Abstract: Background The use of involved-field radiotherapy after chemotherapy for advanced Hodgkin's lymphoma is controversial. Methods We randomly assigned patients with previously untreated stage III or IV Hodgkin's lymphoma who were in complete remission after hybrid chemotherapy with mechlorethamine, vincristine, procarbazine, prednisone, doxorubicin, bleomycin, and vinblastine (MOPP-ABV) to receive either no further treatment or involved-field radiotherapy. Radiotherapy consisted of 24 Gy to all initially involved nodal areas and 16 to 24 Gy to all initially involved extranodal sites. Patients in partial remission were treated with 30 Gy to nodal areas and 18 to 24 Gy to extranodal sites. Results Of 739 patients, 421 had a complete remission; 161 of these patients were assigned to no further treatment, and 172 to involved-field radiotherapy. The median follow-up was 79 months. Abstract: Objective
The risk of severe infection is a crucial factor in the assessment of the short-term risk:benefit ratio of biologic drugs in rheumatoid arthritis (RA). There is no increase in severe infections in RA patients treated with rituximab (RTX) in controlled trials, but this has not yet been assessed in daily practice. We undertook this study to investigate the occurrence of and risk factors for severe infections in off-trial patients using data from the AutoImmunity and Rituximab (AIR) registry.
Methods
The AIR registry was set up by the French Society of Rheumatology. The charts of patients with severe infections were reviewed.
Results
Of the enrolled patients, 1,303 had at least 1 followup visit at 3 months or later, with a mean ± SD followup period of 1.2 ± 0.8 years (1,629 patient-years). Eighty-two severe infections occurred in 78 patients (5.0 severe infections per 100 patient-years), half of them in the 3 months following the last RTX infusion. Multivariate analysis showed that chronic lung disease and/or cardiac insufficiency (odds ratio 3.0 [95% confidence interval 1.3–7.3], P = 0.01), extraarticular involvement (odds ratio 2.9 [95% confidence interval 1.3–6.7], P = 0.009), and low IgG level (<6 gm/liter) before initiation of RTX treatment (odds ratio 4.9 [95% confidence interval 1.6–15.2], P = 0.005) were significantly associated with increased risk of a severe infection.
Conclusion
The rate of severe infections in current practice is similar to that reported in clinical trials. The risk factors for severe infections include chronic lung and/or cardiac disease, extraarticular involvement, and low IgG before RTX treatment. This suggests that serum IgG should be checked and the risk:benefit ratio of RTX discussed for patients found to have low levels of IgG.

Abstract: It has been suggested that the recently discovered exoplanet GJ581d might be able to support liquid water due to its relatively low mass and orbital distance. However, GJ581d receives 35% less stellar energy than Mars and is probably locked in tidal resonance, with extremely low insolation at the poles and possibly a permanent night side. Under such conditions, it is unknown whether any habitable climate on the planet would be able to withstand global glaciation and/or atmospheric collapse. Here we present three-dimensional climate simulations which demonstrate that GJ581d will have a stable atmosphere and surface liquid water for a wide range of plausible cases, making it the first confirmed super-Earth (exoplanet of 2-10 Earth masses) in the habitable zone. We find that atmospheres with over 10 bar CO2 and varying amounts of background gas (e.g., N2) yield global mean temperatures above 0°C for both land and ocean-covered surfaces. Based on the emitted IR radiation calculated by the model, we propose observational tests that will allow these cases to be distinguished from other possible scenarios in the future.

Abstract: A pentanuclear dysprosium hydroxy cluster of composition [Dy5(μ4-OH)(μ3-OH)4(μ-η2-Ph2acac)4(η2-Ph2acac)6] (1; Ph2acac = dibenzoylmethanide) was prepared starting from [DyCl3·6H2O] and dibenzoylmethane. Both static (dc) and dynamic (ac) magnetic properties of 1 have been studied. Below 3 K, the appearance of slow relaxation of the magnetization typical for single-molecule magnets is seen, even if no hysteresis effects on the M vs H data are observed above 1.8 K.
