TL;DR: PCNL in supine position is safe, effective and suitable for the patients, and offers the potential advantages of better urethral access, less patient handling, needing drape only once, ability to perform simultaneous PCNL and ureteroscopic procedures, thus reducing over-all operative time compared to traditional prone position and performing PCNL while the surgeon is sitting.
Abstract: Purpose: PCNL (percutaneous nephrolithotripsy) is usually performed in the prone position. The supine position has potential advantages comparing with the prone position. Our objective is comparison between the outcome of PCNL in the complete supine position and the standard prone position. Material and Methods: A total of 80 patients who underwent PCNL from February 2008 to June 2008 evaluated in a prospective manner, were randomly divided into two groups: 40 patients underwent prone PCNL and 40 patients underwent complete supine PCNL. We performed complete supine PCNL with no rolled towel under the flank and no change in leg position. Results in both positions were compared regarding technical aspects, operative time, stone free rate, complications and hospital stay, and were analyzed using t-test and chi–square. Abstract: Background Malalignment after total knee replacement could cause overloading of the implant bearing as well as of the bone itself, leading to osteolysis and early loosening. To quantify the stresses the implant has to withstand and to define a safe zone of limb alignment, the total contact forces as well as their mediolateral distribution have to be determined. Analytical gait data and mathematical models have been used for this purpose. We performed this study to determine in vivo loads of five patients after implantation of an instrumented tibial baseplate. Methods Five patients with osteoarthritis of the knee received total knee replacement. The tibial component was instrumented with strain gauges for the measurement of three forces and three moments. The signals from the gauges were transferred telemetrically to an external receiver. At twelve months after surgery, postoperative measurements were obtained with the patients walking at a self-selected comfortable speed across a level walkway. Peak axial and medial forces of fifteen to twenty gait cycles were averaged and reported as a percent of individual body weight. Results During the stance phase of the gait cycle, two maxima of the axial force occurred. Typical values were 215% of body weight at the first peak and 266% of body weight at the second peak. The medial load share was typically 73% at the first axial force peak and 65% at the second axial force peak. Analysis of inter-individual variations revealed a linear correlation with limb alignment. A deviation of 1° varus from neutral alignment increased the medial load share by 5%. Conclusions Consistent with the results of previous studies, we found that the force transferred by the medial compartment was usually greater than that transferred by the lateral compartment. Concerning the design of total knee replacements, an asymmetric tibial component with a larger medial contact area could possibly reduce peak contact stress on the bone and improve fixation of the implant. Mediolateral load distribution was quantified and correlated with limb alignment, thereby permitting the effects of malalignment after total knee replacement to be estimated.

Abstract: Using a testing framework, developers may create a test module to centralize resources and results for a software test plan amongst a plurality of systems. With assistance from the testing framework, the test module may facilitate the creation of test cases, the execution of a test job for each test case, the collection of performance statistics during each test job, and the aggregation of collected statistics into organized reports for easier analysis. The test module may track test results for easy comparison of performance metrics in response to various conditions and environments over the history of the development process. The testing framework may also schedule a test job for execution when the various systems and resources required by the test job are free. The testing framework may be operating system independent, so that a single test job may test software concurrently on a variety of systems.

Abstract: A behavioral targeting system determines user profiles from online activity. The system includes a plurality of models that define parameters for determining a user profile score. Event information, which comprises on-line activity of the user, is received at an entity. To generate a user profile score, a model is selected. The model comprises recency, intensity and frequency dimension parameters. The behavioral targeting system generates a user profile score for a target objective, such as brand advertising or direct response advertising. The parameters from the model are applied to generate the user profile score in a category. The behavioral targeting system has application for use in ad serving to on-line users.

Abstract: In multiway data, each sample is measured by multiple sets of correlated attributes. We develop a probabilistic framework for modeling structural dependency from partially observed multi-dimensional array data, known as pTucker. Latent components associated with individual array dimensions are jointly retrieved while the core tensor is integrated out. The resulting algorithm is capable of handling large-scale data sets. We verify the usefulness of this approach by comparing against classical models on applications to modeling amino acid fluorescence, collaborative filtering and a number of benchmark multiway array data.
