Abstract: Objective To explore mirror therapy on upper extremity function in patients with cerebral apoplexy hemiplegia and hand function. Methods Forty patients with of stroke hemiplegia were divided into control group and rehabilitation group. Control group had routine rehabilitation training,including the good limb position puts,the passive joints,muscle strength training,balance training,walk training therapy,homework therapy,physical factor and so on. Rehabilitation group added mirror therapy besides those routine trainings. Both groups had finished 4 weeks treatment,before treatment and 2 weeks and 4 weeks after treatment for the Fugl-Mayer scale,STEF and Barthel index evaluation. Results Two and four weeks after rehabilitation treatment group FMA score were 36. 37 ± 16. 36,44. 36 ± 14. 58,STEF score were 58. 75 ± 7. 57,73. 32 ± 8. 81,BI score were 57. 33 ± 15. 26,67. 50 ± 13. 48,score before the treatment and control group in the two groups after treatment were increased,and the rehabilitation group was better than control group( P < 0. 05). Conclusion Mirror therapy can improve the stroke patients with hemiplegia of upper limb function and hand function,and to improve their ability of daily life activities has a role in promoting.

Abstract: Objective To observe the efficacy,safety in treatment with icotinib as first- line therapy with advanced non-small cell lung cancer(NSCLC). Methods Twenty- one patients with advanced NSCLC were enrolled in the study. All the patients treated with icotinib 125 mg,po,three times a day. The response rate,clinical symptom and toxicity were observed until progression or unacceptable toxicity. Results No patient appeared complete response,nine patients appeared partial response,nine patients appeared stable disease and three patients had progressive disease. The objective response rate was42. 9 %,and the disease control rate was 85. 7 %. Median Time to Progress was 7. 2 months. The main toxicity were skin rash and diarrhea. Conclusion Icotinib as first- ling therapy is an effective in the treatmean of advanced non- small cell lung cancer. All of patients are well- tolerated.

Abstract: Objective To investigate the meaning of soluble triggering receptor expressed on myeloid cells-1( sTREM-1) in the diagnosis of community acquired pneumonia( CAP). Methods Serum sTREM- 1,CRP,Fbg,D- Dimer were determined in sixty patients with community acquired pneumonia and twenty- four healthy volunteers controls. CAP group patients were divided into non- severe pneumonia group( 44 cases) and severe pneumonia group( 16 cases) by IDSA /ATS severe CAP criteria,divided into mild mid range pneumonia group( 50 cases) and grossly pneumonia group( 10 cases) by CURB- 65 criteria,divided into low risk pneumonia group( 42 cases) and high risk pneumonia group( 18 cases)by PSI score. Results Level of serum sTREM- 1 was significant higher in pneumonia group than in the normal control group. Level of serum CRP,Fbg,D- Dimer were higher in pneumonia group than in the normal control group,too. Level of serum sTREM- 1 was higher in severe pneumonia group than in non- severe pneumonia group [( 122. 34 ± 24. 14) ng /L vs( 44. 74 ± 9. 22) ng / L,P 0. 05]. Level of serum sTREM- 1 was higher in grossly pneumonia group than in mild midrange pneumonia group [( 120. 21 ± 39. 24) ng / L vs( 54. 48 ± 9. 87) ng / L,P 0. 05]. Level of serum sTREM- 1was higher in high risk pneumonia group than in low risk pneumonia group [( 126. 06 ± 21. 23) ng / L vs( 39. 45 ± 8. 08)ng / L,P 0. 05]. For ROC curve in diagnosis of CAP,when the cut- off value of sTREM- 1 was set at 15. 79 ng / L,the sensitivity was 63. 30%,specificity was 83. 30%,accuracy was 71. 40%. Conclusion Serum sTREM- 1 level is higher in CAP. Detection of serum sTREM- 1 is helpful in the diagnosis of CAP. Serum sTREM- 1 level could help to evaluate the severity of infection.

Abstract: Objective To evaluate the value of Magnetic Resonance perfusion weighted imaging in diagnosis of glioma recurrence and cerebral radiation injuries by the study of the patients with glioma after surgery combined with radiotherapy. Methods We selected fifty-two postoperative patients with radiotherapy,and underwent routine MR and PWI to the new abnormal enhancing lesions,we measured the value of rCBVmax,compared analyzing the result of imaging and histology, and evaluated the value of PWI in diagnosis of glioma recurrence and cerebral radiation injuries. Results Thirty cases were glioma recurrence and 22 were cerebral radiation injuries. The rCBVmax were statistically different between recurrent glioma( 2. 43 ± 1. 52) and radionecrosis( 0. 67 ± 0. 44). The diagnostic accuracy were statistically different between routine MR( 38. 5%,20 /52) and PWI( 82. 7%,42 /52). Conclusion MR PWI can improve the diagnostic accuracy of glioma recurrence and cerebral radiation injuries,the value of rCBVmax has an important role in the differential diagnosis.

Abstract: Obsjective To investigate the relationship of the high expression of sorcin with multidrug resistance in lung cancer cell NCI- H446. Methods Lung cancer cell NCI- H446 was object of study,set up cell line NCI- H446 / CDDP through cisplatin; Using antisense nucleic acid technology inhibited the expression of sorcin in NCI- H446 / CDDP cell,building antisense nucleic acid group,at same time setting up blank control group. RT- PCR and Western- blot were used to analyze the expression of sorcin at mRNA and protein levels in every cell group. MTT was used to three groups cell( NCI- H446 cell group,antisense nucleic acid group,NCI- H446/CDDP group) survival rate in DDP,VCR,ADM,VP- 16. Pearson correlation was used to analyze the correlation of the high expression of sorcin with multidrug resistance in lung cancer cell NCI- H446. Results Comparison with the expression mRNA and protein of sorcin in every groups,the result showed: comparing NCI- H446 / CDDP group with blank control group has no statistical significance( P 0. 05),Intercomparison in NCI- H446 cell group,antisense nucleic acid group,NCI- H446 / CDDP group has statistical significance( P 0. 05). The expression mRNA and protein of sorcin in NCI- H446/CDDP were obviously higher than it in NCI-H446 cell,9. 81 and 7. 67 times respectively. The result of IC50: NCI- H446 / CDDP group antisense nucleic acid group NCI- H446 cell group,has statistical significance( P 0. 05). Pearson correlation to point out: the expression of sorcin in mRNA and protein was negatively correlated to IC50of the group cells in different chemotherapy drugs. Conclusion The high expression of sorcin is related to multidrug resistance in lung cancer cell,Inhibition of its expression can reverse the drug resistance of lung cancer.

Abstract: Postoperative delirium and cognitive dysfunction are common manifestations of acute brain dysfunction,occurring in up to 70% of post- surgical patients. The development of postoperative delirium and postoperative cognitive dysfunction have long- term consequences,such as higher morbidity and mortality and increased hospital stay,and it increases the risk of dependency and institutionalisation. Delirium presents clinically with differing subtypes ranging from hyperactive to hypoactive. Despite the relevance of these cognitive disorders,the specific aetiology is still unknown,and there are many factors that have been associated with its development. In many forms of delirium,the brain's reaction to a peripheral inflammatory process is considered to be a pathophysiological key element and the aged brain seems to react more markedly to a peripheral inflammatory stimulus than a younger brain. The risk factors for delirium are categorised as predisposing or precipitating factors. In the presence of many predisposing factors,well documented predisposing factors are age,medical comorbidities,cognitive,functional,visual and hearing impairment and institutional residence. Important precipitating factors apart from surgery are admission to an ICU,anticholinergic drugs,alcohol or drug withdrawal,infections,iatrogenic complications,metabolic derangements,pain and the exposure to analgesics( opioids) and hypnotics( benzodiazepines). Several bedside instruments are available for the routine ward and ICU setting. The Confusion Assessment Method( CAM) and a version specifically developed for the intensive care setting( CAM- ICU) have emerged as a standard.Non- pharmacological multicomponent strategies and pharmacological prophylaxis may be useful to reduce the incidence of postoperative delirium. Perioperative administration of haloperidol has been shown to reduce the severity and the incidence of delirium. The multicomponent interventions for prevention and treatment have been shown to reduce the incidence and severity of episodes.

Abstract: Objective To discuss the independent prognostic factors influencing long-term survival rate of the patients with colon cancer after radical resection.Methods Univariate and Cox multivariate regression analysis was performed on 165 patients treated with radical resection from Jan.1995 to Jan.2009.Results The overall recurrence was 16.36%.Univariate analysis showed that lymph node involvement,tumor size,Dukes' stage and histological differentiation were correlated to recurrence of colon cancer after operation;Multivariate analysis showed that lymph node involvement and histological differentiation were significant factors for recurrence rate of colon cancer.Conclusions Histological defferentiation is the most important prognositc factor for patients with colon cancer after radical resection,whereas lymph node metastasis is an important prognostic factor.

Abstract: Arsenic contamination is widespread in water,soil and air,influences 70 countries over 230 regions and has resulted in the global human health hazards.More than 50 million people drink water with arsenic concentrations exceeding 50 μg/L,and more than 140 million population exceeding 10 μg/L.Arsenic increases the production of reactive oxygen species,while reducing the levels of antioxidant enzymes and antioxidant,leading to DNA mutations,lipid peroxidation and protein carbonylation.Arsenic exposure can results in multi-organ damages,including liver and kidney.The mechanism is associated with mitochondrial oxidative damage,cytochrome c release and apoptosis.Some transcription factors,such as Nrf2,play a protective role,and the knockout mice of it showed significant liver necrosis and inflammatory cell infiltration.The major preventions and treatments of arsenic poisoning are chelating agents and antioxidants.

Abstract: Recently,the evident change characteristic in malignant tumor incidence is the trend of rejuvenation.Because caner incidences of children and adolescents have increased rapidly,it has been widely concerned by researches in many countries.Some scholars think the increased caner incidences may be associated with exposures of pregnant women to various carcinogenic environmental factors during gestation and even the preconceptional period.The results of epidemiological investigation or animal experiments showed that there was correlation between caner development in offspring and exposures of them to radiation,smog,some medicine,alcohol,pesticide,organic solvent,heavy metals and some virus etc.in utero.Although there is still lack of relevant human data and definite evidences,the potential risk factors leading to caner development in the offspring due to exposures during the gestation and preconceptional period also increase continuously.Therefore,it is urgent affairs to study on etiology related caner development in the offspring and take effective measures to eliminate these risks during the gestation and preconceptional period,possessing a great meaning for reducing tumor incidence of children and even all people.
