Abstract: This study evaluated by quantitative autopsy correlation a previously developed scoring system for estimating the size of myocardial infarcts based on the QRS complex of the electrocardiogram. This system was tested using electrocardiograms from patients with infarcts shown by autopsy to predominate in the inferior third of the left ventricle. The study was limited to patients whose electrocardiogram did not indicate left or right ventricular hypertrophy, left or right bundle branch block, or left anterior or posterior fascicular block. Thirty-one patients from 6 medical centers met these criteria. In the electrocardiogram of 28 of the 31 patients (90%), lead a VF exhibited a Q wave of at least 30 ms. The correlation coefficient between the total QRS score and the percent infarction of the left ventricle was 0.74. In patients without confounding factors in the electrocardiogram and with single infarcts, the electrocardiogram provides a marker for infarcts in the inferior third of the left ventricle and a quantitative QRS scoring system provides an estimate of infarct size.

Abstract: Background Computer-based diagnostic systems are available commercially, but there has been limited evaluation of their performance. We assessed the diagnostic capabilities of four internal medicine diagnostic systems: Dxplain, Iliad, Meditel, and QMR. Methods Ten expert clinicians created a set of 105 diagnostically challenging clinical case summaries involving actual patients. Clinical data were entered into each program with the vocabulary provided by the program's developer. Each of the systems produced a ranked list of possible diagnoses for each patient, as did the group of experts. We calculated scores on several performance measures for each computer program. Results No single computer program scored better than the others on all performance measures. Among all cases and all programs, the proportion of correct diagnoses ranged from 0.52 to 0.71, and the mean proportion of relevant diagnoses ranged from 0.19 to 0.37. Abstract: Primary and metastatic tumors of the central nervous system are a heterogeneous group of neoplasms with varied outcomes and management strategies. Recently, improved survival observed in 2 randomized clinical trials established combined chemotherapy and radiation as the new standard for treating patients with pure or mixed anaplastic oligodendroglioma harboring the 1p/19q codeletion. For metastatic disease, increasing evidence supports the efficacy of stereotactic radiosurgery in treating patients with multiple metastatic lesions but low overall tumor volume. These guidelines provide recommendations on the diagnosis and management of this group of diseases based on clinical evidence and panel consensus. This version includes expert advice on the management of low-grade infiltrative astrocytomas, oligodendrogliomas, anaplastic gliomas, glioblastomas, medulloblastomas, supratentorial primitive neuroectodermal tumors, and brain metastases. Abstract: Selective laser melting (SLM) has emerged as one of the primary metal additive manufacturing technologies used for many applications in various industries such as medical and aerospace sectors. However, defects such as part distortion and delamination resulted from process-induced residual stresses are still one of the key challenges that hinder widespread adoptions of SLM. For process parameters, the laser beam scanning path will affect the thermomechanical behaviors of the build part, and thus, altering the scanning pattern may be a possible strategy to reduce residual stresses and deformations through influencing the heat intensity input distributions. In this study, a 3D sequentially coupled finite element (FE) model was developed to investigate the thermomechanical responses in the SLM process. The model was applied to test different scanning strategies and evaluate their effects on part temperature, stress and deformation. The major results are summarized as follows. (1) Among all cases tested, the out-in scanning pattern has the maximum stresses along the X and Y directions; while the 45° inclined line scanning may reduce residual stresses in both directions. (2) Large directional stress differences can be generated by the horizontal line scanning strategy. (3) X and Y directional stress concentrations are shown around the edge of the deposited layers and the interface between the deposited layers and the substrate for all cases. (4) The 45° inclined line scanning case also has a smaller build direction deformation than other cases.
