Abstract: Few tools are available to pediatricians for tracking and monitoring disability status in children. We describe the conceptual basis and pilot use of the Functional Independence Measure for Children (WeeFIM). Our pilot use of this instrument in children with limb deficiency, Down's syndrome, spina bifida, cerebral palsy, and extreme prematurity demonstrates that the WeeFIM is a valid measure for tracking disability in preschool age and middle childhood. The WeeFIM measures the impact of developmental strengths and difficulties on independence at home, in school, and in the community. This allows the pediatrician to prioritize interventions for enhancing comprehensive functional outcomes and supporting families.

Abstract: Results Invasive breast cancer incidence was similar in the two groups (528 supplement vs 546 placebo; hazard ratio = 0.96; 95% confidence interval = 0.85 to 1.09). In the nested case – control study, no effect of supplement group assignment on breast cancer risk was seen. Baseline 25-hydroxyvitamin D levels were modestly correlated with total vitamin D intake (diet and supplements) ( r = 0.19, P < .001) and were higher among women with lower BMI and higher recreational physical activity (both P < .001). Baseline 25-hydroxyvitamin D levels were not associated with breast cancer risk in analyses that were adjusted for BMI and physical activity ( P trend = .20).

Abstract: Self-compassion is a self-attitude construct derived from Buddhist psychology (Neff, 2003a). It entails being kind rather than harshly critical toward oneself, perceiving one's experiences as part of the larger human experience, and holding painful feelings in mindful awareness. Given that self-compassion is an Asian construct, this study compares self-compassion levels in the United States, Thailand, and Taiwan. Results indicate that self-compassion is highest in Thailand and lowest in Taiwan, with the United States falling in between. Interdependence is linked to self-compassion in Thailand only, whereas independence is linked to self-compassion in Taiwan and the United States. Results suggest that self-compassion levels in these societies are linked to specific cultural features rather than general East—West differences. However, self-compassion is significantly associated with well-being in all three cultures.

Although conventional randomised controlled trials are widely recognised as the most reliable method to evaluate pharmacological interventions,1 2 scepticism about their role in nonpharmacological interventions (such as surgery) remains.3–6 Conventional randomised controlled trials typically randomise participants to one of two intervenions (A or B) and individual clinicians give intervention A to some participants and B to others. An alternative trial design, the expertise based randomised controlled trial, randomises participants to clinicians with expertise in intervention A or clinicians with expertise in intervention B, and the clinicians perform only the procedure they are expert in. We present evidence to support our argument that increased use of the expertise based design will enhance the validity, applicability, feasibility, and ethical integrity of randomised controlled trials in surgery, as well as in other areas. We focus on established surgical interventions rather than new surgical procedures in which clinicians have not established expertise.
Investigators have used the expertise based design when conventional randomised controlled trials were impossible because different specialty groups provided the interventions under evaluation—for example, percutaneous transluminal coronary angioplasty versus coronary artery bypass graft surgery.7–9 In 1980, Van der Linden suggested randomising participants to clinicians committed to performing different interventions in an area in which a conventional randomised controlled trial was possible.10 Since that time, however, the expertise based design has been little used, even in areas where it has high potential (such as, surgery, physiotherapy, and chiropractic).

Abstract: This paper explores the operational and physical resilience of acute care facilities, recognizing that the key dimension of acute care facilities is not a simple engineering unit. Quantification of resilience is first approached from the broader societal context, from which the engineering subproblem is formulated, recognizing that, to operate, hospitals depend intricately on the performance of their physical infrastructure from the integrity of structural systems and nonstructural systems, lifelines, components, and equipment. Quantification relates the probability of exceeding floor accelerations and interstory drifts within a specified limit space, for the structural and nonstructural performance. Linear and nonlinear structural responses are considered, as well as the impact of retrofit or repair. Impact on time to recovery is considered in all cases. The proposed framework makes it possible to relate probability functions, fragilities, and resilience in a single integrated approach, and to further develop general tools to quantify resilience for sociopolitical-engineering decisions.
