Abstract: Objective To explore pathogens and drug resistance in ventilator-associated pneumonia (VAP), and to provide the corresponding clinical prevention strategies. Method We analyzed 154 VAP patients' clinical data, pathogens distribution and resistance rate of bacilli. Results The incidence of VAP was 36.5%, the aged accounted for 61%. Among all the pathogens caused VAP, 77.93% of sputum culture results were gram-negative bacilli, most of which were Pseudomonas aeruginosa, Acinetobacter baumanii, Chryseobacterium, Pseudomonas maltophilia and Staphylococcus aureus. Most of gram-negative bacilli showed high resistance rate to common used antibiotics. Conclusions Extensive and dynamic monitoring of VAP pathogens and rational use of antibiotics were advocated for controlling VAP effectively.

Abstract: Objective To investigate the immunosuppressive function of extracts of Caesalpinia Sappan L in vitro. Methods In vitro, immunocompetence of lymphocytes, M and Nk cells, taken out from spleen and abdomen fluid of rats after being given extracts of Caesalpinia Sappan L, were examined. The levels of IL-2 in the supernant of mixed lymphocyte culture were checked also. Results Immunocompetence of lymphocytes and M cells were suppressed greatly by ethanol extracts of Caesalpinia Sappan L. Watery extracts of Caesalpinia Sappan L can affect T cells function. Ester extracts of Caesalpinia Sappan L can affect immunocompetence M and Nk cells. Supernant IL-2 in mixed lymphocyte culture dropped in watery and ethanol extracts of Caesalpinia Sappan L groups. Conclusions Different extracts of Caesalpinia Sappan L have defferent immunosuppressive function. Ethanol extract of Caesalpinia Sappan L has the strongest immunosuppressive function.

Abstract: Objective This study aimed at investigating the activity of NFκB and the expression of inducible nitric oxide synthase(iNOS) in lung tissue of acute paraquat poisoned rats.Methods 80 Sprague-Dawley (SD) rats weighing 180 g to 200 g were randomly divided into two experimental groups: Poisoned group and Control group.At 1,3,7,14,21,28 and 35 days after exposure,lung pathological changes of ten rats were observed,NF-κB activity in the lung tissues was assessed by electrophoresis mobility shift assay(EMSA),and expression of iNOS mRNA were evaluated by RT-PCR.Results Compared with that in control group,NF-κB activity in lung tissue of Poisoned group was elevated at the first day,reached the peak at the third day,return to normal level at the 21th day;and expression of iNOS mRNA in lung tissue of Poisoned group markedly increased at the first day,reached the peak at the 7th day,decreased slowly afterwards,return to normal level at the 28th day.Conclusions NF-κB and iNOS could play an important role in lung injury of poisoned rats.

Abstract: Objective To retrospectively analyze the correlation between the prognosis and the lactate clearance rate in severe sepsis patients.Methods The data of 247 severe sepsis patients were col- lected,including the score of APACHEⅡafter admission to ICU,the morbidity of septic shock,the lac- tate clearance rate after 12 hours in ICU and the patients' prognosis.A total of 247 cases were divided in- to survival group and dead group,low lactate clearance rate group and high lactate clearance rate group, and the differences between them were compared for statistical significance.Results There were no differences in age,sex,the score of APACHEⅡafter admission to ICU and the initial blood concentration of lactate(P＞0.05).In survival group,the morbidity of septic shock was significantly less(31.03% vs 83.33%,P＜0.01),but the lactate clearance rate was significantly higher than in dead group[(31.86±16.34)% vs(14.77±10.35)%,P＜0.01];In high lactate clearance rate group,the morbidity of sep- tic shock and the mortality were significantly lower than in the low lactate clearance rate group(29.45% vs 90.48%,P＜0.01;30.67% vs 85.71%,P＜0.01).Conclusion The lactate clearance rate can pre- dict the prognosis of severe sepsis even in early phase.

Abstract: Objective To determine the relationship between C677T polymorphism in methylenetetrahtdrofolate reductase (MTHFR) gene and hemorrhagic stroke in Chinese population.Methods 202 patients with hemorrhagic stroke and 190 controls without cerebrovascular disease were screened by polymerase chain reaction (PCR) and corresponding C677T polymorphism probe hybridization.Results C677T polymorphism was in agreement with Hardy-Weinberg equilibrium.The frequency of 677T allele in patients was more prevalent than that in controls(64 60% vs 51 84%).In comparison with CC homozygote,the presence of TT genotype conferred an odds ratio (OR) for hemorrhagic stroke of 2 648(95%CI:1 507～4 654),the proportion of attributable risk (AR%) was 62 24%.While in comparison with CT heterozygote,TT genotype conferred an OR for hemorrhagic stroke of 1 638(95%CI:1 045～2 566),AR%was 38 94%.Conclusion C677T polymorphism of MTHFR gene may be a genetic risk of hemorrhagic stroke in Chinese population.
