Abstract: and if there are no clinical contrais reflex-controlled. The only way the mother can stop pushing is to start panting. Instruct her to "pant like a puppy," and pant with her if necessary. The uterine muscle will continue to bring the baby down gently and allow time for the perineum to stretch. The patient should be encouraged to "bulge" her perineum. This will facilitate delivery of the head and minimize the need for episiotomy. When the perineum is fully distended and ischemic the area has natural anesthesia, and an episiotomy can be done without further anesthetic. All the patient feels is a welcome relief from perineal distention, and a soothing warm bath of amniotic fluid as the baby is born. It is a great feeling! If stitches are inserted immediately, there is little if any need for local anesthetic infiltration of the incision. But after the placenta is delivered, feeling soon returns to the perineum, and local anesthesia is appreciated for the repair. Delivery of the placenta is best done by the mother. Her abdomen is very sensitive now, and if she has no anesthesia, she is fully capable and most willing to cooperate by pushing the placenta out herself, just as she did the baby. It may take as long as half an hour for the placenta to be delivered. Let her examine her baby thoroughly as soon as possible. This is the fulfillment for which she has prepared herself and worked hard. If she wants to breastfeed, let her nurse while still on the delivery table. This stimulates lactation and encourages contraction of the uterus.

Abstract: When Culliver traveled to Laputa, land in the clouds, he jound rapt theorists wandering about constructing useless ideas. And, generally, that is waht we think of theorists. This author, though, says that the-orists in nursing are not the ivory-tower thinkers; they are the nurses who work direcltly with patients. With every patient, she explains, we select an approach, then use, modify, and expand it—whether or not we are conscious of doing so. Because the-ories in a field have a powerful, long-term influence on the direction that field will take, the author pleads that we struggle to make already-existing, implicit nursing theories—such as TLC, jor example—clear and explicit, so that nursing will develop in the direction of more skilled bedside care.

Abstract: Many Spanish-speaking people living in the southwestern states, Hispanos, continue to observe practices similar to those their ancestors brought from their mother country. Because of isolation, even in cities where they tend to live in compounds or barrios, the Spanish language and customs prevail. In the extended family household, the older members guide the younger in many of their practices and decisions. These descendants of the first European explorers on this continent are found throughout the Southwest and are only one group of the Spanish-speaking people. Among the Spanish-speaking, some variation in beliefs occurs, but a common core of traditional practices does exist. It is important for nurses to recognize not only the existenice but also the persistence of the practice of folk medicine and to realize that when the scientific preventive health measures and treatment of illness they teach are incompatible with this system, persons are apt to reject that which is foreign and contrary to their own tradition. Abstract: Students with Autism Spectrum Disorder (ASD) may have a range of healthcare issues in addition to their diagnosis of ASD. Conditions which may exist alongside ASD are called cooccurring or co-morbid conditions, and may include seizures, gastrointestinal problems, sleep problems, anxiety, and attention issues. The school nurse can help your child regulate these issues while at school. School nurses can also help care for those children who have toileting and feeding issues. Even if your child doesn’t have co-occurring conditions, there will likely be other expected and unexpected events that will cause your child to interact with the school nurse.

Abstract: An experiment to provide a stimulating learning environment for students and staff,promote investigative endeavors in nursing, and provide high quality patient care was begun three years ago at the Frances Payne Bolton School of Nursing with the cooperation of the administration and nursing service department of the University Hospitals of Cleveland. This experiment, with partial support from the W. K. Kellogg Foundation, involves changes in relationships, responsibility, and authority for nursing staff of the hospital and faculty of the school of nursing. This first of three papers to be published in consecutive issues of the Journal describes the plan. Future articles will discuss its implementation and the development of new roles for nursing personnel.
