Abstract: Objectives Intercountry comparability between studies on medication use in pregnancy is difficult due to dissimilarities in study design and methodology. This study aimed to examine patterns and factors associated with medications use in pregnancy from a multinational perspective, with emphasis on type of medication utilised and indication for use. Design Cross-sectional, web-based study performed within the period from 1 October 2011 to 29 February 2012. Uniform collection of drug utilisation data was performed via an anonymous online questionnaire. Setting Multinational study in Europe (Western, Northern and Eastern), North and South America and Australia. Participants Pregnant women and new mothers with children less than 1 year of age. Primary and secondary outcome measures Prevalence of and factors associated with medication use for acute/short-term illnesses, chronic/long-term disorders and over-the-counter (OTC) medication use. Results The study population included 9459 women, of which 81.2% reported use of at least one medication (prescribed or OTC) during pregnancy. Overall, OTC medication use occurred in 66.9% of the pregnancies, whereas 68.4% and 17% of women reported use of at least one medication for treatment of acute/short-term illnesses and chronic/long-term disorders, respectively. The extent of self-reported medicated illnesses and types of medication used by indication varied across regions, especially in relation to urinary tract infections, depression or OTC nasal sprays. Women with higher age or lower educational level, housewives or women with an unplanned pregnancy were those most often reporting use of medication for chronic/long-term disorders. Immigrant women in Western (adjusted OR (aOR): 0.55, 95% CI 0.34 to 0.87) and Northern Europe (aOR: 0.50, 95% CI 0.31 to 0.83) were less likely to report use of medication for chronic/long-term disorders during pregnancy than non-immigrants. Conclusions In this study, the majority of women in Europe, North America, South America and Australia used at least one medication during pregnancy. There was a substantial inter-region variability in the types of medication used.

Abstract: The dangers that future climate change poses to physical, biological, and economic systems are accounted for in analyses of risk and increasingly figure in decision-making about responses to climate change. Yet the potential cultural and social impacts of climate change have scarcely been considered. In this article we bring the risks climate change poses to cultures and social systems into consideration through a focus on places—those local material and symbolic contexts that give meaning and value to peoples' lives. By way of examples, the article reviews evidence on the observed and projected impacts of climate change on the Arctic and Pacific island atoll nations. It shows that impacts may result in the loss of many unique natural and cultural components of these places. We then argue that the risk of irreversible loss of places needs to be factored into decision-making on climate change. The article then suggests ways forward in decision-making that recognizes these non-market and non-instrumental metrics of risk, based on principles of justice and recognition of individual and community identity.

Abstract: This paper presents interrelated indicators for tracking progress towards the Paris Agreement. Findings show broad consistency with keeping warming below 2 °C, but technological advances are needed to achieve net-zero emissions. Current emission pledges to the Paris Agreement appear insufficient to hold the global average temperature increase to well below 2 °C above pre-industrial levels1. Yet, details are missing on how to track progress towards the ‘Paris goal’, inform the five-yearly ‘global stocktake’, and increase the ambition of Nationally Determined Contributions (NDCs). We develop a nested structure of key indicators to track progress through time. Global emissions2,3 track aggregated progress1, country-level decompositions track emerging trends4,5,6 that link directly to NDCs7, and technology diffusion8,9,10 indicates future reductions. We find the recent slowdown in global emissions growth11 is due to reduced growth in coal use since 2011, primarily in China and secondarily in the United States12. The slowdown is projected to continue in 2016, with global CO2 emissions from fossil fuels and industry similar to the 2015 level of 36 GtCO2. Explosive and policy-driven growth in wind and solar has contributed to the global emissions slowdown, but has been less important than economic factors and energy efficiency. We show that many key indicators are currently broadly consistent with emission scenarios that keep temperatures below 2 °C, but the continued lack of large-scale carbon capture and storage13 threatens 2030 targets and the longer-term Paris ambition of net-zero emissions.

Abstract: Males and females often age at different rates resulting in longevity 'gender gaps', where one sex outlives the other. Why the sexes have different lifespans is an age-old question, still fiercely debated today. One cellular process related to lifespan, which is known to differ according to sex, is the rate at which the protective telomere chromosome caps are lost. In humans, men have shorter lifespans and greater telomere shortening. This has led to speculation in the medical literature that sex-specific telomere shortening is one cause of sex-specific mortality. However, telomere shortening may be a cause for and/or a consequence of the processes that govern survival, and to infer general principles from single-taxon studies may be misleading. Here, we review recent work on telomeres in a variety of animal taxa, including those with reverse sexual lifespan dimorphism (i.e., where males live longer), to establish whether sex-specific survival is generally associated with sex differences in telomere dynamics. By doing this, we attempt to tease apart the potential underlying causes for sex differences in telomere lengths in humans and highlight targets for future research across all taxa.

Abstract: Deforestation is a main driver of climate change and biodiversity loss. An incentive mechanism to reduce emissions from deforestation and forest degradation (REDD) is being negotiated under the United Nations Framework Convention on Climate Change. Here we use the best available global data sets on terrestrial biodiversity and carbon storage to map and investigate potential synergies between carbon and biodiversity-oriented conservation. A strong association (rS= 0.82) between carbon stocks and species richness suggests that such synergies would be high, but unevenly distributed. Many areas of high value for biodiversity could be protected by carbon-based conservation, while others could benefit from complementary funding arising from their carbon content. Some high-biodiversity regions, however, would not benefit from carbon-focused conservation, and could become under increased pressure if REDD is implemented. Our results suggest that additional gains for biodiversity conservation are possible, without compromising the effectiveness for climate change mitigation, if REDD takes biodiversity distribution into account.
