Abstract: Rationale, aims, and objectives Laparoscopic sleeve gastrectomy (LSG) is one of the most common bariatric procedures. Gastric leaks and bleeding are the most frequent complications, associated with a high clinical and economic burden. The best method of staple line reinforcement in LSG is debated. Surgical glue is one of the options available. The aim of this study was to assess the safety, efficiency, and relative cost-effectiveness of surgical glue used to perform LSG in morbid obese adults as compared with standard stapling. Methods A prospective, observational, and comparative before-after study was conducted. All consecutive patients undergoing LSG at Montpellier University Hospital in 2011 and 2012 were included and treated according to 2 groups: standard stapling (n = 99, group 1) and surgical glue reinforcement (n = 94, group 2). Clinical and economic outcomes were measured after 6 months. Results The duration of intervention was significantly shorter in group 2 (68 vs 82 minutes, P = .001). There was no significant difference regarding complications, but leaks in group 1 were more severe. Group 2 was also associated with a reduced initial length of stay (4.8 vs 5.2 days, P = .01). Six-month readmissions and total length of stay were also shorter in group 2 (5.5 vs 6.1 days, P = .003). Surgical glue use was associated with a significant reduction in the initial inpatient cost (€5488 vs €6152, P = .005) and in the 6-month total inpatient cost, including readmissions (€6006 vs €6754, P = .005). The incremental cost of glue to avoid a severe complication was −€5446.33 (95 confidence interval, −8202.01 to −2690.66). Conclusions Surgical glue might be a safe and cost-effective intervention in laparoscopic sleeve gastrectomy.

Abstract: Summary Objective: To identify the changes caused by dyslipidemia and obesity in pregnancy suggesting causes for premature birth, and the prognosis for the newborn. Method: Systematic review based on the Medline, Lilacs, Embase and Cochrane library databases between 1996 and 2016. The search for studies included the following keywords: “dyslipidemia, pregnancy, obesity, preterm birth.” A protocol was programmed and a protocol for inclusion/exclusion of studies was implemented. Results: Of the 5,789 articles initially selected between March 1996 and July 2016, only 32 were in accordance with the established criteria. Of these, 28.12% discussed risk factors of prematurity; 37.50%, metabolic alterations and gestational dyslipidemia; 21.87%, dyslipidemic complications in preterm birth; and 12,50%, lipid metabolism, glycemic and placental transfer. Conclusion: There is a reduced adaptation of obese pregnant women to the metabolic changes of gestation. This favors dyslipidemic intercurrences in the mother, which, directly or indirectly, suggests the occurrence of premature births and high lipid transfer to the fetus. Therefore, preterm newborns, whose mothers were dyslipidemic during pregnancy, have greater risk of epicardial fat, both in early (first year of life) and in later (adult) phases of life.

Abstract: Over the last decade, interest has picked up as to emerging economies and their companies. This recent movement does not imply that previous theories and models are no longer applicable, however, one must analyze, in a broader comprehensive manner, a series of specific context factors and those concerning the management of this kind of company. Therefore, this paper analyzed the internationalization strategies adopted by six Brazilian companies before a set of internationalization models. A qualitative research approach was employed and in-depth interviews were conducted with corporate management level professionals. Companies were divided into three groups according to internationalization times (Group I – over ten years, Group II - between five and ten years, and Group III – under five years). Observations indicated that for Groups I and III companies the product lifecycle (Vernon, 1966 and 1979) and the eclectic paradigm (Dunning, 1980, 1988 and 2000) economic models support justifying internationalization motivations for companies’ internalization whilst the Uppsala model (Johanson and VAHLNE, 1979, 2009), at least in part explains, choice decisions concerning a few countries. As to Group II companies, the product lifecycle model is not the underlying motivation for internationalization, choices being more closely related to sector opportunities and the characteristics of both products and services.

Abstract: A caracteristica principal do Acompanhamento Farmacoterapeutico, um dos macrocomponentes da Atencao Farmaceutica, e a documentacao sistematica de informacoes para solucao dos Problemas Relacionados aos Medicamentos (PRM). Essa documentacao e feita, com frequencia, de forma nao automatizada e com o preenchimento de fichas manuais catalogadas em arquivos, o que dificulta a sua organizacao e torna o processo de recuperacao dos dados exaustivo e complicado, limitando a sua aplicacao no cotidiano. Uma maneira de minimizar estes problemas e utilizar um sistema de informacao para dinamizar esse processo, facilitar o acesso aos dados da terapia e melhorar a comunicacao entre o medico e a equipe de saude responsavel pelo paciente. Este artigo apresenta, portanto, o desenvolvimento de um sistema para Atencao Farmaceutica baseado no metodo Dader, denominado Farmatools. Esse sistema tem como objetivo informatizar o metodo Dader, facilitando o acompanhamento farmacoterapeutico, otimizando o tempo de visita e melhorando a recuperacao de informacao e a comunicacao entre os profissionais de saude. O Farmatools visa melhorar o acesso a informacao, diminuindo a inacessibilidade e a perda das informacoes.

Abstract: Background This study investigated the association between sociodemographic, behavioral, oral health, variables of drug consumption, and the short version of the Oral Health Impact Profile (OHIP-14), in Southern Brazilian drug users. Methods In this cross-sectional study, 202 drug users aged 18 or over admitted for treatment at the Institute for Research and Treatment of Alcoholism (Instituto de Pesquisa e Tratamento do Alcoolismo -IPTA) from the municipality of Campo Largo, PR, Brazil, from 2012 to 2014, were involved. They answered a questionnaire and were examined by a calibrated researcher. Data were collected and bivariate (Chi-square test) and multivariate (logistic regression and Wald's test) analyses were performed. Results There was statistical difference (P 10, and number of teeth in mouth ≤27. In the multivariate analysis, statistical difference remained, except for ethnicity (P=0.207). Conclusion Self-perception of oral health was associated with the variables explored, which suggests the need for strategies focused on this population.
