Abstract: Background. Adult mortality in the first 3 months on antiretroviral therapy (ART) is higher in low-income than in high-income countries, with more similar mortality after 6 months. However, the specific patterns of changing risk and causes of death have rarely been investigated in adults, nor compared with children in low-income countries. Methods. We used flexible parametric hazard models to investigate how mortality risks varied over the first year on ART in human immunodeficiency virus–infected adults (aged 18–73 years) and children (aged 4 months to 15 years) in 2 trials in Zimbabwe and Uganda. Results. One hundred seventy-nine of 3316 (5.4%) adults and 39 of 1199 (3.3%) children died; half of adult/pediatric deaths occurred in the first 3 months. Mortality variation over year 1 was similar; at all CD4 counts/CD4%, mortality risk was greatest between days 30 and 50, declined rapidly to day 180, then declined more slowly. One-year mortality after initiating ART with 0–49, 50–99 or ≥100 CD4 cells/μL was 9.4%, 4.5%, and 2.9%, respectively, in adults, and 10.1%, 4.4%, and 1.3%, respectively, in children aged 4–15 years. Mortality in children aged 4 months to 3 years initiating ART in equivalent CD4% strata was also similar (0%–4%: 9.1%; 5%–9%: 4.5%; ≥10%: 2.8%). Only 10 of 179 (6%) adult deaths and 1 of 39 (3%) child deaths were probably medication-related. The most common cause of death was septicemia/meningitis in adults (20%, median 76 days) and children (36%, median 79 days); pneumonia also commonly caused child deaths (28%, median 41 days). Conclusions. Children ≥4 years and adults with low CD4 values have remarkably similar, and high, mortality risks in the first 3 months after ART initiation in low-income countries, similar to cohorts of untreated individuals. Bacterial infections are a major cause of death in both adults and children; targeted interventions could have important benefits.

Abstract: Tropical upland regions have both the world's highest population growth rates and a preponderance of soils less suitable for agriculture. Nitrogen deficiency is almost universal, while acid infertility and the related problem of P deficiency affect more than 40% of tropical soils. Nutrient depletion, as opposed to inherent infertility, affects large areas due to continuous cropping with few inputs. Nitrogen management requires a continual supply of N that can be achieved through fertilization, green manuring, legume rotations or leguminous tree-shrub fallows. Rotations and fallows usually require foregoing one or more staple crop harvests, which is not feasible for smallholder farmers in land-limited areas. In such areas there are few options for farmers except mineral N fertilizers. Phosphorus availability can be built up in soils, but to achieve this, external inputs of inorganic P are essential. Use of P-efficient crops, or mobilisation of soil organic P by various means, are temporary solutions since they involve only offtake with no addition. Phosphorus additions may be either as soluble processed fertilizers or indigenous phosphate rocks (PRs). Most tropical PR deposits are unreactive and require processing before use. Some lower-cost options include partial acidulation, blending with soluble P sources or microbial solubilization. The relevance of microbial solubilization in particular requires testing at larger scale. There are no magic solutions to soil nutrient deficiencies or toxicities; to maintain productivity mineral fertilizers are necessary. They should be used in judicious amounts and coupled with improved organic matter management.

Abstract: Grandparents throughout sub-Saharan Africa have shown immense courage and fortitude in providing care and support for AIDS-affected children. However, growing old comes with a number of challenges which can compromise the quality of care and support they are able to provide, particularly for children infected by HIV and enrolled on antiretroviral therapy (ART) programmes. For ART to be effective, and for infected children not to develop drug-resistance, a complex treatment regimen must be followed. Drawing on the perspectives of 25 nurses and eight grandparents of HIV-infected children in Manicaland, eastern Zimbabwe, we explore some of the challenges faced by grandparents in sustaining children's adherence to ART. These challenges, serving as barriers to paediatric ART, are poverty, immobility, deteriorating memory and poor comprehension of complex treatments. Although older HIV-infected children were found to play an active role in sustaining the adherence to their programme of treatment by contributing to income and food generating activities and reminding their guardians about check-ups and drug administration, such contribution was not available from younger children. There is therefore an urgent need to develop ART services that both take into consideration the needs of elderly guardians and acknowledge and enhance the agency of older children as active and responsible contributors to ART adherence.

Abstract: Agroforestry systems comprise trees and crops, or trees and pastures within the same field. Globally, they cover approximately 1 billion hectares of land and contribute to the livelihoods of over 900 million people. Agroforestry systems have the capacity to sequester large quantities of carbon (C) in both soil and biomass. However, these systems have not yet been fully considered in the approach to C accounting developed by the Intergovernmental Panel on Climate Change, largely due to the high diversity of agroforestry systems and scarcity of relevant data. Our literature review identified a total of 72 scientific, peer-reviewed articles associated with biomass C storage (50) and with soil organic carbon (SOC) (122), containing a total of 542 observations (324 and 218, respectively). Based on a synthesis of the reported observations, we are presenting a set of Tier 1 coefficients for biomass C storage for each of the eight main agroforestry systems identified, including alley cropping, fallows, hedgerows, multistrata, parklands, shaded perennial-crop, silvoarable and silvopastoral systems, disaggregated by climate and region. Using the same agroforestry classification, we are presenting a set of stock change factors (F LU ) and SOC accumulation/loss rates for three main land use changes (LUCs): cropland to agroforestry; forest to agroforestry; and grassland to agroforestry. Globally, the mean SOC stock change factors (± confidence intervals) were estimated to be 1.25 ± 0.04, 0.89 ± 0.07, and 1.19 ± 0.10, for the three main LUCs, respectively. However, these average coefficients hide huge disparities across and within different climates, regions, and types of agroforestry systems, highlighting the necessity to adopt the more disaggregated coefficients provided herein. We encourage national governments to synthesize data from local field experiments to generate country-specific factors for more robust estimation of biomass and SOC storage.
