Abstract: Resumo: Este artigo objetivou compreender percepcoes e praticas relativas ao acolhimento no cuidado puerperal a mulher. Ele e parte de pesquisa qualitativa mais ampla, fundamentada em Gadamer, efetuada de outubro de 2012 a setembro de 2013, em Recife, Pernambuco, Brasil, com observacao participante e entrevista semiestruturada. Participaram equipe de saude da familia (medica, enfermeira e cinco agentes de saude) e dez mulheres (18 anos e mais, ultimo trimestre da gestacao, pre-natal com a equipe). As categorias empiricas foram: acesso ao cuidado e demandas e necessidades de saude. Os resultados destacam insatisfacao com a visita domiciliar: tardia, sem priorizacao, medica/enfermeira ausente, assistematica e descontinuada; falta de consulta puerperal e acesso dificil ao medico. Ha desagrado com a desvalorizacao das necessidades de saude femininas: atencao focada no bebe, escassez de exame fisico e anamnese, orientacoes insuficientes e comunicacao limitada. A incipiencia do acolhimento mostra a necessidade de transformar as praticas da equipe para conferir visibilidade a mulher, qualificando o cuidado puerperal.

Abstract: Objective: To report the incidence of postoperative mediastinitis in cardiovascular surgery. Methods: The medical records of all 1038 patients who underwent cardiovascular surgical procedures between May/ 2007 and June/2009 were reviewed. All operations were performed at the Division of Cardiovascular Surgery of Cardiac Emergency Hospital of Pernambuco – PROCAPE. Results: On average, complication occurred within 13 days after operation; in a total of 25 lethal cases (2.4%), deaths occurred in eight (32%). Several risk factors for mediastinitis were identified as follows: 56% diabetes, 56% smoking, 20% obesity, 16% chronic obstructive pulmonary disease, and 8% end-stage renal disease. Mediastinitis was reported in 21 (84%) patients submitted to coronary artery bypass grafting and it was related to a major risk for development of infection (IC 3.44-8.30, P=0.0001). High rates of complications were observed: respiratory failure (44%), stroke (16%), cardiogenic shock (12%), acute renal failure (28%), pulmonary infection (36%), multiple organs failure (16%), and sternal dehiscence (48%). Bacterial cultures of exudates were positive in 84% of patients; Staphylococcus aureus was the most frequently pathogen (28.8%) detected. Conclusion: Mediastinitis remains as a severe surgical complication and difficult to manage in postoperative cardiovascular surgery. The disease has low incidence rate but high lethality. Coronary bypass was associated to a major risk for development of infection.

Abstract: The purpose of this study was to investigate the tracking of gross motor coordination (GMC) and to profile children at 6 years of age who consistently showed higher stability patterns in different levels of GMC. The participants were 245 children (123 boys and 122 girls) who were assessed longitudinally from 6 to 9 years of age. GMC was assessed using the Korperkoordinationtest fur Kinder (KTK) test battery. Anthropometry, physical activity, and health- and performance-related physical fitness were also measured. Cohen’s kappa (κ) was used to estimate tracking. Tracking was poor for all GMC tests (0.17 ≤ κ ≤ 0.38) and moderate for the GMC motor quotient (MQ) in both boys and girls (0.44 ≤ κ ≤ 0.45). Instability at the extremes was low in GMC tests and negligible for MQ. Children who consistently showed high GMC levels during the 4 years of follow-up were lighter, had lower body mass index and subcutaneous fat, and showed higher scores in physical fitness tests at 6 years of age than those who cons...

Abstract: Particle Swarm Optimization (PSO) has been widely used to solve many different real world optimization problems. Many novel PSO approaches have been proposed to improve the PSO performance. Recently, a communication topology based on Clans was proposed. In this paper, we propose the Dynamic Clan PSO topology. In this approach, a novel ability is included in the Clan Topology, named migration process. The goal is to improve the PSO degree of convergence focusing on the distribution of the particles in the search space. A comparison with the Original Clan topology and other well known topologies was performed and our results in five benchmark functions have shown that the changes can provide better results, except for the Rastrigin function.

Abstract: Objective: The present study consists of a double-blind randomized clinical trial of paired samples. The purpose of the present study was thus to examine the effect of four different local anesthetics of the amida group (2% lidocaine with 1:100.000 adrenaline; 3% prilocaine with 0.30 IU felipressine; 2% mepivacaine with 1:100.000 adrenaline; and 4% articaine with 1:100.100 adrenaline) in patients undergoing extraction of lower third molars and verify the changes in systolic, diastolic and mean blood pressures, heart rate (HR) and oxygen saturation (SpO 2 ). Study design: The sample comprised 168 extractions of lower third molars performed on 84 patients, divided into three groups, in whom lidocaine was the control anesthetic. The anesthetic solution applied differed from one side of the mouth to the other (control and experimental) in the same patient at different time periods. Results: The following significant variations were observed: increase in systolic blood pressure with mepivacaine and articaine; decrease in diastolic blood pressure with lidocaine; increase in heart rate with all the anesthetics, but with no statistical significance in the case of prilocaine. The variations in mean blood pressure and oxygen saturation were not statistically significant. All the hemodynamic changes returned to normal with no need for any further treatment. No complications were observed in any of the 168 procedures performed. Conclusion: All the anesthetics studied behaved, in relation to lidocaine, within the parameters of hemodynamic safety, although the best performance was that of prilocaine, followed by lidocaine. The measurement of systolic blood pressure, diastolic blood pressure and heart rate are appropriate monitoring methods for patients under the effects of local anesthetic, even healthy ones.
