Abstract: The number of reported pertussis has increased in the last two decades. However, many cases of pertussis may be underreported or not diagnosed. The World Health Organization estimates that pertussis causes 200.000 – 400.000 deaths each year, most deaths are in infants and in developing countries. Infants with pertussis can indicate an undetected source cases in the community. At a University Hospital in Brazil individuals that had frequent contacts with a child with confirmed pertussis (the index case) and had recent history of cough were enrolled into the study. Nasopharyngeal swabs were collected from every contact that had cough within the last 21 days. Cases confirmation followed the guidelines of the Center for Disease Control and Prevention – Atlanta, U.S.A. Pertussis diagnosis was confirmed in 51 children, (considered the index cases). Among the index cases, 72.5% (37/51) were under 6 months of age; culture for Bordetella pertussis was positive in 78.4% (40/51). Pertussis was confirmed in 39% (107/276) of the contacts of 51 index cases. Among these contacts identified as a pertussis case, 40.2% (43/107) were between 6 months and 111/2 years of age and 59.8% (64/107) were older than 111/2 years of age. Pertussis was confirmed by culture in 11.2% (12/107) of them and by epidemiologic linkage in 88.8% (95/107). Each index case allowed identifying two new cases of pertussis. Public health authorities should consider implementing early recognition of pertussis index cases and searching for pertussis cases among the contacts. Treatment of the cases and prophylaxis of the contacts is fundamental to control outbreaks in the community.

Abstract: We describe a 47-year-old woman with a 3-year history of a continuum mild-moderate right-side headache, with exacerbations, associated with stabbing volleys of pain on right orbit-temporal region (10/10) and right eye ptosis and lacrimation with conjunctival injection. The pain was completely abolished with indomethacin (100 mg per day). The diagnosis of hemicrania continua was made according to the International Headache Society (IHS) criteria. The headache presentation was precipitated by a stroke and a right-side brainstem lesion was present at magnetic resonance imaging. This case report shows anatomoclinical evidence of the involvement of brainstem structures on the pathophysiology of hemicrania continua.

Abstract: FUNDAMENTO: Otimizacao da veia safena na revascularizacao miocardica. OBJETIVO: Apresentar a tecnica no-touch de preparo da veia safena. Essa tecnica consiste na retirada da veia safena do seu leito, com um pediculo de tecido adiposo, protegendo-a contra espasmos, sendo desnecessario distende-la. METODOS: Estudo prospectivo e randomizado, incluindo 156 pacientes submetidos a cirurgia de revascularizacao miocardica. Comparacao da tecnica no-touch com duas outras tecnicas: convencional e intermediaria. Procedeu-se a avaliacao da morfologia endotelial, utilizando a microscopia. A perviabilidade das pontes foi determinada com exame angiografico num periodo medio de 18 meses apos a operacao. A enzima oxido nitrico sintetase endotelial (eNOS) foi identificada por meio do estudo imunohistoquimico. RESULTADOS: A avaliacao morfologica mostrou integridade endotelial de 97% nas veias do grupo no-touch; enquanto quase metade da superficie endotelial das veias tratadas pelas outras tecnicas exibiu ausencia de celulas endoteliais. A angiografia revelou perviabilidade de 95,4% para as pontes do grupo no-touch, 88,9 e 86,2% para as pontes do grupo convencional e intermediaria, respectivamente. O estudo imunohistoquimico revelou a presenca da eNOS nas tres camadas que compoem a parede da veia no grupo no-touch e reducao dessa enzima no grupo convencional. CONCLUSAO: A integridade endotelial e a atividade da eNOS foram melhor preservadas com o uso da tecnica no-touch. A protecao mecânica fornecida pelo tecido gorduroso circundante a veia e a atividade vasodilatadora e bloqueadora da agregacao plaquetaria causada pelo oxido nitrico podem ser responsaveis pela protecao da veia contra o espasmo, como tambem por sua alta perviabilidade imediata.

Abstract: The assessment of optical networks considering physical impairments is frequently accomplished by using time-consuming analysis tools.We propose in this paper to use artificial neural networks to predict the blocking probability of optical networks with dynamic traffic by using topological metrics and general information of the physical layer. The training process is accomplished by supervised learning based on a historical database of networks. We also propose a new and simple topological property to represent the capacity of the network to distribute traffic. From the results, we found that this novel topological property improves the estimator accuracy. We compared the results of our proposal with the outcome of a discrete event simulator for optical networks. The simulator provides an estimate for blocking probability of alloptical networks considering physical impairments. We show that our approach is faster than discrete event simulators; we obtained a speedup of greater than 7500 times, with comparable estimation errors.

Abstract: Introduction As population ageing becomes a global phenomenon the need to understand the quality of life of older people around the world has become increasingly salient. The CASP-19 is a well established measure of quality of later life. The scale is composed of 19 items which map onto the four domains of control (C), Autonomy (A), Self-Realisation (S) and Pleasure (P). It has already been translated to 12 languages and has been used in a number of national and international studies. However use of the scale outside of Europe has been very limited. The objective of this study was to translate and evaluate the use of the CASP-19 amongst older Brazilians.
