in 14 cases, posterior inferior tibio-fibular ligament injury was found in two cases, deep deltoid ligament injury was found in three cases, and deep transverse tibio-fibular ligament injury was found in five cases. The locations of the osteochondral lesions were on the antero-lateral, antero-medial, centro-medial, centro-central, centro-lateral, and postero-lateral talus in 11, one, two, one, two, and nine cases, respectively. Conclusion: With early diagnosis and treatment arthroscopy performed at the time of intra-articular fracture surgery is expected to result in a good outcome.

Abstract: Background Baker's cysts are usually located in the posteromedial side of the knee and seldom cause neuropathy. Case We describe the rare case of a 57-year-old woman with a popliteal cyst who presented with limping gait and pain in her lower leg. She was electronically diagnosed with common peroneal neuropathy and transferred to our pain clinic. On ultrasound examination, about 2.0 × 1.2 cm sized popliteal cyst was found to extend to the fibular head, compressing the common peroneal nerve. Therefore, ultrasound-guided aspiration of the cyst and a common peroneal nerve block were performed. Immediately after the procedure, the pain, dysesthesia, and limping gait were relieved. Although her pain and dysesthesia were relieved, she underwent the surgery because of limping gait. Conclusions In this case, we found the Baker's cyst, the cause of the common peroneal neuropathy, and treated it immediately by just simple ultrasound examination and aspiration.
