Abstract: SUMMARY The exposure of operating-theatre personnel to vapours of halothane during routine anaesthetic work was studied. The concentrations of halothane in the atmosphere of an operating theatre and in expired air and blood from the personnel were determined by means of gas chromatography. Possible deleterious effects of repeated exposures are discussed. The need for reducing the amount of waste anaesthetic vapours in operating theatres is pointed out. ZUSAMMENFASSUNG Die Beeintrachtigung des Operationssaalpersonals durch Halothandampfe wahrend der Routine-Narkosearbeit wurde untersucht. Mittels der Gaschro-matographie wurden die Konzentrationen von Halothan in der Operations-saalatmosphare, in der Ausatmungsluft und in Blut des Personals bestimmt. Mogliche schadliche Wirkungen des wiederholten Ausgesetztseins werden diskutiert. Schliesslich wird auf die Notwendigkeit hingewiesen, den Uber-schuss von Narkosedampfen in den Operationssalen zu reduzieren.

Abstract: SUMMARY 1 Mepivacaine and lidocaine were infused intravenously and into the brachial artery in healthy human subjects. The effects on arterial blood pressure, forearm blood flow, forearm vascular resistance and venous tone were studied. 2 Intravenous infusion of mepivacaine at a rate of 0.25 mg per kg body weight per min during 20 minutes increased the mean arterial blood pressure, forearm blood flow and venous tone. Blockade of the sympathetic nerves to the arm abolished die effect on blood flow, while the increase in blood pressure and venous tone persisted. 3 Intra-arterial infusions of 0.5-6 mg per min of mepivacaine increased the venous tone and decreased the forearm blood flow. The mean arterial blood pressure was unchanged, while the calculated forearm vascular resistance increased. Following sympadietic blockade, the effect on forearm vascular resistance and blood flow was almost completely eliminated, while the increase in venous tone persisted. 4 Intra-arterial infusion of 1-3 mg per min of lidocaine increased the venous tone, but did not significantly influence die arterial blood pressure of forearm blood flow. ZUSAMMENFASSUNG 1 Mepivacain und Lidocain wurden gesunden Versuchspersonen sowohl intravenos als auch in die Armaterie infundiert. Dabei wurde die Wirkung auf den arteriellen Blutdruck, die Durchblutung des Unterarmes, der Gefass-widerstand des Unterarmes und auch der Venentonus untersucht. 2 Die intra venose Infusion von Mepivacain mit einer Geschwindigkeit von 0,25 mg pro kg Korpergewicht und Minute wahrend 10 Minuten erhohte den mittleren arteriellen Blutdruck, die Durchblutung des Unterarmes und den Venentonus. Blockade der sympadiischen Nerven des Armes loschte die Wirkung auf die Durchblutung aus, wahrend der erhohte Blutdruck und Venentonus bestehen blieben. 3 Intraarterielle Infusion von Mepivacain 0,5-0,6 mg pro Minute er-hohten den Venentonus und verminderten die Durchstromung des Unterarmes. Der mittlere arterielle Blutdruck blieb unverandert, wahrend der er-rechnete Gefasswiderstand des Unterarmes anstieg. Nach Sympathikusblockade verschwand die Wirkung auf den Gefasswiderstand des Unterarmes und auf die Durchblutung nahezu vollstandig, wahrend der erhohte Venentonus bestehen blieb. 4 Die intraarterielle Infusion von Lidocain 1-3 mg/Minute erhohte wohl den Venentonus, hatte aber keinen signifikanten Einfluss auf den arteriellen Blutdruck und die Durchblutung des Unterarmes.

Abstract: SUMMARY The effect of a small prior dose of tubocurarine on intragastric pressure during suxamethonium fasciculations was studied. Two groups were compared, viz. 24 patients given a small dose of tubocurarine before induction of anaesthesia (pre-oxygenation-Leopental® -suxamethonium-intubation sequence) and 27 patients who did not receive tubocurarine. It was found that a dose of tubocurarine 4-6 mg completely prevented the rise in intragastric pressure due to suxamethonium fasciculations. In addition, a remarkably low intragastric pressure was found after die suxamethonium injections in die group with the prior tubocurarine dose. The difference was highly significant (mean (t-test): P < 0.0005, variance (F-test): P < 0.001). It is stated that the use of a small dose of tubocurarine before relaxation with suxamethonium offers some advantages. The intragastric pressure during relaxation before intubation is lower and is not increased by fasciculations, and protection is afforded against cardiac arrhythmias and muscle pains. Disadvantages are that die patients are more resistant to relaxation widi suxamethonium, and that relaxation takes a little more time. The risk of dual block must also be considered. ZUSAMMENFASSUNG Es wurde der Einfluss einer kleinen Vorausdosis von Tubocurarin auf den Mageninnendruck wahrend der Succinylcholin-Muskelzuckungen untersucht. Zwei Gruppen wurden verglichen, namlich 24 Patienten, denen eine kleine Dosis von Tubocurarin vor der Narkoseeinleitung verabreicht wurde (Se-quenz: Sauerstoffanreicherung, Leopental®, Suxamedionium, Intubation) und 27 Patienten, die kein Tubocurarin erhielten. Dabei zeigte sich, dass eine Tubocurarindosis von 4-6 mg den durch die Suxamethonium-Zuckungen bedingten Anstieg des Mageninnendruckes kom-plett zu verhuten imstande war. Uberdies wurde ein bemerkenswert niedriger Mageninnendruck nach Suxamethoniuminjektion bei jener Patientengruppe gefunden, die vorher Tubocurarin erhalten hatte. Der Unterschied war hoch-signifikant(Mittel(T-Test):P < 0,0005, Varianz(F-Test):P < 0,001). Es kann somit festgestellt werden, dass die Gabe einer kleinen Menge von Tubocurarin vor der Erschlaffung mit Suxamethonium gewisse Vorteile bietet. Der Mageninnendruck wahrend der Erschlaffung vor der Intubation ist niedriger und wird auch durch die Muskelfaszikulationen nicht erhbht. Oberdies ist gleichzeitig ein Schutz gegen Arrhydimien und Muskelschmerzen gegeben. Nachteilig ist jedoch, dass die Patienten resistenter gegen Succinylcholin werden, und dass der Eintritt der Erschlaffung langer braucht. Auch das Risiko eines Doppelblocks muss in Erwagung gezogen werden.

Abstract: SUMMARY The acid-base status in lumbar and ventricular samples of cerebrospinal fluid was analysed in 77 patients. Of the patients, 21 had no clinical signs of brain lesion at the time of sampling; 28 conscious patients had mild symptoms, and 28 unconscious patients had severe symptoms of brain lesion. Arterial and, in some cases, jugular venous blood was collected and analysed simultaneously with CSF samples. Low CSF pH and low CSF bicarbonate were found both in lumbar and in ventricular samples from the unconscious patients. The mean CSF-arterial pCOa difference was high in the unconscious patients both in lumbar and ventricular fluid, whereas these differences were normal in conscious patients. ZUSAMMENFASSUNG Bei 77 Patienten wurde der Saure-Basen-Status von Liquorproben analy-siert, die durch Lumbal- bzw. Ventrikelpunktionen gewonnen worden waren. Von diesen Patienten hatten 21 zum Zeitpunkt der Abnahme keine klinischen Zeichen einer Gehirnlasion, 28 wache Patienten hatten milde Symptome und 28 bewusstlose Patienten hatten schwere Symptome von Gehirnschaden. Gleich-zeitig mit den Liquorproben wurden auch arterielle Blutproben und in manchen Fallen Blut aus der V. jugularis abgenommen und analysiert. Bei den bewusstlosen Patienten fanden sich niedrige pH- und Bicarbonat- werte sowohl in den lumbalen als auch in den Ventrikelliquorproben. Die mittlere pC02-DifFerenz zwischen Liquor und arteriellem Blut war bei den bewusstlosen Patienten sowohl bei den lumbalen als auch bei den Ventrikel- proben hoch, wahrend sie bei den wachen Patienten normal war. Die mittlere arterio-vendse pCOj-Differenz war bei den wachen Patienten normal, bei den bewusstlosen jedoch signinkant reduziert. Bei dieser letzteren Gruppe war auch die mittlere pCOa-Differenz zwischen Liquor und Jugularvenenblut signinkant erh6ht. Die Bedeutung dieser Befunde wird diskutiert und dabei der Schluss ge-zogen, dass die Liquorazidose, die bei Bewussdosen sowohl in den lumbalen als auch in den intracraniellen Fliissigkeitsproben vorliegt, sowohl mit niedri-gen Bicarbonat- als auch mit hohen pC02-Werten vergesellschaftet ist. Diese Werte bestatigen die friihere Annahme, dass Patienten mit schweren Gehirn-lasionen eine intrazerebrale Azidose durch die Anhaufung von sauren Meta-boliten, aber auch von Kohlensaure, bekommen. Die spontane Hyperventilation dieser Patienten ist ein Versuch, diese Azidose zu kompensieren. Eine wirkungsvolle Kompensation kann jedoch nicht erreicht werden, wenn die arterielle Kohlensaurespannung nicht auf etwa 25 mm Hg gesenkt wird; es wird deshalb eine Behandlung mit kunstlicher Ventilation empfohlen.

Abstract: ZUSAMMENFASSUNG UND SCHLUSSFOLGERUNGEN Der Ventilations-Perfusionsquotient wurde bei normalen Hunden wahrend der Beatmung mit verschiedenen Respirationsformen bestimmt. Dabei fand sich, dass dieser Quotient anstieg, wenn zwischen das Ende der Insufflation und den Beginn der Exspiration ein stromungsfreies Intervall zwischengeschaltet wurde. Auf der Basis von Berechnungen des physiologischen Totraumes und des venosen Shunts kann angenommen werden, dass die Ursache fur diesen Anstieg eine Neuverteilung sowohl der Ventilation als auch der Perfusion ist. Bei Anwendung eines stromungsfreien Intervalls wird das Verhaltnis zwischen Ventilation und Perfusion gleichmassiger, wenn auch nicht ideal.

Abstract: Solutions containing one of the amino acids 1‐alanine, 1‐arginine, glycine, 1‐histidine, 1‐proline or 1‐glutamic acid and a basic mixture of the essential 1‐amino acids and 1‐arginine and 1‐histidine were infused intravenously to healthy volunteers on a nitrogen‐poor diet. The nitrogen balance and die excretion of urea, ammonia, a‐amino nitrogen and creatinine in the urine were determined. Glutamic acid provoked considerable discomfort to the subjects and histidine caused some nausea. In high doses, arginine competed with lysine for renal excretion resulting in a loss of lysine in the urine. Infusion of arginine, proline, alanine and glutamic acid resulted in a nitrogen retention which was higher than that caused by glycine and histidine. It was concluded that the non‐essential amino acids best suited for intravenous nitrogen nutrition are arginine, proline and alanine.

Abstract: SUMMARY Fifteen patients anaesthetized with halothane were given a beta-adrenergic receptor blocking substance intravenously, either propranolol or l-alprenolol. The drugs usually induced an insignificant drop in blood pressure, a slight bradycardia, and a small reduction in cardiac output in patients not subjected to concomitant surgery. In one patient, the reaction was more pronounced. The immediate administration of atropine seemed to restore the circulatory state. Nine patients were subjected to surgery during the administration of the beta-blocking agents. Pronounced hypotension resulted in seven of them. This effect was observed both after propranolol and l-alprenolol. Again, atropine reversed the hypotension. These observations suggest that the administration of beta-adrenergic receptor blocking agents during anaesthesia should be limited by very strict indications, and be used under carefully controlled conditions. ZUSAMMENFASSUNG 15 mit Halothan narkotisierte Patienten erhielten i.v. einen Beta-Rezep-torenblocker, und zwar entweder Propanolol oder 1-Alprenolol. Diese Mittel fiihrten in der Regel zu einem nicht-signifikanten Blutdruck-abfall, zu einer geringgradigen Bradykardie und einer leichten Verminderung des Herzminutenvolumens bei Patienten, die nicht gleichzeitig operiert wurden. Bei einem Patienten war die Reaktion dramatischer. Die sofortige Gabe von Atropin schien den Kreislaufzustand wieder zu normalisieren. Neun Patienten erhielten Betablocker wahrend einer Operation. Bei sieben von ihnen kam es zu ausgepragten Blutdruckabfall. Diese Wirkung wurde sowohl nach Propranolol als auch nach 1-Alprenolol beobachtet. Diese Beobachtungen geben Grund zur Empfehlung, Betarezeptoren-blocker wahrend der Narkose nur bei strengster Indikation zu verabreichen und sie nur unter sorgfaltig kontrollierten Bedingungen anzuwenden.

Abstract: The effects of spinal anesthesia on renal hemodynamics are still unclear despite several efforts at clarification (SMITH et al. ( 1939)9, ASSALI et al. (1951)', Co RCORAN, TAYLOR and PAGE (1948) s). Certain cardiovascular parameters, i.e., cardiac output, total peripheral resistance and renal vascular resistance, have not been documented. The present study was undertaken to determine these parameters and their effects on renal hemodynamics.

Abstract: UMMARY Six modes of administering atropine and neostigmine to reverse the effect of d-tubocurarine were studied with a view to the effect on die heart rate and rhythm. The steadiest heart rates were obtained when atropine and neostigmine together were injected slowly, or when neostigmine preceded atropine. If atropine was given 2 minutes after neostigmine, arrhythmia occurred more frequendy dian in die other groups. In all groups studied die heart rate fell below die base line within 20 minutes and remained Uiere dirough the entire subsequent observation period. The fall in heart rate was more pronounced in patients with rapid cardiac action. ZUSAMMENFASSUNG Sechs Arten der Verabreichung von Atropin und Neostigmin zur Umkehr der d-Tubocurarin-Wirkung wurden in Bezug auf ihren Effekt auf Herzfre-quenz und -rhythmus untersucht. Die konstantesten Herzfrequenzen wurden erzielt, wenn Atropin und Neostigmin zusammen langsam injiziert wurden, oder wenn Neostigmin vor Atropin verabreicht wurde. Wenn Atropin zwei Minuten nach Neostigmin gegeben wurde, traten Arrhythmien haufiger auf als in den anderen Gruppen. Bei alien untersuchten Gruppen fiel die Herz-frequenz innerhalb von zwanzig Minuten unter den Ausgangswert und ver-blieb dort wahrend der gesamten nachfolgenden Beobachtungsperiode. Der Abfall der Herzfrequenz war bei Patienten mit tachycarder Ausgangslage starker ausgepragt.

Abstract: SUMMARY Plasma lidocaine levels were studied in epileptics and control subjects after slow intravenous injection of 2 mg/kg. Two experiments were made on the epileptics: one when they were on their usual antiepileptic and/or sedative drugs and the other after phenobarbital treatment of 4 weeks' duration. The plasma lidocaine levels were somewhat lower in the epileptics than in the control subjects at 30 and 60 minutes after starting the lidocaine injection, and the differences were statistically significant. Phenobarbital treatment did not cause further lowering in the plasma lidocaine levels of the epileptics. It is concluded that patients who have taken phenobarbital or some other potential inducers of the microsomal drug-metabolizing enzymes may have a somewhat increased tolerance to the systemic effects of cumulative doses of licocaine. ZUSAMMENFASSUNG Bei Epileptikern und Kontrollpersonen wurden die Plasmakonzentrationen von Lidocain nach langsamer i.v. Injektion von 2 mg/kg untersucht. Bei den Epileptikern wurden zwei Untersuchungen angestellt: Eine unter der iiblichen antiepileptischen und/oder sedativen Therapie, die andere nach 4-wochiger Phenobarbitalbehandlung. 30 und 60 Minuten nach dem Beginn der Lido-caininjektion waren die Plasmakonzentrationen bei den Epileptikern etwas niedriger als bei den Kontrollpersonen; die Unterschiede waren statistisch signifikant. Die Phenobarbitalbehandlung fiihrte nicht zu einer weiteren Ver-minderung der Plasmakonzentrationen von Lidocain bei den Epileptikern. Es kann der Schluss gezogen werden, dass Patienten, die Phenobarbital oder einen anderen potentiellen Anreger der mikrosomalen drogenabbauenden Enzyme eingenommen haben, eine etwas erhohte Toleranz gegen die Systemwirkungen kumulativer Dosen von Lidocain haben durften.

Abstract: SUMMARY A total of 114 patients were anesthetized for pelvic laparoscopy, using halo-thane, nitrous oxide, oxygen sequence. Endotracheal intubation and mechanically controlled ventilation were employed. No major surgical or anesthesia complications were observed. The technique, indications and contra-indica-tions of laparoscopy are detailed. Acid-base parameters were determined on 11 patients showing an average rise of 9.28 mm Hg in arterial carbon dioxide tension, in spite of vigorous hyperventilation. ZUSAMMENFASSUNG 114 Patienten wurden fur Beckenlaparoskopien mit der {Combination Halothan-Lachgas-Sauerstoff narkotisiert. Endotracheale Intubation und mechanisch kontrollierte Beatmung kamen zur Anwendung. Es wurden keine grosseren operativen oder Anaesthesiekomplikationen beobachtet. Technik, Indikationen und Kontraindikationen der Laparoskopie werden im Einzelnen dargelegt. Bei 11 Patienten wurden die Saure-Basen-Parameter bestimmt, wo-bei sich ein durchschnitdicher Anstieg von 9,28 mm Hg in der arteriellen Kohlensaurespannung, trotz kraftiger Hyperventilation, ergeben hat.

Abstract: SUMMARY Sixteen patients scheduled for open-heart surgery were studied before, during and after cardiopulmonary bypass. Measurements were made of oxygen uptake, cardiac output, physiological dead space, blood-gas tension, acid balance, peripheral vascular resistance and lung-chest compliance. Anaesthesia was maintained with fentanyl and small concentrations of halothane. Oxygen uptake was higher than predicted values and was not influenced by anaesthesia and controlled ventilation. The uptake after cardiopulmonary bypass did not show any significant changes in comparison with preperfusion values. However, there was a significant increase at the end of anaesthesia and surgery. This can probably be attributed to the anaesthetic technique used with the light level of analgesia at the end of the procedures. Cardiac output showed only minor variations with no significant changes associated with the surgical procedures. Arterial mean blood pressure was stable with only minor variations within the range of 64-80 mm Hg as measured before, during and after bypass. Blood gases showed only small variations and buffer-base did not undergo any important changes. Alveolo-arterial oxygen tension differences varied between 370-439 mm Hg, corresponding to a venous admixture of 18-24 per cent. There were no significant changes between the different periods of investigation. When calculating alveolar ventilation, physiological dead space and VD/VT ratios, an equation for the correction of the influence of venous admixture on Pcoa was taken into account. Significantly lower Pcog levels for all periods of determinations as compared with actually measured values were obtained. The values for VA, VD and VD/VT were significantly lower immediately before and after bypass as compared with non-corrected values. The VD/VT ratios were virtually constant, varying between 0.47 and 0.5. Peripheral resistance did not change before and after bypass. During bypass there was a significant increase in peripheral resistance in the hypothermia group. Volume-pressure changes of the lungs and chest varied between 27 and 39 ml/cm H20 with the highest values when the chest was open. At the end of anaesthesia there was a significant decrease in compliance. This decrease was not associated with an increase in alveolo-arterial oxygen-tension differences or on an increased venous admixture. In two patients, the decrease was probably due to left-heart failure. In the other patients, the decrease might be explained by increased muscular tonus, where a contributory factor might have been the use of fentanyl. In comparison with our earlier experience of anaesthesia, where halodiane was the principal agent, the use of more analgesics and less halothane seems to have certain clinical advantages as regard a more stable circulation. However, there were no differences of major importance as compared with our previous findings. It is our opinion that low concentrations of inspired halothane provide a smooth and balanced anaesthesia level, counteracting to a certain extent peripheral vasoconstriction, as demonstrated in this series. The increased oxygen uptake observed at the end of the procedures in this series may be a disadvantage, as it is associated with a higher demand on circulatory and respiratory reserves. ZUSAMMENFASSUNG 16 Patienten, die Operationen am offenen Herzen unterzogen werden sollten, wurden vor, wahrend und nach kardiopulmonalem Bypass untersucht. Gemessen wurden: Sauerstoffaufnahme, Schlagvolumen, physiologischer Totraum, Blutgasspannung, Saure-Basen-Verhaltnisse, peripherer Gefass-widerstand und Lungen-Thorax-Compliance. Die Narkose wurde mit Fentanyl und Halothan in geringen Konzentrationen aufrecht erhalten. Die Sauerstoffaufnahme war hoher als die erwarteten Werte, und wurde von der Narkose und der kontrollierten Beatmung nicht beeinflusst. Die Sauerstoffaufnahme nach kardiopulmonalem Bypass zeigte, verglichen mit den Werten vor der Perfusion, keine signifikanten Anderungen. Am Ende der Operation und Narkose wurde allerdings ein bedeutsamer Anstieg beobachtet. Dieser ist aber wahrscheinlich der Anaesthesiemethode und der leichten Analgesie am Ende des Eingriffes zuzuschreiben. Das Schlagvolumen zeigte nur geringe Anderungen im Zusam-menhang mit dem chirurgischen Eingriff. Der arterielle Mitteldruck war konstant, abgesehen von kleinen Abweichungen im Bereich von 64-80 mm Hg vor, wahrend und nach dem Bypass. Ebenso waren die Blutgaswerte und BufFer-Basen nur geringfiigigen Anderungen unterworfen. Die alveolar-arteriellen Sauerstoffspannungsdifferenzen bewegten sich zwischen 370-439 mm Hg, entsprechend einer venosen Beimischung von 18-24%. Zwischen den einzelnen Untersuchungen traten keine wesentlichen Anderungen auf. Beim Berechnen der alveolaren Beluftung, des physiologischen Totraumes und des VD/VT-Verhaltnisses wurde eine Gleichung angewendet, um den Einfluss der venosen Beimischung auf den Pco2_Wert zu korrigieren. Wir erhielten, im Vergleich zu den tatsachlich gemessenen Werten, signifikant niedrigere Pcc>2 Werte fur alle Bestimmungszeitraume. Die Werte fiir VA, VD und VD/VT waren unmittel-bar vor und nach dem Bypass signifikant niedriger als bei den nichtkorrigierten Werten. Die VD/VT-Quotienten blieben praktisch konstant, und zwar zwischen 0,47 und 0,5. Der periphere Wiederstand anderte sich weder vor noch nach dem Bypass. Wahrend des Bypass wurde ein deutlicher Anstieg des peripheren Wiederstandes in der Hypodiermiegruppe beobachtet. Die Volumen-Druck-Anderungen von Lunge und Thorax bewegten sich zwischen 27 und 39 ml/cm H20, wobei die hochsten Werte bei geoffnetem Brustkorb erreicht wurden. Am Ende der Narkose kam es zu einer signinkanten Abnahme der Compliance. Diese Abnahme stand aber in keinem Zusammenhang mit einer Zunahme der alveolar-arteriellen Sauerstoffspannungsdifferenz oder mit einer Erhohung der venosen Beimischung. Bei zwei Patienten war die Abnahme wahrscheinlich durch ein Linksherzversagen bedingt. Bei anderen Patienten konnte dies wiederum durch einen verstarkten Muskeltonus erklart werden. Ein weiterer zusatzlicher Faktor konnte auch die Verwendung von Fentanyl gewesen sein. Verglichen mit unseren fruheren Anaesthesieerfahrungen, bei denen hauptsach-lich Halothan verwendet wurde, scheint der Gebrauch von mehr Analgetika und weniger Halothan gewisse klinische Vorteile in Bezug auf einen stabileren Kreislauf zu bringen. Es wurden jedoch keine Unterschiede von gesteigerter Bedeutung in Vergleich zu fruheren Ergebnissen gefunden. Wir sind der Mei-nung, dass niedrige Konzentrationen von Halothan im Inhalationsgemisch einen gleichmassigen Narkosespiegel gewahrleisten und bis zu einem gewissen Grad der peripheren Vasokonstriktion entgegenwirken, wie dies in den vor-liegenden Untersuchungen gezeigt wurde. Die vermehrte Sauerstoffaufnahme, die am Ende des Eingriffes in unserer Untersuchungsserie beobachtet wurde, mag ein Nachteil sein, da sie mit hoheren Anforderungen an die zirkulatorischen und respiratorischen Reserven verbunden ist.

Abstract: SUMMARY The effect on the nitrogen balance, the nutritive value and the excretion of urea and a-amino nitrogen of various amino acid mixtures infused intravenously to healthy volunteers was studied. All solutions contained the essential amino acids in proportions suggested by ROSE (1949)34 or by SWENDSEID and DUNN (1956)48. They also contained 1-histidine and 1-arginine. To this basic content were added four or five of the amino acids 1-alanine, 1-arginine, 1-aspartic acid, 1-glutamic acid, glycine or 1-proline in different mutual proportions. In the first series of experiments the entire quantity of 1-amino acids corresponded to 10 g of nitrogen per day for two consecutive days. In the second part of these studies different quantities of one of these solutions were infused. In the first series, no significant differences could be detected between the effect of the different solutions. In the second series, the groups receiving 10 and 14 g of amino acid nitrogen per day showed a higher nutritive value than the groups receiving 6 and 8 g per day. From the reported results and from results previously published by other investigators and by the present author and co-workers, it was concluded that the following amino acids should be suitable for parenteral nitrogen nutrition (beside the essential ones): 1-arginine, 1-histidine, 1-alanine, 1-proline, 1-aspartic acid and glycine. The highest nitrogen balance was achieved by use of a dose of 10 g of amino acid nitrogen. ZUSAMMENFASSUNG An freiwilligen gesunden Versuchspersonen wurde nach intravenoser Infusion verschiedener Aminosauregemische die Wirkung auf die Stickstoffbilanz, der nutritive Wert, sowie die Ausscheidung von Harnstoff- und a-Aminostick-stofT untersucht. Alle Losungen enthielten die essentiellen Aminosauren entwe-der in dem von ROSE (1949)84 oder von SWENDSEID und DUNN (1956)48 vorge-schlagenen Verhaltnis. Ferner endiielten sie auch 1-Histidin und 1-Arginin. Diesen Basislosungen wurden 4 oder 5 der folgenden Aminosauren in verschie-denen Konzentrationen zugesetzt: 1-Alanin, 1-Arginin, 1-Asparaginsaure, 1-Glu-taminsaure, Glyzin und 1-Prolin. In der ersten Untersuchungsserie entsprach die gesamte Menge der 1-Aminosauren 10 g Stickstoff pro Tag wfirend zwei aufeinanderfolgender Tage. Im zweiten Teil der Untersuchung wurden ver-schiedene Mengen einer dieser Losungen infundiert. In der ersten Serie konnten keine signifikanten Unterschiede in der Wirkung der verschiedenen Losungen festgestellt werden. In der zweiten Versuchsserie ergab sich bei den Gruppen, die 10 und 14 g Aminosaurestickstoff pro Tag er-halten hatten, ein hoherer nutritiver Effekt als bei der Gruppe, die 6 und 8 g pro Tag erhalten hatten. Aus den hier berichteten Resultaten und aus den Ergebnissen, die teils von anderen Untersuchern, teils von uns selbst bereits publiziert wurden, kann ge-schlossen werden, dass die nachstehend genannten Aminosauren geeignet fur die parenterale Ernahrung (neben den essentiellen Aminosauren) sind: 1-Arginin, 1-Histidin, 1-Alanin, 1-Prolin, 1-Asparaginsaure und Glyzin. Abstract: SUMMARY The relationship between total blood volume (TBV) and stroke volume (SV) before and after pulmonary surgery was studied in two different ways. In five patients, 15 simultaneous estimations of TBV and SV were performed before and one and three days after operation. In another five patients, SV was determined before and after transfusion of 500 ml ACD-adenine blood. A close relationship was found between the changes in TBV and SV. The decrease in TBV was greater than estimated clinically. A considerable deficit in circulating albumin after operation was revealed. The findings point to the necessity of a more intense and also differentiated volume substitution under the present circumstances. ZUSAMMENFASSUNG Die Beziehung zwischen Gesamtblutvolumen (TBV) und Schlagvolumen (SV) vor und nach Lungenoperationen wurde auf zwei verschiedene Arten untersucht. Bei 5 Patienten wurden vor, ein und drei Tage nach der Operation 15 gleichzeitige Bestimmungen des TBV und SV durchgefuhrt. Bei 5 anderen Patienten wurde das SV vor und nach Transfusion von 500 ml ACD-Adenin-Blut bestimmt. Es fand sich eine enge Beziehung zwischen der Veranderung von TBV und SV. Die Abnahme des TBV war grosser als die klinisch ge-schatzte. Postoperativ zeigte sich ein betrachtliches Defizit an zirkulierendem Albumin. Die Untersuchungen unterstreichen die Notwendigkeit einer krSfti-geren und auch difFerenzierteren Volumensubstitution unter den gegebenen Verhaltnissen.

Complete elimination of pain reduces, e.g., the muscular work done by the mother (metabolic acidosis), and results in decreased hyperventilation (FISHER and PRYS-ROBERTS (1968)l). It is possible that these changes have a beneficial effect on the foetus, e.g. by reducing acidosis during delivery (SALING ( 1966) 4).

Abstract: SUMMARY The circulatory effects of oxygen administration were studied before and early after pulmonary surgery. Oxygen administration did not significantly influence the arteriovenous oxygen difference or left atrial pressure. The mechanisms contributing to an observed postoperative increase in pulmonary vascular resistance (PVR) were investigated. There was no correlation between variations in arterial oxygen tension and in PVR during air breathing, but a consistent decrease in PVR was found on oxygen breathing. The observations indicate that hypoxic pulmonary vasoconstriction as well as other factors contribute to the postoperative increase in PVR. The consequence of the observed increase in PVR is further that die diastolic pulmonary arterial pressure cannot be used as an index of the left atrial pressure—not even during oxygen breathing. ZUSAMMENFASSUNG Die Wirkung von Sauerstoffzufuhr auf den Kreislauf wurde vor und in einem fruhen Stadium nach Lungenoperationen untersucht. Die Zufuhr von Sauerstoff beeinflusste nicht signifikant die arterio-venose Sauerstoffdifferenz oder den Druck im linkem Vorhof. Die Mechanismen, die zum beobachteten postoperativen Anstieg der pulmonalen vascularen Resistenz (PVR) beitragen, wurden untersucht. Es bestand keine Korrelation zwischen Veranderungen des arteriellen SauerstofTdruckes und der PVR wahrend Luftatmung; bei Sauer-stoffatmung fand man dagegen stets einen Abfall der PVR. Diese Beobachtun-gen deuten darauf hin, dass sowohl hypoxische Verengung der Lungengefasse als auch andere Faktoren zum postoperativen Anstieg der PVR beitragen. Die Schlussfolgerung aus dem beobachteten Anstieg der PVR ist ferner, dass der diastolische Druck in der Arteria pulmonalis nicht als Index des linksseitigen Vorhofdruckes - auch nicht unter Sauerstoffatmung - verwendet werden kann.

Abstract: SUMMARY 1 Propanidid (Epontol®), like methohexital (Brietal®), has a direct, cardio-depressant action on the isolated heart. The negative chronotropic and inotropic actions of propanidid are about 0.6 times that of methohexital (whereas the anaesthetic potency of propanidid is about one third of that of methohexital). 2 The Walton-Brodie strain-gauge arch technique for recording changes in the myocardial contractile force was compared with an intraventricular balloon technique. Abstract: Local analgesic agents have long been used to provide analgesia during labour. Lumbar epidural, caudal epidural and paracervical blocks are used commonly to-day. In the last 15 years, lignocaine has held a prominent place in regional analgesia. The principal disadvantage of lignocaine has been that the duration of action, even when administered with adrenaline, is relatively short, somewhere between 60 and 90 minutes. The introduction of continuous techniques has improved the situation, but the phenomenon of tachyphylaxis soon became apparent. However, since the introduction of the long-acting analgesic bupivacaine it has been possible to produce pain relief for about 150-200 minutes after each dose, and tachyphylaxis has not been demonstrated. The purpose of this paper is to present the blood concentrations that were found following the use of bupivacaine during lumbar epidural and paracervical block. Whole blood rather than plasma concentrations were analysed. Bupivacaine was found to be present almost entirely in the plasma, either protein-bound or free. A negligible amount enters the red blood cells. The measurement of blood concentrations of local analgesics was carried out by gas chromatography, using a modification of the method described by REYNOLDS and BECKETT. The sensitivity and specificity afforded by gas chromatography was found to be essential in this study.
