Abstract: Research Summary: The Los Angeles County Repeat Offender Prevention Program (ROPP) was designed to improve school performance and prevent reoffending among the enrolled juvenile offenders through a milieu of social services. It was a 3-year pilot project established as part of a statewide juvenile correctional program funded by the California State Legislature, which commenced in 1999 and ended in 2001. A true experimental design was employed, and a total of 327 first-time juvenile probationers were randomly assigned to treatment and control groups. At the end of the project, 204 subjects (106 in the treatment group and 98 in the control group) had a successful program termination. The data show that subjects in the treatment group were more likely to have better school performance than those in the control group. In the first 6-month period after program enrollment, they were also less likely to have new criminal offenses than their counterparts in the control group; however, no significant effect was found in any additional 6-month periods. Both groups were not significantly different in their probation technical violations. Policy Implications: The study results suggest that intensive supervision and coordinated team efforts in providing probation services can markedly improve juvenile offenders' school-related performances and prevent new criminal offenses at the starting period. However, such an intervention model is unlikely to produce consistent and cost-effective justice system effects in the long run. Juveniles who fit the high-risk profiles do not necessarily become chronic offenders; future correctional interventions need to focus not only on individual level factors but also on the social context that gave rise to their problem behaviors in the first place. Abstract: Insulin-like growth factor (IGF) I, a polypeptide synthesized by skeletal cells, and its amino terminus truncated derivative desamino-(l–3)-IGF I (des-IGF I) were compared for their effects on bone formation in vitro. Des-IGF I and IGF I were studied for their effects on DNA and collagen synthesis in cultures of intact fetal rat calvariae and of osteoblast-enriched (Ob) cells from fetal rat parietal bone, and for their ability to bind to IGF receptors in Ob cells and to IGF binding proteins (IGF-BPs) from calvariae. Des-IGF I and IGF I increased [3H] thymidine incorporation into DNA, [3H]proline incorporation into collagen and noncollagen protein, and the mitotic index in intact calvariae. Both factors had similar actions in calvariae. Des-IGF I stimulated all parameters studied at 1 nM, and IGF I was effective on the labeling of DNA at 1 nM, but concentrations of 10 nM were required to observe changes in collagen and noncollagen protein synthesis and in the mitotic index. Abstract: Smoking continues to be a leading cause of preventable deaths and rates of trying cigarettes and progression to daily smoking among adolescents continues to remain high. A plethora of risk factors for smoking among adolescents has been addressed in the research literature. One that is gaining particular interest is the relationship between adolescent mental health and smoking (both initiation and progression). This paper reviews the evidence for adolescent mental health as a risk factor for cigarette smoking. We focus on the specific mental health conditions that have been more thoroughly addressed as possible risk factors in community-dwelling adolescents. We discuss the multiple hypotheses that have been posited as to the nature of the relationship between adolescent mental health and smoking, as well as detailing so called third factors that may account for the observed relationship. We highlight the contribution of the existing studies to the body of knowledge on this topic, as well as the limitations and open questions that remain as a result. We conclude with discussion of a broad research agenda going forward.

Abstract: In a retrospective analysis, we evaluated the safety and efficacy of peripheral nerve blocks (PNB) compared to epidural anesthesia in 221 consecutive patients undergoing same-day bilateral total knee arthroplasty (TKA). Primary outcome measures included: hypotension requiring physician intervention, number of blood transfusions, perioperative hespan and crystalloid consumption, incidences of respiratory desaturation, pruritis, urinary retention, and nausea/vomiting. The incidences of hypotension, urinary retention, and pruritis were all higher in the epidural group, compared to PNB. Epidural patients also required more blood transfusions and greater volumes of hespan and crystalloid. PNB are safe and efficacious modality of analgesia for same day bilateral TKA and provide adequate pain relief with a significant decrease in postoperative complications compared to epidural anesthesia.
