Abstract: The HIV/AIDS epidemic is a dynamic unstable global phenomenon, constituting a veritable mosaic of regional sub-epidemics. As a consequence of the deep inequalities that exist in Brazilian society, the spread of HIV infection has revealed an epidemic of multiple dimensions undergoing extensive epidemiological transformations. Initially restricted to large urban centers and markedly masculine, the HIV/AIDS epidemic is currently characterized by heterosexualization, feminization, interiorization and pauperization. The evolution of the profile of AIDS in Brazil is above all due to the geographical diffusion of the disease from large urban centers towards medium and small municipalities in the interior, to the increase in heterosexual transmission and the persistent growth of cases among injecting drug users. The increase in transmission through heterosexual contact has resulted in substantial growth of cases among women, which has been pointed out as the most important characteristic of the epidemic's current dynamic in Brazil.

Abstract: The purpose of this retrospective study was to analyse data from the records of patients seen in the dental trauma emergency clinic in a general hospital in the city of Recife, Brazil, during the years 1997-1999, according to sex, age, cause, number of injured teeth, type of tooth and type of trauma. The records of all patients seen by dentists were collected. Altogether, 250 patients from 1 to 59 years of age presenting 403 dental injuries were examined and/or treated. The causes of dento-alveolar trauma were classified in five categories: home injuries, street injuries, school injuries, sports activities, violence. The type of trauma was classified by dentists working at the dental trauma clinic on the basis of Andreasen's classification. The gender difference in the number of cases of trauma was statistically significant (males 63.2% vs females 36.8) (P<0.0001). Fracture in enamel only (51.6%) and fractures in dentine (40.8%) were the most commonly occurring types of injury. Injuries were most frequently diagnosed as serious among the youngest patients (up to 15 years of age); 82.4% of intrusive luxation cases were diagnosed in the 1-5 years age group. The main causes of tooth injury were falls (72.4%), collisions with objects (9.2%), violence (8.0%), traffic accidents (6.8%) and sports (3.6%). Trauma caused by violence was found to be statistically significant in the 6-15 years age group (P<0.0005).

Abstract: Pro-inflammatory cytokines are believed to play an important role in the pathogenesis of dengue infection. This study reports cytokine levels in a total of 54 patients examined in Recife, State of Pernambuco, Brazil. Five out of eight patients who had hemorrhagic manifestations presented tumor necrosis factor-alpha (TNF-alpha) levels in sera which were statistically higher than those recorded for controls. In contrast, only one out of 16 patients with mild manifestations had elevated TNF-alpha levels. The levels of interleukin-6 (IL), IL-1beta tested in 24 samples and IL-12 in 30 samples were not significantly increased. Interferon-g was present in 10 out of 30 patients with dengue. The data support the concept that the increased level of TNF-alpha is related to the severity of the disease. Soluble TNF receptor p75 was found in most patients but it is unlikely to be related to severity since it was found with an equivalent frequency and levels in 15 patients with dengue fever and another 15 with dengue hemorrhagic fever.

Abstract: We studied the ability of ATP to inhibit in vitro the degrading activity of insulin-degrading enzyme. The enzyme was purified from rat skeletal muscle by successive chromatographic steps. The last purification step showed two bands at 110 and 60 kDa in polyacrylamide gel. The enzyme was characterized by its insulin degradation activity, the substrate competition of unlabeled to labeled insulin, the profile of enzyme inhibitors, and the recognition by a specific antibody. One to 5 mM ATP induced a dose-dependent inhibition of insulin degradation (determined by trichloroacetic acid precipitation and insulin antibody binding). Inhibition by 3 mM adenosine 5'-diphosphate, adenosine 5'-monophosphate, guanosine 5'-triphosphate, pyrophosphate, beta-gamma-methyleneadenosine 5'-triphosphate, adenosine 5'-O-(3 thiotriphosphate), and dibutiryl cyclic adenosine 5'-monophosphate was 74%, 4%, 38%, 46%, 65%, 36%, and 0%, respectively, of that produced by 3 mM ATP. Kinetic analysis of ATP inhibition suggested an allosteric effect as the plot of 1/v (insulin degradation) versus ATP concentration was not linear and the Hill coefficient was more than 1 (1.51 and 2.44). The binding constant for allosteric inhibition was KiT = 1.5 x 10(-7) M showing a decrease of enzyme affinity induced by ATP. We conclude that ATP has an inhibitory effect on the insulin degradation activity of the enzyme.

Abstract: OBJECTIVE: To analyze the magnitude and trends of mortality due to external causes in specific age groups, from 0 to 19 years old in a population living in the city of Recife, Brazil, from 1979 to 1995. METHODS: Data from the Mortality Data System of the Ministry of Health and Health Department of Pernambuco State was used. The studied population, aged 0 to 19 years old, represented 41.8% of total city population in 1991. A time series exploratory ecological model was created to analyze trends in mortality coefficients due to external causes. Using simple linear regression these coefficients were assessed in specific age groups categorized by gender. RESULTS AND CONCLUSIONS: In the time series, mortality coefficients due to external causes showed an increasing trend, in particular due to homicides among adolescents, where the coefficient increased on average 3.05 per year, yielding a relative increase of 601, 3% over the study period. In 1995, more than 90% of these homicides were perpetrated with fire weapons. These data reinforce the seriousness of this problem and the need to deal with it, taking into account the difficulties in determining the causes of violence.

Os resultados demonstraram a necessidade de trabalho educativo como forma de prevencao da gravidez indesejada.

Abstract: This article develops an original framework supporting health policy analysis that may be carried out by distinct research on various situations. The Walt and Gilson model for health policy analysis and its categories: Context, Content, Actors and Process, was taken as the basic framework of analysis. However, to be applied in practice that model needs to be unravelled. Its four major categories need to be operationalised and transformed into a matrix, with each one revealling the elements or sub-categories, and the necessary procedures to conduct a systematic analysis on the Context, Content, Actors and Process of a given health policy. Such an initiative was treated in this work.

Abstract: Upper Precambrian-Lower Cambrian sequences in the Tucuman, Salta and Jujuy provinces, NW Argentina, comprise sandstone, slate, conglomerate and black limestone (Las Tienditas/Volcan Fms.) with abundant Vendian/Tommotian trace fossils in the clastic facies rocks. The Precordillera basin, San Juan province, represents a continuous carbonate sequence belonging to the Lower to Middle Cambrian La Laja Fm. The Pie-de-Palo Range, Pampean Range, characterized by carbonates intercalated with greenschist/amphibolite facies metaclastic rocks, forms a part of the Precordillera basement. The δ13C values in carbonates of the Las Tienditas Fm. show a gradual decrease from the base (+3.4 ‰PDB) to the top with a minimum of −1.6‰ observed at ∼15m from the top, the latter having a higher clay content. Carbonates in a 700m thick section within La Laja Fm. is marked by a slightly positive δ13C values at the base (marly) with a negative anomaly (−2.0‰) at ∼20 m above, followed by a small positive anomaly (+0.5‰) ∼100 m from the base. All the values above this point are around −0.5‰ with a negative anomaly (−2.0‰) recorded at ∼240m above the base. Seawater87Sr/86Sr values define a non-monotonic increase (0.70870–0.71082) through the carbonates in Las Tienditas Fm. while the La Laja Fm carbonates vary from 0.70926 to 0.71030, with higher values at the base. C and Sr isotopes, thus suggest that the Las Tienditas carbonates record the Precambrian-Cambrian transition (∼15m from the top of studied section). The same is also evident at ∼30 m from the base of the La Laja Fm. The narrow range of δ13C variation (−1.4 to +1.3‰) and87Sr/86Sr in the 0.709–0.710 range for the Caucete Group carbonates of the Pie-de-Palo Range, although unequivocally, appear to be in consonance with a Vendian to Tommotian age, reinforced by the presence of the trace fossils Didymaulichnus and Gordia.

Abstract: Objetivo. Validar a causa basica da morte nas declaracoes de obito por causas externas de menores de 20 anos residentes em Recife (PE), Brasil, em 1995. Metodos. O estudo foi dividido em duas etapas: codificacao e validacao. Nas duas etapas, os dados oficiais sobre causas de morte foram comparados aos dados obtidos na pesquisa. Para tanto, as declaracoes de obito foram agrupadas em 5 categorias principais e subdivididas em 14 categorias secundarias de causa da morte; foram, tambem, comparadas individualmente ate o quarto digito da classificacao suplementar para causas de morte da nona revisao da Classificacao Internacional de Doencas. Analisou-se a concordância entre os dados oficiais e os dados do estudo atraves do indice kappa e da sensibilidade. A categorizacao da pesquisa foi tomada como padrao. Resultados. Na etapa de codificacao, para o total das causas externas, o percentual global de concordância variou de 94% (cinco categorias) para 92% (14 categorias), caindo, por fim, para 81% (comparacao ate o quarto digito). Na etapa de validacao, o percentual global de concordância variou de 94% (cinco categorias) para 91% (14 categorias) e 73% (comparacao ate o quarto digito). Conclusoes. Os resultados revelam que, para obtencao de dados mais fidedignos, e necessario que as declaracoes de obito sejam preenchidas pelo Instituto de Medicina Legal dentro de padroes recomendados; por outro lado, as guias de remocao de cadaveres precisam ser melhor preenchidas em hospitais e delegacias quando do encaminhamento ao Instituto. E fundamental gerar e divulgar dados mais precisos para que a sociedade conheca melhor o seu padrao de violencia.

Abstract: Com o objetivo de avaliar o desempenho das agulhas de calibres 14 e 12 na obtencao de "core biopsies" da mama sob controle estereotaxico, os autores compararam biopsias de lesoes mamarias impalpaveis obtidas com agulhas desses calibres quanto ao tamanho dos fragmentos e frequencia dos tipos de lesoes diagnosticadas. De um total de 1.009 biopsias, os autores analisaram 900 obtidas por agulhas 14 e 109 obtidas por agulhas 12, para analise da frequencia das lesoes. Os tamanhos dos fragmentos foram analisados nos 700 primeiros casos para os especimes obtidos por agulhas 14. A utilizacao de agulhas 12 resultou em maior tamanho dos fragmentos (p < 0,001), ausencia de material insatisfatorio, reducao na proporcao das lesoes benignas nao-neoplasicas (p = 0,03) e aumento na frequencia de carcinomas (p < 0,001), tanto in situ como invasivos. Tais achados podem refletir aumento da acuracia do diagnostico, em virtude da maior quantidade de material e/ou melhor selecao radiologica das biopsias obtidas por agulhas 12, pela utilizacao coincidente da mamografia digital em campo limitado (estereotaxia digital).

Abstract: OBJECTIVES: to discuss the experience, conception and development of a simplified model to evaluate the nutritional status and correlated health situation of mothers and children using the "national vaccination day" as strategy, when 90% of children and 80% of mothers appear at the vaccination center. METHODS: five health units of vaccination were observed in a town from the coastal area (Zona da Mata) of Pernambuco. The sample size was estimated considering a prevalence error from 3% to 5% in relation to the basic variables of interest, such as, prevalence of malnutrition, anemia, diarrhea, acute respiratory infection, breastfeeding, monitoring of growth and development and vaccine situation, and four aspects from mothers: body mass index, prevalence of anemia, antenatal care and educational level. RESULTS: the study involved a sample of 579 children and 417 mothers. Among the mothers, 80,3% were literate. 18,0% of the children had had acute respiratory infection in the last week, while only 7,4% of those who were from six to 12 months had an incomplete vaccine situation in relation to Sabin and DPT. 37,8% were anemic and 11% showed stature deficit. The mean and the median of exclusive breastfeeding were 11,5 and 9,0 days, respectively. We observed twice as many obese women as thin women (10,6%; 4,9% respectively), and 16,2% were anemic. CONCLUSIONS: the results were similar to the expected values, compared with previous epidemiological studies that took place in Pernambuco state, which showed the validity of the simplified model in Brazil and others countries that use a "national vaccination day" as a strategy.

Abstract: BACKGROUND AND AIM OF THE STUDY Aprotinin is effective in promoting hemostasis, notably in cardiac surgery with cardiopulmonary bypass. Its efficacy has been shown in coronary bypass graft operations. However, few reports exist of aprotinin use in valve operations, and in those studies only the full dose was used. Thus, our aim was to evaluate the effects of low-dose aprotinin in patients undergoing heart valve reoperation. METHODS Eighteen patients having reoperative valve surgery received 10(6) KIU aprotinin after induction of anesthesia, and a further 10(6) KIU in the pump prime. A group of 18 similar patients who were operated on but did not receive aprotinin were used as controls. RESULTS A significant reduction in postoperative blood loss (approximately 470 ml) occurred in patients receiving aprotinin. These patients also presented less postoperative bleeding than untreated patients in 70.4% of cases. No adverse effects of the drug were noted, except for one case of allergic reaction. CONCLUSION The systematic use of low-dose aprotinin should be considered in valve reoperation, except in cases of re-exposure to the drug, or allergic reaction.

Abstract: We report on a patient presenting Pick's disease similar to the one reported by Pick in 1892, with ubiquitin-positive and tau-negative inclusions. His diagnosis was made on the basis of clinical (language disturbance and behavioural disorders), neuropsychological (progressive aphasia of the expression type and late mutism), neuroimaging with magnetic resonance (bilateral frontal and temporal lobes atrophy) and brain single photon emission computed tomography (frontal and temporal lobes hypoperfusion) studies. Macroscopic examination showed atrophy on the frontal and temporal lobes. The left hippocampus displayed a major circumscribed atrophy. The diagnostic confirmation was made by the neuropathological findings of the autopsy that showed neuronal loss with gliosis of the adjacent white matter and apearence of status spongiosus in the middle frontal and especially in the upper temporal lobes. There were also neuronal swelling (ballooned cell) and argyrophilic inclusions (Pick's bodies) in the left and right hippocampi. Anti-ubiquitin reaction tested positive and anti-tau tested negative.

Abstract: Summary. In a genome survey for Alzheimer’s disease (AD), Zubenko et al. (1998) reported that the 234 bp allele of the D10S1423 locus was more frequent among AD cases than in controls. We have analyzed this polymorphic locus in patients and healthy controls and observed that the 226 bp allele is the most frequent allele in the D10S1423 locus in Brazilian AD patients. However, no statistically significant association between any D10S1423 allele was observed in AD patients as well as in controls.

Abstract: The paper presents a multicriteria decision approach to select modules of an information system. The decision maker's preferences indicate priorities. These preferences are modelled via a multicriteria method, named PROMETHEE, which deals with criteria such as: impacts on strategic factors and operational aspects of processes. An analytical approach has been achieved to assign priorities for information modules. The paper includes the methodology and a case study with numerical results, including the sensitivity analysis to classify the modules of information systems. The approach can be applied in a planning stage or on a management basis. An analytical structure is proposed to incorporated the multicriteria PROMETHEE method integrated with the information system planning methodology. Strategic factors and operational aspects of the processes are integrated. A multicriteria decision model is obtained. A numerical application based on a real case study illustrates the use of the model. The approach was applied in a planning stage of power electric company. Practical results obtained show that the decision makers can have a better evaluation of information system priority analysing strategies and process operational aspects when using these modules of information system.

Abstract: There are compelling reasons to combine the concepts of an architecture description language with those of an object-oriented framework to provide support for the development of reusable and extensible software architectures. This paper presents C++CL, an object-oriented framework for developing reconfigurable distributed software architectures, which follows that approach. The reconfigurability of C++CL enables us to develop distributed systems, which require changes to take place during run time without having to shut the system down. Examples of systems of this kind are evident in banking, industrial control and telecommunications and are becoming more common in many other areas. The main focus of the paper is to show how C++CL supports re-usability and the evolution of distributed systems through the use of inheritance and the modelling of architectural styles. Inheritance can be applied not ony to the components of an architecture but to the description of the architectural configuration itself. An architectural style, similarly to a pattern, defines an architecture-based solution to a family of problems. The combination of those two concepts provides powerful support for re-usability of architecture descriptions.

This retrospective study aimed at finding out the epidemiological profile of the adolescents admitted at the Cleriston Andrade General Hospital, in Feira de Santana, Bahia, that were submitted to a curettage from January 1995 to December 1997. The results showed the need for educative programs in order to prevent unplanned pregnancies.

Abstract: Purpose: to evaluate the results of the first 104 hysteroscopic polypectomies in a teaching hospital. Methods: a retrospective descriptive study was designed. Medical records of the first 136 operative hysteroscopies - 104 of which polypectomies - were reviewed. Patient characteristics such as age, parity, period of reproductive function and symptoms; number and size of polyps and results concerning complications and symptom relief were evaluated. Results: the average age of patients was 52.7 years. Three quarters of them were multiparous. Fifty-seven percent of the patients were menopaused. About half of the patients had symptoms related to polyps. Abnormal bleeding was the most frequent symptom (47.1%). In 16.3% of the patients more than 1 polyp were detected and 84% of the polyps were larger than 1 cm. The only immediate complication was a uterine perforation. Late complications were rare and mild. The follow-up period was 9 months on average. In 82% of teh patients the symptoms were controlled. Hysterectomy was necessary in 8.2% of the patients, all of them with other uterine diseases such as leiomyomas, adenomyosis and atypical endometrial hyperplasia in one patient). Conclusion: hysteroscopic polypectomy is a simple, safe and effective method for the treatment of endometrial polyps. Selection of patients must be rigorous to avoid further operative procedures.

Abstract: The paper deals with a marketing dispute for the future sale of electric energy among companies that generate energy and a company that distributes energy in the new private sector of Brazilian electric power system. Privatization of this sector in Brazil is based on the principle of implementation of competition wherever is possible, and regulation whenever necessary. The price and availability of energy to be offered are considered as a criteria or attributes for the analysis and selection. The ELECTRE I and II methods were used. These are methods of the multicriteria decision-aid field based on the dominance relation. Results of both methods were similar: they favored the companies with lowest prices but with reasonable availability indexes. The decision model formulation is presented, including the preferred modeling of the decision maker. The case study analyzed includes numerical results with recommendations and a sensitivity analysis of the model. This case study is based on a real context of a power electric company.
