Abstract: Aortoesophageal fistula (AEF) is a rare cause of massive upper gastrointestinal hemorrhage. Thoracic aortic aneurysm, esophageal foreign body, esophageal cancer and post-surgical complications are common causes of AEF; however, AEF induced by radiation therapy is a rare phenomenon and seldom described in the literature. It is a catastrophic condition which requires rapid implementation of resuscitative measures, broad-spectrum antibiotics and surgical or endovascular intervention. Transthoracic endovascular aortic repair (TEVAR) is a newer and less invasive technique, which helps to achieve rapid hemostasis in patients with severe hemodynamic instability and offers advantages over conventional repair of the aorta in emergency situations. However initial TEVAR should be followed up with a more definitive surgical repair of the aorta and the esophagus, to lower the mortality rate and achieve better outcomes. We describe here a case of a seventy-year-old male who presented with massive upper gastrointestinal bleeding due to AEF induced by radiation therapy, and his subsequent successful initial management with TEVAR.

Abstract: Although lung cancer is the leading cause of cancer death in the USA, there have been few studies on patient-centered advanced lung cancer treatment practices. As part of a larger research study on how to use a patient-inclusive approach in late-stage lung cancer treatment, this present study describes patient, caregiver, and provider perspectives on the role of the health care system in helping patients cope with an advanced stage lung cancer diagnosis. Four focus group sessions were conducted with six to eleven participants per group for a total of 36 participants. Two focus groups were held with patients and family members/caregivers and two with physicians and nurses. A major theme that emerged concerned coping with an advanced lung cancer diagnosis, which is the subject of this paper. The patients, caregivers, and providers spoke passionately about interactions with the health care system and volunteered examples of supportive and non-supportive relationships between patients and clinicians. They advocated for better patient-provider communication practices as well as the expanded use of patient navigation and new patient orientation programs. This study contributes additional knowledge by including the perspectives of caregivers and providers who live and work closely with patients with advanced lung cancer. The findings can inform the development of comprehensive patient-centered care plans for patients living with an advanced lung cancer diagnosis.

Abstract: Background: The mammalian target of rapamycin (mTOR) pathway inhibition has emerged as one of the main directions for the development of new targeted agents in renal cell carcinoma (RCC). A prominent member in its class of medications, temsirolimus has already been shown to improve overall survival in advanced kidney cancer, when compared with the previous standard, IFN-α. Objective: The aim of this study was to review the most relevant preclinical and clinical data on the mTOR inhibitors, both in clinical use or in current development. Methods: The authors give a comprehensive review of the existing English literature on the role of the mTOR pathway in renal tumorigenesis, as well as a detailed safety and efficacy analysis of older and newer rapamycin analogs. Results/conclusions: Rapamycin derivatives temsirolimus and everolimus have significant clinical activity in patients with advanced-stage RCC. Abstract: Several intermediates for the CH(3)SH + OH(*) --> CH(3)S(*) + H(2)O reaction were identified using MP2(full) 6-311+g(2df,p) ab initio calculations. An adduct, CH(3)S(H)OH(*), I, with electronic energy 13.63 kJ mol(-1) lower than the reactants, and a transition state, II(double dagger), located 5.14 kJ mol(-1) above I, are identified as the entrance channel for an addition-elimination reaction mechanism. After adding zero-point and thermal energies, DeltaH(r,298) ( degrees )(reactants --> I) = -4.85 kJ mol(-1) and DeltaH(298) (double dagger)(I --> II(double dagger)) = +0.10 kJ mol(-1), which indicates that the potential energy surface is broad and flat near the transition state. The calculated imaginary vibrational frequency of the transition state, 62i cm(-1), is also consistent with an addition-elimination mechanism. These calculations are consistent with experimental observations of the OH(*) + CH(3)SH reaction that favored an addition-elimination mechanism rather than direct hydrogen atom abstraction. An alternative reaction, CH(3)SH + OH(*) --> CH(3)SOH + H(*), with DeltaH(r,298) ( degrees ) = +56.94 kJ mol(-1) was also studied, leading to a determination of DeltaH(f,298) ( degrees )(CH(3)SOH) = -149.8 kJ mol(-1).
