Abstract: Objective. To discuss the implementation of single disease payment by analyzing the situation of clinical single disease in our hospital. Methods. A total of 710 patients with medical insurance single disease or self-supporting inguinal hernia/femoral hernia in 2011 were retrospectively analyzed. Results. The hospital total cost, medicine fee, surgical anesthesia fee, treatment costs, and other fees of medical insurance inguinal hernia or femoral hernia patients with single disease payment management are all less than that of self-supporting patients. There are statistical differences when p is less than 0.05. The five main factors that affect hospital cost orderly are single disease (self-supporting), general anesthesia, double-sided operation, length of stay and laparoscope. Conclusion. Single disease can effectively control the high-speed growth of one part of disease medical costs, but its further development is limited by defects, such as narrow disease coverage area and so on.

Abstract: Objective. To analyze the factors influencing the hospitalization costs of patients undergoing lung cancer surgery, based on the back-propagation (BP) neural network model, and to provide support for controlling the costs effectively. Method. To collect the hospitalization data of 168 cases of patients with lung cancer surgery from January 2010 to December 2013, and then analyze the influencing factors of the different classified hospitalization costs based on the BP neural network. Results. The expenses on drugs and consumables comprise the largest proportion of the total cost of hospitalization, at 34.35% and 23.24% respectively. The length of hospital stay is the most important factor in the hospital costs. The patients with longer hospital stay have higher costs and the difference is significant (P < 0.05). Conclusion. Abstract: Objective. The effect of applying the clinical pathway in vaginal delivery due to full-term premature rupture of fetal membranes is discussed. Methods. Through asynchronous cohort studies, the indices, including the average days in hospital, total hospitalization expenses, patient satisfaction and the coefficient of variation of various indicators of patients with vaginal delivery due to mature premature rupture of membranes, who are treated with and without applying the clinical pathway, etc., are comprehensively and comparatively analyzed. Results. The comparison results of the pathway group and the control group are as follows: the hospitalization time is (3.2±0.6) vs. (3.3±0.6) days, total hospitalization expense is (2817.4±146.2) vs. (2838.7±194.0) Yuan, and the differences have no statistical significance (P>0.05). Abstract: Objective. To understand the trend of 2002–2012 outpatient amounts, to build a time series model forecasting the monthly outpatient amount, and to provide a scientific statistical foundation for the optimization of medical treatment resource allocation. Methods. A seasonal autoregressive integrated moving average (SARIMA) model was built and the outpatient amount in July 2012 to December 2012 was verified by using yearly outpatient data in January 2002 to December 2012 and SPSS18.0 software. Results. The forecast values were consistent with the upward and downward trends of the actual values; the SARIMA(0,1,1)(0,1,1)12 model was the optimal model with the least Normalized Bayesian Information Criteria (BIC) of 13.82 and Mean Absolute Percent Error (MAPE) of 7.70; and the Ljung-Box test (Q18 = 17.93=p = 0.3281 > 0.05) was not statistically significant. Achieving effective supervision of medical insurance and adopting measures for appropriate payment of basic medical insurance under the family physician system, through the analysis of two groups of urban workers in the Community Health Center. Methods. A statistical analysis was made on the settlement data of basic medical insurance for outpatients consisting of urban workers in the Community Health Center, using SPSS and Excel. Result. The administrative departments of Medicare should strengthen the supervision of specific groups, and carry out application of the hybrid medical insurance payment methods, which can effectively improve the use of medical insurance funds, and make the family doctor service more targeted.
