Abstract: RECENT results1,2 from two ice cores drilled in central Greenland have revealed large, abrupt climate changes of at least regional extent during the late stages of the last glaciation, suggesting that climate in the North Atlantic region is able to reorganize itself rapidly, perhaps even within a few decades. Here we present a detailed stable-isotope record for the full length of the Greenland Ice-core Project Summit ice core, extending over the past 250 kyr according to a calculated timescale. We find that climate instability was not confined to the last glaciation, but appears also to have been marked during the last interglacial (as explored more fully in a companion paper3) and during the previous Saale–Holstein glacial cycle. This is in contrast with the extreme stability of the Holocene, suggesting that recent climate stability may be the exception rather than the rule. The last interglacial seems to have lasted longer than is implied by the deep-sea SPECMAP record4, in agreement with other land-based observations5,6. We suggest that climate instability in the early part of the last interglacial may have delayed the melting of the Saalean ice sheets in America and Eurasia, perhaps accounting for this discrepancy.

Abstract: BACKGROUND Although many patients with intermediate-grade or high-grade (aggressive) non-Hodgkin's lymphoma are cured by combination chemotherapy, the remainder are not cured and ultimately die of their disease. The Ann Arbor classification, used to determine the stage of this disease, does not consistently distinguish between patients with different long-term prognoses. This project was undertaken to develop a model for predicting outcome in patients with aggressive non-Hodgkin's lymphoma on the basis of the patients' clinical characteristics before treatment. METHODS Adults with aggressive non-Hodgkin's lymphoma from 16 institutions and cooperative groups in the United States, Europe, and Canada who were treated between 1982 and 1987 with combination-chemotherapy regimens containing doxorubicin were evaluated for clinical features predictive of overall survival and relapse-free survival. Features that remained independently significant in step-down regression analyses of survival were incorporated into models that identified groups of patients of all ages and groups of patients no more than 60 years old with different risks of death. RESULTS In 2031 patients of all ages, our model, based on age, tumor stage, serum lactate dehydrogenase concentration, performance status, and number of extranodal disease sites, identified four risk groups with predicted five-year survival rates of 73 percent, 51 percent, 43 percent, and 26 percent. In 1274 patients 60 or younger, an age-adjusted model based on tumor stage, lactate dehydrogenase level, and performance status identified four risk groups with predicted five-year survival rates of 83 percent, 69 percent, 46 percent, and 32 percent. In both models, the increased risk of death was due to both a lower rate of complete responses and a higher rate of relapse from complete response. These two indexes, called the international index and the age-adjusted international index, were significantly more accurate than the Ann Arbor classification in predicting long-term survival. CONCLUSIONS The international index and the age-adjusted international index should be used in the design of future therapeutic trials in patients with aggressive non-Hodgkin's lymphoma and in the selection of appropriate therapeutic approaches for individual patients.

Abstract: OXYGEN isotope measurements in Greenland ice demonstrate that a series of rapid warm-cold oscillations—called Dansgaard–Oeschger events—punctuated the last glaciation1. Here we present records of sea surface temperature from North Atlantic sediments spanning the past 90 kyr which contain a series of rapid temperature oscillations closely matching those in the ice-core record, confirming predictions that the ocean must bear the imprint of the Dansgaard–Oeschger events2,3. Moreover, we show that between 20 and 80 kyr ago, the shifts in ocean-atmosphere temperature are bundled into cooling cycles, lasting on average 10 to 15 kyr, with asymmetrical saw-tooth shapes. Each cycle culminated in an enormous discharge of icebergs into the North Atlantic (a 'Hein-rich event'4,5), followed by an abrupt shift to a warmer climate. These cycles document a previously unrecognized link between ice sheet behaviour and ocean–atmosphere temperature changes. An important question that remains to be resolved is whether the cycles are driven by external factors, such as orbital forcing, or by inter-nal ice-sheet dynamics.

Abstract: RECENT results1,2 from the Greenland Ice-core Project (GRIP) Summit ice core suggest that the climate in Greenland has been remarkably stable during the Holocene, but was extremely unstable for the time period represented by the rest of the core, spanning the last two glaciations and the intervening Eemian inter-glacial. Here we present the complete oxygen isotope record for the Greenland Ice Sheet Project 2 (GISP2) core, drilled 28 km west of the GRIP core. We observe large, rapid climate fluctuations throughout the last glacial period, which closely match those reported for the GRIP core. However, in the bottom 10% of the cores, spanning the Eemian interglacial and the previous glacia-tion, there are significant differences between the two records. It is possible that ice flow may have altered the chronological sequences of the stratigraphy for the bottom part of one or both of the cores. Considerable further work will be necessary to evaluate the likelihood of this, and the extent to which it will still be possible to extract meaningful climate information from the lowest sections of the cores.

Abstract: Background The results of case-control studies and anecdotal reports suggest that pancreatitis may be a risk factor for pancreatic cancer, but there have been no studies of sufficient size and power to assess the magnitude of the relation between these two diseases. Methods and Results We undertook a multicenter historical cohort study of 2015 subjects with chronic pancreatitis who were recruited from clinical centers in six countries. A total of 56 cancers were identified among these patients during a mean (±SD) follow-up of 7.4 ±6.2 years. The expected number of cases of cancer calculated from country-specific incidence data and adjusted for age and sex was 2.13, yielding a standardized incidence ratio (the ratio of observed to expected cases) of 26.3 (95 percent confidence interval, 19.9 to 34.2). For subjects with a minimum of two or five years of follow-up, the respective standardized incidence ratios were 16.5 (95 percent confidence interval, 11.1 to 23.7) and 14.4 (95 percent confidence interval, 8...

Abstract: Objective. Different sets of diagnostic criteria have been proposed for Sjogren's syndrome (SS), but none have been validated with a large series of patients or in a multicenter study. We conducted the present study involving 26 centers from 12 countries (11 in Europe, plus Israel), with the goals of reaching a consensus on the diagnostic procedures for SS and defining classification criteria to be used in epidemiologic surveys and adopted by the scientific community. Methods. The study protocol was subdivided into two parts. For part I, questionnaires regarding both ocular and oral involvement were developed; they included 13 questions and 7 questions, respectively. For part II a limited set of diagnostic tests was selected, and the exact procedure to be followed in performing these tests was defined. Part I of the study ipcluded 240 patients with primary SS and 240 age- and sex-matched controls. Two hundred forty-six patients with primary SS, 201 with secondary SS, 113 with connective tissue diseases but without associated SS, and 133 control patients were studied in part II. Results. The study resulted in (a) the validation of a simple 6-item questionnaire for determination of dry eyes and dry mouth, which showed good discriminant power between patients and controls, to be used in the initial screening for sicca syndrome; and (b) the definition of a new set of criteria for the classification of SS. The sensitivity and specificity of the criteria in correctly identifying patients with either the primary or the secondary variant of SS were also determined. Conclusion. Using the findings of this prospective multicenter European study, general agreement can be reached on the diagnostic procedures to be used for patients with SS. Final validation of the preliminary classification criteria for SS is underway.

Abstract: The guanosine triphosphate (GTP)-binding protein Ras functions in regulating growth and differentiation; however, little is known about the protein interactions that bring about its biological activity. Wild-type Ras or mutant forms of Ras were covalently attached to an insoluble matrix and then used to examine the interaction of signaling proteins with Ras. Forms of Ras activated either by mutation (Gly12Val) or by binding of the GTP analog, guanylyl-imidodiphosphate (GMP-PNP) interacted specifically with Raf-1 whereas an effector domain mutant, Ile36Ala, failed to interact with Raf-1. Mitogen-activated protein kinase (MAP kinase) activity was only associated with activated forms of Ras. The specific interaction of activated Ras with active MAP kinase kinase (MAPKK) was confirmed by direct assays. Thus the forming of complexes containing MAPKK activity and Raf-1 protein are dependent upon the activity of Ras.

Abstract: The strength and generality of the relationship between decomposition rates and detritus carbon, nitrogen, and phosphorus concentrations was assessed by comparing published reports of decomposition rates of detritus of photosynthetic organisms, from unicellular algae to trees. The results obtained demonstrated the existence of a general positive, linear relationship between plant decomposition rates and nitrogen and phosphorus concentrations. Differences in the carbon, nitrogen, and phosphorus concentrations of plant detritus accounted for 89% of the variance in plant decomposition rates of detritus orginating from photosynthetic organisms ranging from unicellular microalgae to trees. The results also demonstrate that moist plant material decomposes substantially faster than dry material with similar nutrient concentrations. Consideration of lignin, instead of carbon, concentrations did not improve the relationships obtained. These results reflect the coupling of phosphorus and nitrogen in the basic biochemical processes of both plants and their microbial decomposers, and stress the importance of this coupling for carbon and nutrient flow in ecosystems.

Abstract: We studied the effect of intravenous infusion of synthetic truncated GLP-1 (proglucagon 78–107-amide) on fasting and postprandial gastric acid secretion, gastric emptying, and pancreatic secretion of trypsin and lipase in eight normal volunteers using marker dilution and aspiration technique. The infusion resulted in a plasma concentration of 110±14 pmol/liter (mean±SEM). Truncated GLP-1 significantly inhibited postprandial acid secretion by 43±11%, in spite of unchanged plasma gastrin concentration. Gastric emptying rate decreased significantly; 50% emptying time increased from 16±2 min to 30±5 min. Postprandial trypsin and lipase outputs were significantly inhibited by 47±17% and 40±9%, during truncated GLP-1 infusion. Pancreatic enzyme output was linearly correlated to gastric emptying, and truncated GLP-1 did not affect this relationship, suggesting that the effect on pancreatic secretion was secondary to the effect on gastric emptying. Postprandial insulin and glucagon concentrations were similar with and without truncated GLP-1 infusion in spite of significantly lower blood glucose levels (5.2 ±0.2 versus 3.7±0.3), indicating that GLP-1 stimulated insulin secretion and inhibited glucagon secretion. In conclusion, our results suggest that truncated GLP-1 act as a physiological inhibitor of gastric and pancreatic functions in man.

Abstract: 1. Heat acclimation was induced in eight subjects by asking them to exercise until exhaustion at 60% of maximum oxygen consumption rate (VO2) for 9-12 consecutive days at an ambient temperature of 40 degrees C, with 10% relative humidity (RH). Five control subjects exercised similarly in a cool environment, 20 degrees C, for 90 min for 9-12 days; of these, three were exposed to exercise at 40 degrees C on the first and last day. 2. Acclimation had occurred as seen by the increased average endurance from 48 min to 80 min, the lower rate of rise in the heart rate (HR) and core temperature and the increased sweating. 3. Cardiac output increased significantly from the first to the final heat exposure from 19.6 to 21.4 l min-1; this was possibly due to an increased plasma volume and stroke volume. 4. The mechanism for the increased plasma volume may be an isosmotic volume expansion caused by influx of protein to the vascular compartment, and a sodium retention induced by a significant increase in aldosterone. 5. The exhaustion coincided with, or was elicited when, core temperature reached 39.7 +/- 0.15 degrees C; with progressing acclimation processes it took progressively longer to reach this level. However, at this point we found no reduction in cardiac output, muscle (leg) blood flow, no changes in substrate utilization or availability, and no recognized accumulated 'fatigue' substances. 6. It is concluded that the high core temperature per se, and not circulatory failure, is the critical factor for the exhaustion during exercise in heat stress.

Abstract: The cellular protein p107 shares many structural and biochemical features with the retinoblastoma gene product, pRB. We have isolated a full-length cDNA for human p107 and have used this clone to study the function of p107. We show that, like pRB, p107 is a potent inhibitor of E2F-mediated trans-activation, and overexpression of p107 can inhibit proliferation in certain cell types, arresting sensitive cells in G1. Several experiments, however, showed that growth inhibition by pRB and p107 did not occur through the same mechanism. First, in the cervical carcinoma cell line C33A, p107 was able to block cell proliferation, whereas pRB could not, even though both proteins were potent inhibitors of E2F-mediated transcription in this cell line. Second, growth arrest by pRB and p107 was rescued differentially by various cell cycle regulators. Third, some mutants of p107 that cannot associate with adenovirus E1A were still able to inhibit cell proliferation, whereas analogous mutants in pRB are known to be unable to block cell growth. Together, these results suggest a biological role of p107 that is related, but not identical, to that of pRB.

Abstract: Atopic dermatitis is a multifactorial disease that seems both to rise in frequency and to be dependent on a genetic predisposition. In order to clarify these issues we encircled a representative twin series with atopic dermatitis from a total twin population of 592 like-sexed twin pairs. We found (1) that the cumulative incidence rate (0-7 years) of atopic dermatitis in Denmark has increased significantly from 0.03 for the birth cohort 1960-1964 to 0.10 for the birth cohort 1970-1974, (2) that monozygotic twin pairs are more often concordant for atopic dermatitis than dizygotic twin pairs, (3) that monozygotic twins run a risk of 0.86 of having atopic dermatitis if the twin partner has the disease, whereas the disease risk of 0.21 run by dizygotic partners does not differ from the frequency seen in ordinary brothers and sisters. The results indicate (1) that genetic factors play a decisive role in the development of atopic dermatitis and (2) that widespread environmental factors are operating in genetically susceptible individuals.

Abstract: The widely used and closely related Escherichia coli "wild types" W3110 and MG1655, as well as their common ancestor W1485, starve for pyrimidine in minimal medium because of a suboptimal content of orotate phosphoribosyltransferase, which is encoded by the pyrE gene. This conclusion was based on the findings that (i) the strains grew 10 to 15% more slowly in pyrimidine-free medium than in medium containing uracil; (ii) their levels of aspartate transcarbamylase were highly derepressed, as is characteristic for pyrimidine starvation conditions; and (iii) their levels of orotate phosphoribosyltransferase were low. After introduction of a plasmid carrying the rph-pyrE operon from strain HfrH, the growth rates were no longer stimulated by uracil and the levels of aspartate transcarbamylase were low and similar to the levels observed for other strains of E. coli K-12, E. coli B, and Salmonella typhimurium. To identify the mutation responsible for these phenotypes, the rph-pyrE operon of W3110 was cloned in pBR322 from Kohara bacteriophage lambda 2A6. DNA sequencing revealed that a GC base pair was missing near the end of the rph gene of W3110. This one-base-pair deletion results in a frame shift of translation over the last 15 codons and reduces the size of the rph gene product by 10 amino acid residues relative to the size of RNase PH of other E. coli strains, as confirmed by analysis of protein synthesis in minicells. The truncated protein lacks RNase PH activity, and the premature translation stop in the rph cistron explains the low levels of orotate phosphoribosyltransferase in W3110, since close coupling between transcription and translation is needed to support optimal levels of transcription past the intercistronic pyrE attenuator.

Abstract: The EORTC Study Group on Quality of Life (the Study Group) has adopted a modular approach to quality of life (QOL) assessment in cancer clinical trials. A ‘core’ instrument—the QLQ-C30—has been designed to cover a range of QOL issues relevant to a broad spectrum of cancer patients. It is intended that this QLQ-C30 be supplemented by more specific subscales (‘modules’) to assess aspects of QOL of particular importance to specific subgroups of patients. Since individual members of the Study Group were to be involved in module development, guidelines were required to standardize the module development process in order to ensure uniformly high quality across modules. These guidelines are presented in this paper. The term ‘module’ is defined, the composition of modules is outlined, and the criteria used to develop modules are specified. The module development process, consisting of four phases (generation of relevant QOL issues, operationalization of the QOL issues into a set of items, pretesting the module questionnaire, and large-scale field-testing) is described in detail. Further, issues related to cross-cultural instrument development, and the need for monitoring the module development process from within the Study Group are discussed. Finally, experiences with developing two site-specific modules (i.e., for head and neck, and breast cancer), are presented and the extent to which the guidelines meet practical requirements is discussed. The guidelines appear to provide a practical tool for module construction, that can facilitate the development of a comprehensive system for assessing the QOL of cancer patients internationally.

Abstract: In a cross-sectional epidemiological study of headache disorders information on precipitating factors, age at onset, influence of menstruation and pregnancy and use of oral contraceptives was collected. The presence of migraine and tension-type headache was ascertained by a clinical interview and examination using the operational diagnostic criteria of the International Headache Society. The prevalence of migraine and tension-type headache was also analysed in relation to variables of lifestyle: physical activity, smoking, consumption of coffee, alcohol intake and sleep pattern. In both migraine and tension-type headache, the most conspicuous precipitating factor was stress and mental tension. Other common precipitants were alcohol, weather changes and menstruation. Disappearance or substantial improvement of the headache during pregnancy was more frequent in migraineurs than in tension-type headache sufferers. The age at onset of both migraine and tension-type headache differs between men and women. Female hormones may be an important factor responsible for the sex difference of headache disorders. The level of physical activity showed no association with migraine, but a significantly higher prevalence of tension-type headache in men with exclusively sedentary activity emerged. Smoking, coffee and alcohol consumption showed no significant associations with the headache disorders. Sleep pattern was significantly associated with migraine and tension-type headache in both univariate and multivariate analyses. In conclusion, migraine and tension-type headache seem to be different with regard to a number of endogenous and exogenous factors.

Abstract: Seventy-three transitional cell carcinomas of the bladder were analyzed by immunohistochemistry for p53 nuclear accumulation, and the results were compared to mutations detected in the p53 gene by single strand conformational polymorphism analysis (SSCP) and DNA sequence analysis Immunohistochemical studies were performed on formalin-fixed, paraffin-embedded tissue sections A highly significant association between the presence of p53 mutations and p53 nuclear reactivity as detected by immunohistochemistry was found (P = 00001) Of 32 tumors that demonstrated p53 mutations by SSCP, 27 (84%) showed p53 nuclear reactivity Of the five cases that did not demonstrate p53 nuclear reactivity, four had mutations in exon 5 However, of 41 tumors with no evidence of p53 mutation by molecular analysis, 12 (29%) showed p53 immunoreactivity This indicates that immunohistochemical methods may be more sensitive than SSCP in detecting p53 mutations or that discordant cases represent tumors with accumulation of wild type p53 protein, without mutations at the p53 locus Of the 15 tumors that were found to have mutations at exon 8, 13 demonstrated high-intensity homogeneous p53 nuclear reactivity by immunohistochemistry, and all mutations located at codon 280 demonstrated high-intensity homogeneous immunoreactivity However, three of three tumors with exon 6 mutations demonstrated low-level p53 immunoreactivity, and four of six tumors with mutations in exon 5 showed no detectable p53 nuclear reactivity This indicates that the heterogeneity of immunoreactivity observed when analyzing p53 nuclear accumulation may be related to the site of the p53 gene mutation Information on tumor grade, stage, lymph node status, disease-free interval, and overall survival were available in 54 patients who had undergone cystectomy A significant association was observed between p53 alterations (detected by immunohistochemistry and SSCP) and histological tumor grade (P = 0003) and stage (P = 001) We conclude that the immunohistochemical detection of p53 nuclear accumulation in formalin-fixed, paraffin-embedded tissue is highly associated with mutations in the p53 gene; this association has now been demonstrated in a large number of tumors The heterogeneity of p53 nuclear reactivity seems to be related to the site of mutation in the p53 gene A small proportion of tumors with a p53 gene mutation do not demonstrate immunohistochemically detectable p53 nuclear accumulation Furthermore, a small but substantial proportion of tumors demonstrate p53 nuclear reactivity but do not show detectable mutations in the p53 gene by SSCP Finally, both grade and stage of bladder cancer are related to p53 alterations, detected by immunohistochemistry or molecular methods(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract: 1. The influence of training-induced adaptations in skeletal muscle tissue on the choice between carbohydrates (CHO) and lipids as well as the extra- vs. intracellular substrate utilization was investigated in seven healthy male subjects performing one-legged knee-extension exercise. In each subject one of the knee extensors was endurance trained for eight weeks, whereafter the trained (T) and non-trained (NT) thighs were investigated a week apart. 2. The activity of beta-hydroxy-acyl-coenzyme A dehydrogenase (HAD) and capillary density in the knee extensors were significantly larger in T than in NT. 3. During dynamic knee-extension exercise, performed at the same absolute intensity for 2 h, femoral venous blood flow was lower in T than in NT (P < 0.05), but oxygen uptake was similar. 4. Respiratory quotient (RQ) values over the exercising thigh, averaging 0.81 (T) vs. 0.91 (NT; P < 0.05) indicated that a shift towards a larger fat combustion occurred with endurance training. 5. Both free fatty acids (FFA) and serum triacylglycerol contributed to the utilization of fat in NT and T muscles with no significant contribution from muscle fibre triacylglycerol. 6. At high plasma FFA concentrations net uptake of FFA plateaued in NT but not in T muscles. 7. The findings suggest that FFA uptake in exercising muscle is a saturable process and that the transport capacity is enhanced by training. The lower CHO utilization in the T leg was mainly a function of the glycogenolysis of the muscle being reduced. Hormones such as insulin, noradrenaline and adrenaline are unlikely to play a role in this shift as differences in plasma levels during T and NT leg exercise were small and insignificant, implying that local structural and functional adaptations of the training muscle are crucial for the observed shifts in the metabolic response to exercise.

Abstract: The main aim of this paper is to assess the validity of the life cycle model of consumption. In particular, we address an issue that has recently received much attention, especially in the macroeconomic literature: that of "excess sensitivity" of consumption growth to income growth. We do this using a time series of cross sections and a novel and flexible parameterization of preferences. The former allows us to' address aggregation issues directly, while with the latter we can allow both the discount factor and the elasticity of intertemporal substitution eis to be affected by various observable variables and lifetime wealth. The main findings can be summarized as follows: (i) the excess sensitivity of consumption growth to labor income disappears when we control for demographic variables. This is true both at life cycle and business cycle frequencies. (ii) estimation of a flexible specification of preferences indicates that the elasticity of intertemporal substitution is a function of several variables, including the level of consumption. The eis increases with the level of consumption, as expected. (iii) the variables that change the eis are also important in explaining why we observe excess sensitivity over the business cycle. (iv) we are able to reconcile our results with those reported both in the macro and micro literature. (v) in our specification the elasticity of intertemporal substitution is not very well determined. This result, however, should be taken with care, as we have not made an effort to construct a 'preferred' specification, which would probably include additional controls for labor supply behavior. The evidence presented shows that the life cycle model cannot be easily dismissed. Indeed, we believe that the model does a good job at representing consumption behavior both over the life cycle and over the business cycle.

Abstract: The purpose of this study was to investigate the normal sonographic measurements of the kidney in adult volunteers.Length, width, and thickness of the kidney and its central echogenic area and the parenchymal thickness of the upper pole were measured in an age- and sex-stratified random sample of 665 volunteers 30, 40, 50, 60, and 70 years old. Measurements were made with the volunteers prone. Volumes of the kidney, the central echogenic area, and the renal parenchyma were calculated. Renal dimensions and renal and parenchymal volume were correlated with age, height, weight, body mass index, and total body area. In 94 subjects, renal length was measured with the volunteers supine also.Median renal lengths were 11.2 cm on the left side and 10.9 cm on the right side. Median renal volumes were 146 cm3 in the left kidney and 134 cm3 in the right kidney. Renal size decreased with age, almost entirely because of parenchymal reduction.
