Too much of a good thing: a retrospective study of β-lactam concentration-toxicity relationships.

Abstract: Objectives To determine the existence of concentration-toxicity relationships for common β-lactam antibiotic adverse effects and define thresholds above which toxicity is more likely. Patients and methods Retrospective review of consecutive patients treated with piperacillin, meropenem or flucloxacillin who underwent therapeutic drug monitoring (TDM) at St Vincent's Hospital (Sydney, Australia) between January 2013 and December 2015. Adverse events investigated included neurotoxicity, nephrotoxicity, hepatotoxicity and opportunistic Clostridium difficile infection. Toxicity was measured using observational grading criteria, clinical assessment and relevant serum biomarkers. These findings were correlated with trough TDM measurements at the time of toxicity presentation. Results TDM results from 378 patients (piperacillin = 223, meropenem = 94 and flucloxacillin = 61) were investigated. There was no difference in baseline patient characteristics across antibiotic groups. A statistically significant elevation in mean serum trough concentrations (Cmin) was found in patients diagnosed with neurotoxicity (piperacillin, P 361.4 mg/L; meropenem, Cmin >64.2 mg/L; flucloxacillin, Cmin >125.1 mg/L) or nephrotoxicity (piperacillin, Cmin >452.65 mg/L; meropenem, Cmin >44.45 mg/L) varied across antibiotics. Conclusions Our data reveal an association between toxic concentrations for a number of β-lactam agents and neurotoxic/nephrotoxic effects. We have defined threshold concentrations above which these toxicities become more likely. Clinicians should balance concerns for therapeutic efficacy with potential toxicity when considering aggressive therapy.

Percent body fat via DEXA: comparison with a four-compartment model.

Abstract: This study compared body composition by dual-energy X-ray absorptiometry (DEXA; Lunar DPX-L) with that via a four-compartment (4C; water, bone mineral mass, fat, and residual) model. Relative body fat was determined for 152 healthy adults [30.0 +/- 11.1 (SD) yr; 75.10 +/- 14.88 kg; 176.3 +/- 8.7 cm] aged from 18 to 59 yr. The 4C approach [20.7% body fat (%BF)] resulted in a significantly (P < 0.001) higher mean %BF compared with DEXA (18.9% BF), with intraindividual variations ranging from -2.6 to 7.3% BF. Linear regression and a Bland and Altman plot demonstrated the tendency for DEXA to progressively underestimate the %BF of leaner individuals compared with the criterion 4C model (4C %BF = 0.862 x DEXA %BF + 4.417; r(2) = 0.952, standard error of estimate = 1.6% BF). This bias was not attributable to variations in fat-free mass hydration but may have been due to beam-hardening errors that resulted from differences in anterior-posterior tissue thickness.

A framework for lean manufacturing implementation

Abstract: The lean implementation initiatives can be categorised as roadmap, conceptual/implementation framework, descriptive and assessment checklist initiatives. A literature review on the lean initiatives has examined 28 initiatives. A set of rules is proposed to evaluate these initiatives with respect to nine factors impacting lean implementation. The evaluation has proved that the implementation frameworks have highest association with lean factors. However, existing lean initiatives are not demonstrated in a structured nature. The failure in managing lean implementation process is often consolidated to poor mind-set and inadequate understanding of the lean concept itself. In this paper, an attempt has been made to propose a framework to overcome some of the limitations.
