Abstract: Lynch syndrome is a peculiar disease, accounting for 5% of the total burden of colon cancer. Characteristics of this disease are autosomal dominant transmission, early onset, and frequent right colon localization. Diagnostic criteria, aimed to collaborative studies, are based on these features (so called Amsterdam criteria). Lynch syndrome has specific biomolecular features (microsatellite instability); mismatch repair genes have been identified as responsible of this syndrome. Lynch syndrome causes high risk for extracolonic malignancies, particularly for endometrial cancer, supposed to be related to mutation of hMSH2 gene. Another feature of Lynch syndrome tumours is better survival with respect to sporadic counterpart. Genetic test allows identifying the state of mutation carriers and selects the patients to submit to screening. Endoscopic screening has been demonstrated to reduce incidence of colorectal malignancies in this syndrome.

Abstract: Latent autoimmune diabetes in adults (LADA) is a disorder with onset after age 30, insulin independence for at least 6 months after diagnosis, and the presence of circulating pancreatic islet autoantibodies. The prevalence of LADA varies substantially across ethnic groups and ranges approximately from 1% to 10% among patients with type 2 diabetes. In this review we discuss the nomenclature, diagnostic criteria, immunologic and genetic markers, metabolic alterations and therapy of this form of diabetes.

Abstract: Objective(s): In the present study, the nephroprotective and curative effects of aqueous extract of AV on gentamicin-induced tubular toxicity in male Wistar rats were investigated. Antioxidant activity, phenolic and flavonoid contents of AV were also determined. Materials and Methods: In a preclinical study, 60 male Wistar rats were designated into 6 groups of 10 and treated as follows: Group I: Treated with vehicle (distilled water) and kept as control. Group II: Received 300 mg/kg/d AV for three days then, AV plus GM (80 mg/kg; intraperitoneally) for 7 more days and sacrificed. Group III: Received 300 mg/kg AV orally for 10 days, then sacrificed. Group IV: Received GM for 7 days and sacrificed. Group V: Received GM for 7 days and saline for ten days and sacrificed on 17th day. Group VI: Received GM for 7 days, then AV for 10 days and sacrificed. Results: The levels of flavonoid and phenolic compounds in Aloe vera extract were 43±3.8 mg/g (Rutin equivalent) and 80.1±3.8 mg/g (Gallic acid equivalent), respectively. The antioxidant activity of the extract was 38. The intensity of nephrotoxicity in group 2 that received AV 300 mg/kg/d as prophylaxis was not significantly different from the group I and from the group II which treated with 300 mg/kg/d AV without GM. This finding reveals that AV as prophylaxis could potentially attenuate the GM-induced nephrotoxicity. Conclusion: Aole vera significantly protects the renal cells and reduces the severity of tubular damage caused by gentamicin. However, it cannot regenerate tubular damage. © Societa Editrice Universo (SEU).

Abstract: Objective Gabapentin is an antiepileptic drug of new generation that increases brain GABA levels. We report the results of a three-month randomised double-blind placebo-controlled study on the effects of gabapentin in the prophylaxis of patients with migraine meeting the IHS criteria. Patients and methods We treated 63 patients suffering from migraine with or without aura. Patients treated their attack at home using symptomatic drugs and clinical assessment was recorded on a diary. After a washout of 8 week from any other prophylactic treatment, all patients were treated with 1200 mg/day of gabapentin; this is our therapeutic plan: 400 mg/day from 1st to 3rd day, 800 mg/day from 4th to 6th day and 1200 mg/day from 7th day. Results No patients withdrew, gabapentin was well tolerated; adverse events (somnolence, dizziness, tremor, fatigue and ataxia) generally were transient and mild to moderate in severity and in 13 patients (27%) only occurred. At the end of treatment, in such case, we reported a significative reduction of frequency and intensity of migraine in 30 patients treated with gabapentin. Discussion Our observations indicate that gabapentin is well tolerated by patients and that reduces headache frequency and use of symptomatic drugs in both groups. Gabapentin shows to have an effective therapeutic action in the prophylactic treatment of migraine.

Abstract: The neuroendocrine system, which plays an important role in regulation of mood, is dysfunctional in patients suffering from mood disorders. In order to improve the quality of life for patients, additional research is needed to define clinical implications of neuroendocrine dysfunction in mood disorders. It would be important to define which specific hormonal responses that are blunted in affective disorders contribute to mood symptoms and which medications that normalize neuroendocrine function are conditioning the impact of mood symptoms. Consideration and evaluation of endocrine status result important in psychiatric patients, both to ensure proper diagnosis and adequate treatment.
