Abstract: A comprehensive description of a sample of spinal cord injured individuals is provided in order to evaluate the extent to which injury affected their employment and financial situation, their interpersonal relationships and their opinions of themselves. Findings from structured interviews conducted with III patients are summarized and discussed in terms of their implications for several goals. These include (a) gaining increased understanding of the special problems faced by these individuals, (b) providing practitioners with information which can be used to guide treatment choices, and (c) defining new research questions and areas of inquiry.

Abstract: Subjects in experimental group l-viewed a television program and subjects in experimental group 2 a professionally produced film depicting blindness. Attitudes were signficimtiy higher on pasttest in experimental group 2. It was concluded that although the professionalfilm had a positive effect on the attitudes of the second group, the television program depiction of disability did not provide enough information to positively alter attitudes of experimental group 1.

Abstract: A national survey was conducted in order to identify the roles and functions of rehabilitation specialists working in the private sector. Participants consisted of 155 randomly selected members of the National Association of Rehabilitation Professionals in the Private Sector (NARPPS) who employed or worked with an additional 695 rehabilitation specialists, such that, a total of 850 persons participated in the survey. Participants were mailed the Rehabilitation Specialist Task Inventory (RSTI) which was developed for this investigation. A total of 283 persons responded which yielded 254 usable questionnaires which represented responses from 84 graduates of accredited rehabilitation programs, 50 rehabilitation nurses, and 120 other private rehabilitation practitioners. Factor analysis of the RSTl revealed 122 tasks distributed across five major work role categories: (1) Planning and Coordinating Client Services, (2) Business and Office Management, (3) Job Development and Placement, (4) Diagnostic Assessment, and (5) Other Professional Activities. Rational sorting of the items contained in the first three work role categories yielded 15 sub-groupings; six in the first, four in the second, and five in the third category. Data were analyzed for the five work role categories, as well as for each task item, using one-way analysis of variance and Duncan's post hoc tests. Participants were also divided in terms of whether or not they were Certified Rehabilitation Counselors. The t statistic was used to compare responses between CRCs and Non-CRCs on the five work role categories. Results and their implications were discussed for the roles and functions of the three groups of private rehabilitation specialists, as well as for CRCs and Non-CRCs, relative to task importance and frequency of task performance.

Abstract: Traumatic head injury is a topic of growing concern for rehabilitation professionals. Between 30,000 and 50,000 individuals each year are left with disabilities severe enough to preclude a return to normal life following traumatic head injury. Approximately one-half of the head injuries are caused by motor vehicle accidents, the incidence among males is almost twice that among females, and males fifteen to twenty-four years of age incur more head injuries than any other age group. Brain damage following traumatic injury is typically diffuse. Therefore, a unique set of temporary or permanent limitations may be evident, encompassing physical, cognitive-intellectual, and/or personality-emotional functioning. Flexibility in rehabilitation planning is of key importance in considering the complexity of potential limitations and their changeable nature. Professional awareness and training, specialized services and programs, advocacy for program eligibility in established agencies and programs, and individualized assessment and counseling are part of the exciting rehabilitation challenge in returning the head injured to productive lives.

Abstract: The Navajo Tribe is the largest and fastest growing Indian Tribe in the Continental United States. Somewhere near 180,000 Navajo people reside on the Navajo reservation located in three states: New Mexico, Arizona, and Utah. Tens of thousands more Navajo people live in cities throughout the United States. Ability to work effectively with more traditional Navajo people will require of the counselor some background and respect for the ancient language and culture of the client.

Abstract: Extends the concept of job placement to a theory of career counseling for disabled adults. The history of placement in rehabilitation is reviewed and several theories of career counseling are considered. A model of career development for disabled adults is presented that includes implications for rehabilitation counselors in its components of self-understanding, self-acceptance and acceptance of disability, career exploration, career self-concept, career selection, and career reevaluation.

Abstract: Alcoholism, America's third leading disabling condition, is discussed in regard to (a) its relation to other disabilities relevant to rehabilitation counseling, (b) its more critical symptoms, (c) intervention and confrontation, and (d) signs of recovery. Based on a theoretical “hourglass” model of alcoholism, aids for diagnosis and treatment, as well as recommendations for counselor training, are presented.

Abstract: Disabled older persons are a population traditionally excluded from rehabilitation services because they were considered unfeasible for substantial gainful employment. The emerging focus on independent living services, along with increases in the average life span and changes in policies such as mandatory retirement age, has forced a reexamination of the potential for rehabilitation of this population. This article examines what is known about disabled older people, including both medical and psychological aspects, and discusses the implications of these conditions for rehabilitation planning. The definition of the state of the art in working with older persons includes attention in counseling practices and concludes with resources for additional information.

Abstract: The profession of vocational rehabilitation counseling has a definite mission in the rehabilitation and return to work of chronic pain syndrome patients. Research and descriptive/theoretical articles are much needed to enable rehabilitation counselors to more effectively work with this population. The rehabilitation counselor has a valid and vital role as a member of a multidisciplinary pain,cmanagement team. The population of pain syndrome patients are complex and difficult to manage, but are certainly deserving of the very best of rehabilitation services. Discussion is made of the rehabilitation population of chronic pain syndrome patients, including the identified patient, hislher family and social system, goals of behavioral pain management, and specific counseling interventions.

Abstract: Following a discussion of the importance of philosophy to the field of rehabilitation, the practical significance of philosophical issues germane to rehabilitation clients, counselors, and agencies is presented. Examples and illustrations of the relevance of philosophy across these three areas, with foci on the selected philosophical issues of awareness, reality, truth and meaning, responsibility and choice, commitment, and freedom, are included. The argument is made that philosophical enlightenment is sine qua non to enlightened decisions regarding rehabilitation's future.

Abstract: In the fall of 1982, 42 adolescents with epilepsy referred for neuropsychological testing were followed 6.44 years after testing (4.55 years after high school) with respect to vocational outcome. Forty-three percent of the sample were either employed or involved in continuing education (N = 18); 57% reported no competitive employment (N = 24). Thirty-one percent were receiving federal living subsidies. Thirty-six percent (N = 15) had received state vocational rehabilitation (VR) services, with a placement rate of 13% (N = 2). The low VR placement is consistent with recent literature. The sample demonstrated lowered performance on an expanded Halstead-Reitan Battery. Significant t-test differences (P< .01) were observed between working and non-working portions of the sample on the Weschler Adult Intelligence Scale and on percentage of neuropsychological tests outside normal limits. Similar results have been reported with adult epileptics seen at this Center. A discussion of priorities in rehabilitation planning and of the use of neuropsychological testing to aid in identifying vocationally “at risk” youth is presented.

Abstract: Levels of attractiveness and disability were varied to assess their effects on client ratings by students in the helping professions. Students in counseling, rehabilitation, and social work served as subjects. The same female “client” was photographed in four different conditions. Each subject received a photograph of the client in one of four conditions, the appropriate background sketch, and listening to a brief audiotape of an interview between the client and a “helping professional,” subjects rated the client on 22 bipolar adjectives which factored into six orthogonal factors: Social Attractiveness; Prognosis; Physical Attractiveness; Personal Evaluation; Severity of the Presenting Problem; and Adjustment. Multivariate anova indicated significant overall main effects and interactions, and that a female client was rated as more attractive in the disabled condition.

Abstract: Legally blind clients present a unique challenge to the rehabilitation counselor in that visual impairment includes a host of conditions that may arise from disease, genetic factors, and accidents that affect a very important sense organ. Psychosocial and attitudinal aspects can be barriers or they can facilitate adjustment to the disability. Many devices and methods of service have enhanced the rehabilitation prospects of visually impaired persons. Technological advances, programs for independent living and vocational development, the expertise of the rehabilitation counselor, and many service organizations provide the potential to meet the needs of this clientele.

Implications of the findings are discussed, and suggestions for future research are given.

Abstract: This paper reviews the industrial relations program of the Kentucky Bureau of Vocational Rehabilitation. This program represents a serviceoriented approach to job development. It is based on the premise that the primary goal of vocational rehabilitation is to get work-ready individuals with handicaps into appropriate employment. To achieve this goal, the backing and support of business and industry is needed. One effective means of obtaining and maintaining this support is through the provision of essential cost-free services to the business community. This paper reviews the factors and assumptions that lead to the development of this program and discusses the cost-free consultant/educational and direct services offered to business and industry through the Industrial Relations Manager. Through this service-oriented program of job development, Kentucky Vocational Rehabilitation has been able to: (1) develop and maintain an excellent working relationship with business and industry across the state, (2) provide meaningful and cost-beneficial services that were not being provided by any other state agency or organization, (3) break-down many of the man-made barriers to successful employment of individuals with handicaps, (4) provide exceptional follow-along services, and (5) increase the rehabilitation success rate.

Abstract: Cross-cultural counseling is an emerging field in counseling and has implications for the subcultures of various disability groups and for international rehabilitation. Empathy, the self-conscious awareness of the consciousness of others, which is important in counseling, requires that counselors know and understand the culture of those that differ from themselves. This article discusses rehabilitation counseling and the disabled Hispanic. It presents traditional Hispanic cultural attributes, language, extended family, family roles, machismo, dignidad, respeto, personalismo, la palabra, folk medicine, health problems of migrant and seasonal farm workers, and their implications for rehabilitation counseling. The writers qualify these variables due to the impact of acculturation, socioeconomic status, and the diversity within the Hispanic community. They also consider the dichotomy of the human universal, etic approaches vs. the cultural specific, emic approaches and the implications of each. The impact of the Hispanic cultural attributes on function, psychosocial aspects and rehabilitation planning are dealt with, e.g., reactions to medical and psychological examinations. Counseling in general and specific counseling theories, client-centered, behavioristic, and insightful, are related to the Hispanic culture in order to support an adaptive; culturally sensitive counseling approach.

Abstract: Developmental disabilities are reviewed from the standpoint of definition, service systems, and interventions. Definitions from relevant laws outline the characteristics of the population and of the services necessary to meet rehabilitative needs. Intensive, comprehensive, coordinated, on-going delivery mechanisms are emphasized, including vocational, residential, and generic services. Careful management of alternate settings and altered expectations and attitudes can maximize the developmentally disabled person's participation in the habilitation process and in society.

Abstract: The job-related difficulties of those with epilepsy are well documented in the literature. These clients with epilepsy have traditionally had difficulty benefiting from contact with the state-federal rehabilitation systems. The key to successful placement in epilepsy rehabilitation is comprehensive evaluation as a baseline. The neurological impairment that can be associated with the disability makes this even more imperative. The major thrust of this article is a description of the kinds of data that need to be collected and counseling issues the counselor may need to address. Following comprehensive evaluation and active involvement in a rehabilitation plan, the person with epilepsy may be among the more employable individuals entering the rehabilitation system. Various programs specializing in epilepsy rehabilitation have achieved placement rates of 50% and higher.

Finally, a number of suggestions were offered to the rehabilitation service provider assisting cancer patients in an effort to optimize the cancer patient's rehabilitation.

Abstract: Historically master's level programs in rehabilitation counseling produced trained graduates, many of whom became employed in state Divisions of Vocational Rehabilitation. In recent years an increased number of these experienced public sector counselors have switched from public to private sector employment which offers better salary incentives, training opportunities, smaller caseloads and a new sense of professional esteem. Some difficulty in job adjustment was reported by the seven counselors interviewed because of the differing philosophies and practices found in private rehabilitation companies. Despite these detriments, the trend away from the public sector is expected to continue as government budgetary cutbacks progress. Public agencies may need to develop and re-emphasize employment incentives to stall the exodus of trained counselors from the public to private sector of rehabilitation.

Abstract: The focus of this article encompasses an exploration of the psychosocial aspects of sexuality and the spinal cord injured person, individual adjustment strategies, and the role of the rehabilitation counselor in facilitating sexual adjustment through counseling. Further, implications for counselor training are also considered. It was pointed out that although the rehabilitation counselor's role is debated in the area of incorporating discussions of sexual concerns in the counseling process, professionals in rehabilitation are increasingly reporting the importance of relating to this area of life adjustment. A primary conclusion related to the importance of sexual adjustment of spinal cord injured persons as a significant element in the overall comprehensive rehabilitation process.

Abstract: Industrially injured clients represent an array of disabilities that impose varying degrees of functional limitations that affect subsequent vocational rehabilitation planning. A common feature among this client population is that their medical and vocational needs are underwritten frequently by insurance coverage policies. Regardless of the specialized knowledge required to adequately serve specific disability types, counselors must understand the insurance system operating in these cases in order to plan and implement effective vocational rehabilitation services. This article provides information for counselors working with industrially injured clients within an insurance context.

Using a case study of a young Japanese woman, the article presents a paradigm for use in assessing problems presented that have a cross-cultural basis. The focus of the paradigm is on environmental and value characteristics as perceived by the client and the counselor.
