Abstract: We study a longitudinal sample of over one million French workers from more than five hundred thousand employing firms. We decompose real total annual compensation per worker into components related to observable employee characteristics, personal heterogeneity, firm heterogeneity, and residual variation. Except for the residual, all components may be correlated in an arbitrary fashion. At the level of the individual, we find that person effects, especially those not related to observables like education, are a very important source of wage variation in France. Firm effects, while important, are not as important as person effects. At the level of firms, we find that enterprises that hire high-wage workers are more productive but not more profitable. They are also more capital and high-skilled employee intensive. Enterprises that pay higher wages, controlling for person effects, are more productive and more profitable. They are also more capital intensive but are not more high-skilled labor intensive. We find that person effects explain about 90% of inter-industry wage differentials and about 75% of the firm-size wage effect while firm effects explain relatively little of either differential.

Abstract: Prospective studies on the detection of minimal residual disease (MRD) in acute leukemia patients have shown that large-scale MRD studies are feasible and that clinically relevant MRD-based risk group classification can be achieved and can now be used for designing new treatment protocols. However, multicenter international treatment protocols with MRD-based stratification of treatment need careful standardization and quality control of the MRD techniques. This was the aim of the European BIOMED-1 Concerted Action 'Investigation of minimal residual disease in acute leukemia: international standardization and clinical evaluation' with participants of 14 laboratories in eight European countries (ES, NL, PT, IT, DE, FR, SE and AT). Standardization and quality control was performed for the three main types of MRD techniques, ie flow cytometric immunophenotyping, PCR analysis of antigen receptor genes, and RT-PCR analysis of well-defined chromosomal aberrations. This study focussed on the latter MRD technique. A total of nine well-defined chromosome aberrations with fusion gene transcripts were selected: t(1;19) with E2A-PBX1, t(4;11) with MLL-AF4, t(8;21) with AML1-ETO, t(9;22) with BCR-ABL p190 and BCR-ABL p210, t(12;21) with TEL-AML1, t(15;17) with PML-RARA, inv (16) with CBFB-MYH11, and microdeletion 1p32 with SIL-TAL1. PCR primers were designed according to predefined criteria for single PCR (external primers A B) and nested PCR (internal primers C D) as well as for 'shifted' PCR with a primer upstream (E5' primer) or downstream (E3' primer) of the external A B primers. The 'shifted' E primers were designed for performing an independent PCR together with one of the internal primers for confirmation (or exclusion) of positive results. Various local RT and PCR protocols were compared and subsequently a common protocol was designed, tested and adapted, resulting in a standardized RT-PCR protocol. After initial testing (with adaptations whenever necessary) and approval by two or three laboratories, the primers were tested by all participating laboratories, using 17 cell lines and patient samples as positive controls. This testing included comparison with local protocols and primers as well as sensitivity testing via dilution experiments. The collaborative efforts resulted in standardized primer sets with a minimal target sensitivity of 10-2 for virtually all single PCR analyses, whereas the nested PCR analyses generally reached the minimal target sensitivity of 10-4. The standardized RT-PCR protocol and primer sets can now be used for molecular classification of acute leukemia at diagnosis and for MRD detection during follow-up to evaluate treatment effectiveness.

Abstract: Semiclassical approximation addresses the important relationship between quantum and classical mechanics. There has been a very strong development in the mathematical theory, mainly thanks to methods of microlocal analysis. This book develops the basic methods, including the WKB-method, stationary phase and h-pseudodifferential operators. The applications include results on the tunnel effect, the asymptotics of eigenvalues in relation to classical trajectories and normal forms, plus slow perturbations of periodic Schrodinger operators appearing in solid state physics. No previous specialized knowledge in quantum mechanics or microlocal analysis is assumed, and only general facts about spectral theory in Hilbert space, distributions, Fourier transforms and some differential geometry belong to the prerequisites. This book is addressed to researchers and graduate students in mathematical analysis, as well as physicists who are interested in rigorous results. A fairly large fraction can be (and has been) covered in a one semester course.

Abstract: α-Galactosylceramide (α-GalCer) is a glycolipid with potent antitumor properties that binds to CD1d molecules and activates mouse Vα14 and human Vα24 NKT cells. Surprisingly, we found that, as early as 90 min after α-GalCer injection in vivo, NK cells also displayed considerable signs of activation, including IFN-γ production and CD69 induction. NK activation was not observed in RAG- or CD1-deficient mice, and it was decreased by pretreatment with anti-IFN-γ Abs, suggesting that, despite its rapid induction, it was a secondary event that depended on IFN-γ release by NKT cells. At later time points, B cells and CD8 T cells also began to express CD69. These findings identify a high-speed communication network between the innate and adaptive immune systems in vivo that is initiated upon NKT cell activation. They also suggest that the antitumor effects of α-GalCer result from the sequential recruitment of distinct innate and adaptive effector lymphocytes.

Abstract: Performance bounds for criteria for model selection are devel- oped using recent theory for sieves. The model selection criteria are based on an empirical loss or contrast function with an added penalty term moti- vated by empirical process theory and roughly proportional to the number of parameters needed to describe the model divided by the number of ob- servations. Most of our examples involve density or regression estimation settings and we focus on the problem of estimating the unknown density or regression function. We show that the quadratic risk of the minimum penal- ized empirical contrast estimatoris bounded by an index of the accuracy of the sieve. This accuracy index quanties the trade-off among the candidate models between the approximation error and parameter dimension relative to sample size. If we choose a list of models which exhibit good approximation prop- erties with respect to different classes of smoothness, the estimator can be simultaneously minimax rate optimal in each of those classes. This is what is usually called adaptation. The type of classes of smoothness in which one gets adaptation depends heavily on the list of models. If too many models are involved in order to get accurate approximation of many wide classes of functions simultaneously, it may happen that the estimator is only approx-

Abstract: Background Cytomegalovirus (CMV) disease is a major complication of organ transplantation. We hypothesized that prophylactic treatment with valacyclovir would reduce the risk of CMV disease. Methods A total of 208 CMV-negative recipients of a kidney from a seropositive donor and 408 CMV-positive recipients were randomly assigned to receive either 2 g of valacyclovir or placebo orally four times daily for 90 days after transplantation, with the dose adjusted according to renal function. The primary end point was laboratory-confirmed CMV disease in the first six months after transplantation. Results Treatment with valacyclovir reduced the incidence or delayed the onset of CMV disease in both the seronegative patients (P<0.001) and the seropositive patients (P=0.03). Among the seronegative patients, the incidence of CMV disease 90 days after transplantation was 45 percent among placebo recipients and 3 percent among valacyclovir recipients. Among the seropositive patients, the respective values were 6 percent and 0 percent. At six months, the incidence of CMV disease was 45 percent among seronegative recipients of placebo and 16 percent among seronegative recipients of valacyclovir; it was 6 percent among seropositive placebo recipients and 1 percent among seropositive valacyclovir recipients. At six months, the rate of biopsy-confirmed acute graft rejection in the seronegative group was 52 percent among placebo recipients and 26 percent among valacyclovir recipients (P=0.001). Treatment with valacyclovir also decreased the rates of CMV viremia and viruria, herpes simplex virus disease, and the use of inpatient medical resources. Hallucinations and confusion were more common with valacyclovir treatment, but these events were not severe or treatment limiting. The rates of other adverse events were similar among the groups. Conclusions Prophylactic treatment with valacyclovir is a safe and effective way to prevent CMV disease after renal transplantation. (N Engl J Med 1999; 340:1462-70.) Abstract: BACKGROUND In an 11-country European collaboration, 14 population-based surveys included 21,724 subjects aged > or = 65 years. Most participating centres used the Geriatric Mental State (GMS), but other measures were also used. AIMS To derive from these instruments a common depression symptoms scale, the EURO-D, to allow comparison of risk factor profiles between centres. METHOD Common items were identified from the instruments. Algorithms for fitting items to GMS were derived by observation of item correspondence or expert opinion. The resulting 12-item scale was checked for internal consistency, criterion validity and uniformity of factor-analytic profile. RESULTS The EURO-D is internally consistent, capturing the essence of its parent instrument. A two-factor solution seemed appropriate: depression, tearfulness and wishing to die loaded on the first factor (affective suffering), and loss of interest, poor concentration and lack of enjoyment on the second (motivation). CONCLUSIONS The EURO-D scale should permit valid comparison of risk-factor associations between centres, even if between-centre variation remains difficult to attribute.

Abstract: The results in this paper concern computations of Floer cohomology using generating functions. The first part proves the isomorphism between Floer cohomology and Generating function cohomology introduced by Lisa Traynor. The second part proves that the Floer cohomology of the cotangent bundle (in the sense of Part I), is isomorphic to the cohomology of the loop space of the base. This has many consequences, some of which were given in Part I, others will be given in forthcoming papers. The results in this paper had been announced (with indications of proof) in a talk at the ICM 94 in Z¨ urich.

Abstract: Family and twin studies have suggested a genetic component in autism. We performed a genome-wide screen with 264 microsatellites markers in 51 multiplex families, using non-parametric linkage methods. Families were recruited by a collaborative group including clinicians from Sweden, France, Norway, the USA, Italy, Austria and Belgium. Using two-point and multipoint affected sib-pair analyses, 11 regions gave nominal P -values of 0.05 or lower. Four of these regions overlapped with regions on chromosomes 2q, 7q, 16p and 19p identified by the first genome-wide scan of autism performed by the International Molecular Genetic Study of Autism Consortium. Another of our potential susceptibility regions overlapped with the 15q11-q13 region identified in previous candidate gene studies. Our study revealed six additional regions on chromosomes 4q, 5p, 6q, 10q, 18q and Xp. We found that the most significant multipoint linkage was close to marker D6S283 (maximum lod score = 2.23, P = 0.0013).

Abstract: The effects of elevated atmospheric CO2 concentration on growth of forest tree species are difficult to predict because practical limitations restrict experiments to much shorter than the average life-span of a tree. Long-term, processbased computer models must be used to extrapolate from shorter-term experiments. A key problem is to ensure a strong flow of information between experiments and models. In this study, meta-analysis techniques were used to summarize a suite of photosynthetic model parameters obtained from 15 field-based elevated [CO2] experiments on European forest tree species. The parameters studied are commonly used in modelling photosynthesis, and include observed light-saturated photosynthetic rates (Amax), the potential electron transport rate (Jmax), the maximum Rubisco activity (Vcmax) and leaf nitrogen concentration on mass (Nm) and area (Na) bases. Across all experiments, light-saturated photosynthesis was strongly stimulated by growth in elevated [CO2]. However, significant down-regulation of photosynthesis was also observed; when measured at the same CO2 concentration, photosynthesis was reduced by 10‐20%. The underlying biochemistry of photosynthesis was affected, as shown by a down-regulation of the parameters Jmax and Vcmax of the order of 10%. This reduction in Jmax and Vcmax was linked to the effects of elevated [CO2] on leaf nitrogen concentration. It was concluded that the current model is adequate to model photosynthesis in elevated [CO2]. Tables of model parameter values for different European forest species are given.

Abstract: Objective To establish the sequence of events leading from vitreofoveal traction to full-thickness macular hole formation. Methods Both eyes of 76 patients with a full-thickness macular hole in at least 1 eye were examined by biomicroscopy and optical coherence tomography. Results Sixty-one fellow eyes had a normal macula. Optical coherence tomograms showed central detachment of the posterior hyaloid over the posterior pole in 19 cases (31%) and a perifoveal hyaloid detachment not detected on biomicroscopy in 26 cases (42%). In the 4 impending macular holes, optical coherence tomography disclosed various degrees of intrafoveal split or cyst, with adherence of the posterior hyaloid to the foveal center and convex perifoveal detachment. In the 14 stage 2 holes, eccentric opening of the roof of the hole was observed, and in the 24 stage 3 holes, the posterior hyaloid was detached from the entire posterior pole. Conclusions In fellow eyes of eyes with macular holes posterior hyaloid detachment begins around the macula, but the hyaloid remains adherent to the foveolar center, indicating the action of anteroposterior forces. This results in an intraretinal split evolving into a cystic space, and then to the disruption of the outer retinal layer and the opening of the foveal floor, thus constituting a full-thickness macular hole.

Abstract: PCR-ribotyping, a typing method based on polymorphism in the 16S-23S intergenic spacer region, has been recently used to investigate outbreaks due to Clostridium difficile. However, this method generates bands of high and close molecular masses which are difficult to separate on agarose gel electrophoresis. To improve reading of banding patterns of PCR-ribotyping applied to C. difficile, a partial sequencing of the rRNA genes (16S and 23S) and intergenic spacer region has been performed, then a new set of primers located closer to the intergenic spacer region has been defined. The new PCR gave reproducible patterns of bands easy to separate on agarose gel electrophoresis. Each of the 10 serogroups and 11 subgroups of serogroup A produced a different pattern. This typing method has evidenced major qualities such as easiness, rapidity and reproducibility. However, its discriminatory power has to be evaluated to validate its importance as a typing tool for C. difficile.

Abstract: For a system with one degree of freedom, coherent states that are parametrized by classical canonical action-angle variables are introduced. These states also possess continuity of labelling, a resolution of unity, and temporal stability. The insistence on canonical action-angle variables strongly restricts any remaining arbitrariness in the coherent state definition. Such states are introduced for semibounded Hamiltonian operators having either a discrete or a continuous spectrum. Hamiltonians that have both discrete and continuous parts in their spectrum are also discussed.

Abstract: Electrosynthesis of poly(3,4-ethylenedioxythiophene) (PEDOT) films was performed in a micellar aqueous solution containing sodium dodecyl sulfate (SDS) by cyclic voltammetry, chronoamperometry, and chronopotentiometry on a platinum electrode. The electrocatalytic effect of SDS was characterized by a significant decrease of the EDOT oxidation potential (Ep) in the micellar medium relative to 0.1 M LiClO4 acetonitrile as well as aqueous solutions. Linear variation of Ep with SDS concentration indicated the formation of a pseudocomplex (Keq = 5.4 × 103 M-1). PEDOT films were characterized electrochemically and spectroscopically (UV−visible, X-ray photoelectron spectroscopy, IR, Raman spectra). Regular, well-ordered, and adherent films were obtained in SDS medium. The PEDOT film morphologies investigated by atomic force microscopy suggested the possible presence of columnar structures when the electrosynthesis is performed in the micellar medium.
