Abstract: Volumetric modulated arc therapy (VMAT) has found widespread clinical application in recent years. A large number of treatment planning studies have evaluated the potential for VMAT for different disease sites based on the currently available commercial implementations of VMAT planning. In contrast, literature on the underlying mathematical optimization methods used in treatment planning is scarce. VMAT planning represents a challenging large scale optimization problem. In contrast to fluence map optimization in intensity-modulated radiotherapy planning for static beams, VMAT planning represents a nonconvex optimization problem. In this paper, the authors review the state-of-the-art in VMAT planning from an algorithmic perspective. Different approaches to VMAT optimization, including arc sequencing methods, extensions of direct aperture optimization, and direct optimization of leaf trajectories are reviewed. Their advantages and limitations are outlined and recommendations for improvements are discussed.

Abstract: The purpose of this study was to compare the cardiovascular capacities of individuals with Down syndrome (DS) to individuals without Down syndrome who are mentally retarded. Sixteen young adults with DS and 16 individuals without DS (12 males and 4 females, respectively), all with mild/moderate mental retardation, participated in this study. Peak VO2 (absolute and relative), VE (1.min-1), heart rate (HR, b.min-1), and RER (VCO2/VO2) were determined by exercise tests utilizing a treadmill (TM) and Schwinn Air-Dyne ergometer (SAE). The best test result was chosen from the TM and SAE tests and used for statistical comparisons. Cardiac output (Q, 1.min-1) was measured while standing quietly and while walking at 3 mph, 0% grade, using the CO2 rebreathing method for 11 (9 males and 2 females) subjects from each group. Arteriovenous oxygen differences (a-v O2), cardiac index (QI), and stroke volume (SV) were calculated from VO2, Q, HR, and body surface area. Peripheral vascular resistance (PVR), left ventricular work index (LVWI), and left ventricular stroke work index (LVSWI) were calculated from mean arterial pressure, Q, QI, and stroke volume index. Results showed that individuals without DS had statistically significant (P less than 0.01) higher mean peak VO2 (35.6 vs 24.6 ml.kg-1.min-1; 2567 vs 1683 ml.min-1), VE (89.3 vs 59.2 1/min-1), and HR (179 vs 159 b.min-1) than individuals with DS, respectively. No differences in RER were seen between the groups. No differences were seen in cardiovascular parameters measured while quietly standing.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract: We propose the use of Twitter analysis as an alternative source of data to document weekly trends in emotion and stress, and attempt to use the method to estimate the work recovery effect of weekends. On the basis of 2,102,176,189 Tweets, we apply Pennebaker's linguistic inquiry word count (LIWC) approach to measure daily Tweet content across 18 months, aggregated to the US national level of analysis. We derived a word count dictionary to assess work stress and applied p-technique factor analysis to the daily word count data from 19 substantively different content areas covered by the LIWC dictionaries. Dynamic factor analysis revealed two latent factors in day-level variation of Tweet content. These two factors are: (a) a negative emotion/stress/somatic factor, and (b) a positive emotion/food/friends/home/family/leisure factor, onto which elements of work, money, achievement, and health issues have strong negative loadings. The weekly trend analysis revealed a clear “Friday dip” for work stress and negative emotion expressed on Twitter. In contrast, positive emotion Tweets showed a “mid-week dip” for Tuesday-Wednesday-Thursday and “weekend peak” for Friday through Sunday, whereas work/money/achievement/health problem Tweets showed a small “weekend dip” on Fridays through Sundays. Results partially support the Effort-Recovery theory. Implications and limitations of the method are discussed.

Abstract: This study examined the associations between lifetime mental disorder, comorbidity, and self-reported postrunaway arrests among 428 (187 males, 241 females) homeless and runaway youth. The analysis examined the pattern of arrests across five lifetime mental disorders (alcohol abuse, drug abuse, conduct disorder, major depressive episode, and posttraumatic stress disorder). The adolescents, ranging from 16 to 19 years old, were interviewed directly on the streets and in shelters in four Midwestern states using computer-assisted personal interviewing. Extensive self-reports of early life history, behaviors since running away from home, and diagnostic interviewing (UM-CIDI and DISC-R) were used to estimate possible disorders. There was a high level of postrunaway arrests reported by the youth; more than half were arrested at least once after the initial runaway, with the average of 4.4 times. Consistent with the hypotheses, there were differential associations between individual mental disorders and involvement with the criminal justice system. Only externalizing disorders such as substance abuse and conduct disorder were related to arrest. Street youth with multiple externalizing and internalizing disorders were more likely to be arrested than nondisordered youths.

Abstract: Much has happened in the municipal bond market during the past 25 years. This article provides a retrospective of some of the significant developments in the market during that period of time. These developments include passage of the Tax Reform Act of 1986, innovations in the market in response to changing economic and social conditions, and the regulation, increase in disclosure requirements, and proliferatioin of credit enhancements that renewed the efficacy of municipal securities for American state and local governments.
