Abstract: Route directions are instructions, primarily verbal, that explain how to get from one place to another. The current study examines several methods for assessing the quality of verbal route directions by characterizing them in terms of the number of elements (such as landmarks, segments or turns) and by subjective ratings of their goodness. Route directions for routes which were both familiar and unfamiliar to the participant were studied. Subjective ratings of the quality of route directions were reliable and consistent across individuals. More complete route directions were rated as being of higher quality. For all routes, inclusion of more segment and turn mentions were correlated with higher quality route directions. Good route descriptions for familiar versus unfamiliar routes differed in terms of the types of landmarks included.

Abstract: Methods and compositions for estimating the physiological specificity of a candidate drug involve: (a) detecting reporter gene product signals from each of a plurality of different, separately isolated cells of a target organism, wherein each cell contains a recombinant construct comprising a reporter gene operatively linked to a different endogenous transcriptional regulatory element of the target organism such that the transcriptional regulatory element regulates the expression of the reporter gene, and the sum of the cells comprises an ensemble of the transcriptional regulatory elements of the organism sufficient to model the transcriptional responsiveness of said organism to a drug; (b) contacting each cell with a candidate drug; (c) detecting reporter gene product signals from each cell; (d) comparing reporter gene product signals from each cell before and after contacting the cell with the candidate drug to obtain a drug response profile which provides an estimate of the physiological specificity of the candidate drug.

Abstract: Implementation science in mental health is informed by other academic disciplines and industries. Conceptual and methodological territory charted in psychotherapy research is pertinent to two elements of the conceptual model of implementation posited by Aarons and colleagues (2010)—implementation fidelity and innovation feedback systems. Key characteristics of scientifically validated fidelity instruments, and of the feasibility of their use in routine care, are presented. The challenges of ensuring fidelity measurement methods are both effective (scientifically validated) and efficient (feasible and useful in routine care) are identified as are examples of implementation research attempting to balance these attributes of fidelity measurement.

Abstract: Alan Turing should have been a national hero in Great Britain after the Second World War; by his unique accomplishment in solving the code for a captured German encrypting device called “Enigma”, he saved more lives and treasure than any other of his countrymen. Yet his work was “Top Secret” and slated to remain so for another generation; he received no reward or recognition. To the contrary his then unacceptable life style and his naivete in social and political affairs led him afoul of British law. Faced with the choice between prison and pharmaceutical castration he accepted the latter and within a year was dead from the taste of an apple he had laced with cyanide, a macabre re-play of the fairy tale of Snow White.

Abstract: The invention provides a method of fundoplication of the stomach to the esophagus comprising introducing an esophageal manipulator (72) into the stomach lumen (10), maneuvering the manipulator within the lumen so as to create an intussusception (48) of the esophagus (14) into the stomach, introducing a stapler (68) to the lumen of the stomach, and stapling the intussuscepted esophagus to the stomach operation of the stapler is from within the lumen of the stomach. The invention also provides a medical or surgical stapler comprising an elongated body (80) having a passageway (70) which is adapted to receive a medical component such as a forceps (72) or fluid.
