Abstract: Taxol, an antimicrotubule agent, has shown promise for efficacy in treatment of breast cancer, but severe hypersensitivity reactions led to cessation of many phase I clinical trials. Consequently, investigators and the National Cancer Institute recommended that phase I and II studies of this agent use 24-hour infusions and antiallergic medications. Using a premedication regimen effective in preventing hypersensitivity reactions, we have performed a phase II trial of taxol in patients with metastatic breast cancer. Taxol was administered to 25 patients at a dose of 250 mg/m2 by 24-hour infusion every 21 days. These patients had received only one prior chemotherapy regimen, either adjuvant to surgery or for metastatic disease; all but two had received doxorubicin. In 60% of the patients, the dominant site of disease was the viscera. All patients were assessable. In April 1991, at a median time on study of 9 months (range, 5-13+ months), the objective response rate was 56% (12% complete and 44% partial; 95% confidence interval, 35%-76%). Disease progressed in only 8% of the patients. The median number of courses of therapy was 11. Granulocytopenia was the dose-limiting toxic effect, but neutropenia with fever occurred in only 5% of 232 courses. A chronic glove-and-stocking neuropathy developed in most patients, but no allergic reactions occurred. We conclude that taxol is an active agent in the treatment of metastatic breast cancer and that it warrants continued study. Currently, we are conducting a phase I trial of taxol plus doxorubicin. Future trials should address the optimal effective dose, the optimal sequencing of combinations, mechanisms of drug resistance in tumors, and dose-limiting toxic effects (particularly cardiac toxic effects of taxol given as a single agent or in drug combinations).

Abstract: Evidence supports the hypothesis that the Laurentian and East Antarctic-Australian cratons were continuous in the late Precambrian and that their Pacific margins formed as a conjugate rift pair. Both margins extend for approximately 40° of latitude. They have a similar rift history throughout their length—i.e., Late Proterozoic rifting and Early Cambrian carbonate platform development. A geometrically acceptable computer-generated reconstruction for the latest Precambrian juxtaposes and aligns the Grenville front that is truncated at the Pacific margin of Laurentia and a closely comparable tectonic boundary in East Antarctica that is truncated along the Weddell Sea margin. These may prove to be critical, perhaps even unique, "piercing points" for relating the northern and southern continents. Geologic and paleomagnetic evidence also suggests that the Atlantic margin of Laurentia rifted from the proto-Andean margin of South America in earliest Cambrian time. Early Phanerozoic sea-floor spreading that isolated Laurentia from South America and East Antarctica-Australia in an Eocambrian supercontinent appears to balance convergence along the Mozambique suture which resulted in final amalgamation of the smaller Gondwana supercontinent at ∼500 Ma.

Abstract: Because of their ubiquitous presence in high-speed flight and their impact on vehicle and component performance, shock-wave/boundary-layer interactions have been studied for about 50 years. Despite truly remarkable progress in computational and measurement capabilities, there are still important quantities that cannot be predicted very accurately, that is, peak heating in strong interactions, or cannot be predicted at all, that is, unsteady pressure loads. There remain observations that cannot be satisfactorily explained and physical processes that are not well understood. Much work remains to be done. Based on the author's own views and those of colleagues, some suggestions are made as to where future efforts might be focused. Just as the first workers in the field could not have foreseen the capabilities generated by the computer/instrumentation revolution of the past 20 years, it is probably fair to assume that the extent of our vision and imagination in the year 2000 is equally limited. New simulation and measurement techniques will doubtlessly become available in the next 10 or 20 years, the results from which will render many of the current concerns moot
