Abstract: Remodeling of the cytoskeleton is central to the modulation of cell shape and migration. Filamin A, encoded by the gene FLNA, is a widely expressed protein that regulates re-organization of the actin cytoskeleton by interacting with integrins, transmembrane receptor complexes and second messengers. We identified localized mutations in FLNA that conserve the reading frame and lead to a broad range of congenital malformations, affecting craniofacial structures, skeleton, brain, viscera and urogenital tract, in four X-linked human disorders: otopalatodigital syndrome types 1 (OPD1; OMIM 311300) and 2 (OPD2; OMIM 304120), frontometaphyseal dysplasia (FMD; OMIM 305620) and Melnick-Needles syndrome (MNS; OMIM 309350). Several mutations are recurrent, and all are clustered into four regions of the gene: the actin-binding domain and rod domain repeats 3, 10 and 14/15. Our findings contrast with previous observations that loss of function of FLNA is embryonic lethal in males but manifests in females as a localized neuronal migration disorder, called periventricular nodular heterotopia (PVNH; refs. 3-6). The patterns of mutation, X-chromosome inactivation and phenotypic manifestations in the newly described mutations indicate that they have gain-of-function effects, implicating filamin A in signaling pathways that mediate organogenesis in multiple systems during embryonic development.

Abstract: (1) Moist tropical forests in Amazonia and elsewhere are subjected to increasingly severe drought episodes through the El Nino-Southern Oscillation (ENSO) and possibly through deforestation-driven reductions in rainfall. The effects of this trend on tropical forest canopy dynamics, emissions of greenhouse gases, and other ecological functions are potentially large but poorly understood. We established a throughfall exclusion experiment in an east-central Amazon forest (Tapajos National Forest, Brazil) to help understand these effects. After 1-year intercalibration period of two 1-ha forest plots, we installed plastic panels and wooden gutters in the understory of one of the plots, thereby excluding � 890 mm of throughfall during the exclusion period of 2000 (late January to early August) and � 680 mm thus far in the exclusion period of 2001 (early January to late May). Average daily throughfall reaching the soil during the exclusion period in 2000 was 4.9 and 8.3 mm in the treatment and control plots and was 4.8 and 8.1 mm in 2001, respectively. During the first exclusion period, surface soil water content (0-2 m) declined by � 100 mm, while deep soil water (2-11 m) was unaffected. During the second exclusion period, which began shortly after the dry season when soil water content was low, surface and deep soil water content declined by � 140 and 160 mm, respectively. Although this depletion of soil water provoked no detectable increase in leaf drought stress (i.e., no reduction in predawn leaf water potential), photosynthetic capacity declined for some species, the canopy thinned (greater canopy openness and lower leaf area index) during the second exclusion period, stem radial growth of trees <15 m tall declined, and fine litterfall declined in the treatment plot, as did tree fruiting. Aboveground net primary productivity (NPP) (stemwood increment and fine litter production) declined by one fourth, from 15.1 to 11.4 Mg ha � 1 yr � 1 , in the treatment plot and decreased slightly, from 11.9 to 11.5 Mg ha � 1 yr � 1 , in the control plot. Stem respiration varied seasonally and was correlated with stem radial growth but showed no treatment response. The fastest response to the throughfall exclusion, and the surface soil moisture deficits that it provoked, was found in the soil itself. The treatment reduced N2O emissions and increased CH4 consumption relative to the control plot, presumably in response to the improved soil aeration that is associated with soil drying. Our hypothesis that NO emissions would increase following exclusion was not supported. The conductivity and alkalinity of water percolating through the litter layer and through the mineral soil to a depth of 200 cm was higher in the treatment plot, perhaps because of the lower volume of water that was moving through these soil layers in this plot. Decomposition of the litter showed no difference between plots. In sum, the small soil water reductions provoked during the first 2 years of partial throughfall exclusion were sufficient to lower aboveground NPP, including the stemwood increment that determines the amount of carbon stored in the

Abstract: Rationale: In normal lungs, local changes in pleural pressure (Ppl) are generalized over the whole pleural surface. However, in a patient with injured lungs, we observed (using electrical impedance tomography) a pendelluft phenomenon (movement of air within the lung from nondependent to dependent regions without change in tidal volume) that was caused by spontaneous breathing during mechanical ventilation.Objectives: To test the hypotheses that in injured lungs negative Ppl generated by diaphragm contraction has localized effects (in dependent regions) that are not uniformly transmitted, and that such localized changes in Ppl cause pendelluft.Methods: We used electrical impedance tomography and dynamic computed tomography (CT) to analyze regional inflation in anesthetized pigs with lung injury. Abstract: In 1985, WHO stated that there was no justification for caesarean section (CS) rates higher than 10–15 % at population-level. While the CS rates worldwide have continued to increase in an unprecedented manner over the subsequent three decades, concern has been raised about the validity of the 1985 landmark statement. We conducted a systematic review to identify, critically appraise and synthesize the analyses of the ecologic association between CS rates and maternal, neonatal and infant outcomes. Four electronic databases were searched for ecologic studies published between 2000 and 2014 that analysed the possible association between CS rates and maternal, neonatal or infant mortality or morbidity. Two reviewers performed study selection, data extraction and quality assessment independently. We identified 11,832 unique citations and eight studies were included in the review. Seven studies correlated CS rates with maternal mortality, five with neonatal mortality, four with infant mortality, two with LBW and one with stillbirths. Except for one, all studies were cross-sectional in design and five were global analyses of national-level CS rates versus mortality outcomes. Although the overall quality of the studies was acceptable; only two studies controlled for socio-economic factors and none controlled for clinical or demographic characteristics of the population. In unadjusted analyses, authors found a strong inverse relationship between CS rates and the mortality outcomes so that maternal, neonatal and infant mortality decrease as CS rates increase up to a certain threshold. In the eight studies included in this review, this threshold was at CS rates between 9 and 16 %. However, in the two studies that adjusted for socio-economic factors, this relationship was either weakened or disappeared after controlling for these confounders. CS rates above the threshold of 9–16 % were not associated with decreases in mortality outcomes regardless of adjustments. Our findings could be interpreted to mean that at CS rates below this threshold, socio-economic development may be driving the ecologic association between CS rates and mortality. On the other hand, at rates higher than this threshold, there is no association between CS and mortality outcomes regardless of adjustment. The ecological association between CS rates and relevant morbidity outcomes needs to be evaluated before drawing more definite conclusions at population level.
