Abstract: OBJECTIVE: To assess the efficacy of systemic corticosteroids (CS) alone as first-line treatment in patients with Churg-Strauss syndrome (CSS) without poor-prognosis factors, as defined by the Five-Factors Score (FFS), and to compare the efficacy and safety of oral azathioprine (AZA) versus intravenous pulse cyclophosphamide (CYC) as adjuvant immunosuppressive therapy for treatment failure or relapse. METHODS: This multicenter, prospective, randomized, open-label therapeutic trial included 72 patients with newly diagnosed CSS (FFS of 0) treated with CS alone. At treatment failure or relapse, patients were randomized to receive 6 months of oral AZA or 6 pulses of CYC. Analyses were performed according to an intent-to-treat strategy. RESULTS: The mean +/- SD followup was 56.2 +/- 31.7 months. Among the 72 patients studied, 93% achieved remission with CS therapy alone, and 35% relapsed, mainly during the first year of treatment. Among the 19 patients randomized to additional immunosuppression because of treatment failure or relapse, 5 of 10 receiving AZA and 7 of 9 receiving pulse CYC achieved remission, but the difference was not statistically significant. Survival rates in all patients at 1 and 5 years were 100% and 97%, respectively. At the end of followup, 79% of the patients whose disease was in remission required low-dose CS therapy, mainly to control respiratory disease. CS-related adverse events were observed in 31% of the 72 patients. CONCLUSION: In CSS patients with an FFS of 0, survival was excellent, confirming the predictive value of the FFS in this disease. First-line therapy with CS achieved remission in most patients, but relapses were common, and one-third of them required additional immunosuppressive therapy. AZA or pulse CYC was fairly effective in treating CS-resistant disease or major relapses. Over the long term, most patients continued to take oral CS, which might explain the high rate of CS-related adverse events.

Abstract: Aurora-A protein kinase, which is the product of an oncogene, is required for the assembly of a functional mitotic apparatus and the regulation of cell ploidy. Overexpression of Aurora-A in tumour cells has been correlated with cancer susceptibility and poor prognosis. Aurora-A activity is required for the recruitment of CDK1-cyclin B1 to the centrosome prior to its activation and the commitment of the cell to mitosis. In this report, we demonstrate that the CDC25B phosphatase, an activator of cyclin dependent kinases at mitosis, is phosphorylated both in vitro and in vivo by Aurora-A on serine 353 and that this phosphorylated form of CDC25B is located at the centrosome during mitosis. Knockdown experiments by RNAi confirm that the centrosome phosphorylation of CDC25B on S353 depends on Aurora-A kinase. Microinjection of antibodies against phosphorylated S353 results in a mitotic delay whilst overexpression of a S353 phosphomimetic mutant enhances the mitotic inducing effect of CDC25B. Our results demonstrate that Aurora-A phosphorylates CDC25B in vivo at the centrosome during mitosis. This phosphorylation might locally participate in the control of the onset of mitosis. These findings re-emphasise the role of the centrosome as a functional integrator of the pathways contributing to the triggering of mitosis.

Abstract: Sensing, monitoring and actuating systems are expected to play a key role in reducing buildings overall energy consumption. Leveraging sensor systems to support energy efficiency in buildings poses novel research challenges in monitoring space usage, controlling devices, interfacing with smart energy meters and communicating with the energy grid. In the attempt of reducing electricity consumption in buildings, identifying individual sources of energy consumption is key to generate energy awareness and improve efficiency of available energy resources usage. Previous work studied several non-intrusive load monitoring techniques to classify appliances; however, the literature lacks of an comprehensive system that can be easily installed in existing buildings to empower users profiling, benchmarking and recognizing loads in real-time. This has been a major reason holding back the practice adoption of load monitoring techniques. In this paper we present RECAP: RECognition of electrical Appliances and Profiling in real-time. RECAP uses a single wireless energy monitoring sensor easily clipped to the main electrical unit. The energy monitoring unit transmits energy data wirelessly to a local machine for data processing and storage. The RECAP system consists of three parts: (1) Guiding the user for profiling electrical appliances within premises and generating a database of unique appliance signatures; (2) Using those signatures to train an artificial neural network that is then employed to recognize appliance activities (3) Providing a Load descriptor to allow peer appliance benchmarking. RECAP addresses the need of an integrated and intuitive tool to empower building owners with energy awareness. Enabling real-time appliance recognition is a stepping-stone towards reducing energy consumption and allowing a number of major applications including load-shifting techniques, energy expenditure breakdown per appliance, detection of power hungry and faulty appliances, and recognition of occupant activity. This paper describes the system design and performance evaluation in domestic environment.
