Abstract: A triangular volar flap has been used successfully for the reconstruction of amputated finger tips. It can be applied to most finger amputations except those with extensive soft-tissue loss on the volar side of the amputated finger tip. The distally based triangular flap, carefully prepared with intact nerve and blood supply, is advanced over exposed bone and approximated to the nail matrix. The V incision is closed by converting it to a Y.

Abstract: Under normal conditions the pericardium, with its fluid, lubricates the moving surfaces of the heart, holds the heart in a fixed geometric position and isolates the heart from other structures in the thorax, thus preventing adhesions and spread of infection. It also serves the following functions: (1) prevents dilatation of the heart chambers and insures that the level of transmural cardiac pressures will be low, never exceeding a few mm Hg; (2) prevents hypertrophy of the heart under conditions of strenuous exercise; (3) limits right ventricular stroke work under conditions of increased left ventricular outflow resistance; (4) prevents ventriculo-atrial regurgitation under conditions of increased ventricular end-diastolic pressures; (5) in association with the lungs and tissue surrounding the pericardium it facilitates the filling of the atria by the development of negative pericardial pressure during ventricular systole; (6) responds to nerve stimulation and reflexly affects blood pressure and heart rate; and (7) in association with the pleural fluid constitutes a hydrostatic system that automatically applies compensating hydrostatic pressures on the outside of the heart when gravitational or inertial forces acting on the heart are altered (during acceleration, for example). This automatic hydrostatic compensation insures that end-diastolic transmural pressure is the same at all hydrostatic levels of the ventricle; as a result the stretch of the muscle fibers is uniform, and the Frank-Starling mechanism operates equally at all hydrostatic levels within the ventricles.

Abstract: Death rates for cervix cancer and total uterine cancer among women 20 years old and older have decreased significantly in Jefferson County, Ky., from 1953-1967. Death rates have not changed significantly in Kentucky outside of Jefferson County during this time period. The average age at time of death from cervix cancer is 7.6 years greater for Jefferson County residents during the last 3 years of the study compared to the first 3 years. The only known factor which could account for these observations is a mass cytologic screening program for cervix cancer in Jefferson County; there was no such mass screening effort in the rest of the state.

Abstract: Fluorescent dye was used to record the occurrence of craze lines during in vitro stainless steel pin procedures. Pinholes were drilled in dentin at various distances from the dentinoenamel junction, and friction-lock and self-threading pins were inserted in the pinholes. Under the conditions of this study, the self-threading regular pin group produced more craze lines than did the other pin groups.

Abstract: The results of 12 years of mass cytologic screening of a community for protection from uterine cancer are reported, and the effects upon incidence rates of the various component cancers are analyzed. A preceding 3-year period during which no screening was performed was used to calculate the expected rates. Total cancers of the uterus, including carcinoma in-situ, had an average annual rate of 125.5 per 100,000 women 20 years of age and older during the 15-year period. During the last 3-year period, carcinoma of the cervix was diagnosed at an earlier stage, and the rates decreased by one third. Carcinoma in-situ increased in proportion to screening activity. There was an increase in the rate for endometrial carcinoma. The greatest response to screening was in women under 50 years of age. This group showed the greatest decrease in rates and was, for the most part, diagnosed at an earlier stage of the disease. On the other hand, older women were less inclined to avail themselves of cytologic screening.

Abstract: Seventy-two cases of proven familial multiple polyposis and 8 deduced cases of familial multiple polyposis were detected in a large Kentucky kindred of 1,422 members. The mean age at onset of symptoms in 41 polyposis patients was 28.9 years, the mean age at diagnosis of polyposis in 72 kindred members was 27.7 years, the mean age at diagnosis of cancer in 30 proven polyposis cases was 38.8 years, and the mean age at cancer diagnosis in 8 deduced polyposis cases was 47.1 years. The comparison of males and females for these age parameters showed no significant difference. In 49 percent of the patients with proven polyposis who manifested symptoms prior to polyposis diagnosis, cancer was present at the time of polyposis diagnosis or 2 years thereafter. This was in sharp contrast to proven cases without prior symptoms in which only 12 percent were found to have cancer at the time of polyposis diagnosis or 2 years thereafter. These findings indicate the importance of symptoms in planning surgical treatment.

Abstract: Outlined and solid-surfaced metric histoforms and polygons we;e employed in an identification task where choice forms were fixed at 0 deg and target forms at six points along the temporal horizontal meridian in the periphery (nasal retina). Accuracy in the identification of solid-surfaced polygons in the far periphery (80 deg from the fovea) was much higher than has been previously reported. Polygons were identified more quickly than histaforms, but a significant difference was not obtained between the speed of identification of outlined and solid shapes. Combined speed and error data indicated that shape (histoform or polygon) is the most salient dimension of a form less than 50 deg from the fovea. whereas beyond 50 deg from the fovea the surface (outlined or solid) of a form is its most salient dimension. Finally. comparisons of the functional relations provided by these performance data with anatomical and physiological data produced the following hypothesis: Spatial summation of rod receptors beyond 20 deg of angular eccentricity in the nasal retina is directly proportional to the density of rod receptors.

Abstract: Electron micrographs of dog papillary muscle fibers show the sarco-plasmic reticulum (SR) as an extensive three-dimensional network of diverging and converging tubules that is continuous throughout each sarcomere and in successive sarcomeres. The network surrounds each myofibril and is continuous across the fiber. Tubules of the SR network are oriented transversely and longitudinally. Interconnections of transversely and longitudinally oriented SR tubules form circular pathways in the network. A structural relationship appears to exist between the Z line and longitudinally and transversely oriented tubules of the SR network at their points of apposition with the Z line. A rather constant space approximately 100 A wide is seen between SR tubules apposed at the Z line in cross-sections and in favorable longitudinal sections of fibrils. This space is traversed by electron-opaque strands which appear to be connections between SR tubules and the Z line. Other lines of evidence support the view that Z lines and SR tubules are connected. One of these is the change of position of Z line SR tubules in relation to the change of position of Z lines in adjacent fibrils. Another line of evidence is the continuity of SR apposed at the Z line and SR connected to the sarcolemma.

Abstract: ATMOSPHERIC attack on stone statuary has increased considerably during the past few decades because of the presence of increasing amounts of gaseous pollutants such as SO2 and CO2 (ref. 1). Abstract: Studies of the comparative effects of a semi-synthetic diet containing supplements of corn oil, no fat, linolenate ortrans,trans-linoleate on blood coagulation parameters are reported. In spite of large differences in fatty acid composition of the tissue lipids of the different groups, the only diets that appeared to produce abnormal hematologic and hemostatic properties were those containingtrans,trans-linoleate. These groups of animals showed significant differences from a control group of animals fed Purina rat chow in platelet and fibrinogen concentration, and values for hematocrit and prothrombin time. A positive fibrinolysin test was also obtained in about 50% of the animals fedtrans,trans-linoleate.

Abstract: The space between the sarcoplasmic reticulum (SR) apposed to the transverse (T) tubules and the sarcolemma in dog papillary muscle fibers is traversed by structures of varying densities. The distance between apposed membranes is about 70 A at the level of densities and about 100 A at the level of less dense areas that alternate with the densities. The apposed SR shows a marked increase in electrondense material when comparison is made with the SR not apposed. The electron-dense material is located in a dense layer parallel to and about midway between the membranes of the apposed SR and in numerous projections from the inside surface of the membranes. With appropriate staining procedures the central dense layer and the projections are as thick as membranes and they frequently show paired densities separated by a distance about equal to the distance between the outer dense layers of trilaminar membranes. The paired densities in the central dense layer and in the projections indicate that the electron-dense material within apposed SR has structure. The projections are frequently in register with scallops or serrations in the SR membranes and they also appear to contribute to a waviness in the central dense layer. The serrations and waviness are interpreted as evidence that the projections exert a holding force because they project from the SR membranes and connect with the central dense layer.
