TL;DR: Ureteroscopy (URS)-assisted retrograde nephrostomy (UARN) was reported as a new technique for percutaneous nephrolithotomy (PCNL) in 2011, and this institute performed more than 30 cases of UARN.
Abstract: Introduction: Lawson et al first developed retrograde nephrostomy in 1983.1 Hunter et al reported 30 cases of retrograde nephrostomy in 1985.2 Ureteroscopy (URS)-assisted retrograde nephrostomy (UARN) was reported as a new technique for percutaneous nephrolithotomy (PCNL) in 2011, and this institute performed more than 30 cases of UARN.3 This video presents the details of the UARN technique based on this experience. Materials and Methods: The patient is placed in the Galdakao-modified Valdivia position under general and epidural anesthesia.4 A ureteral access sheath (UAS) is inserted and a flexible ureteroscope (Flex-X2®; Karl Storz) is inserted through the UAS. The target calculi are carefully observed to identify the appropriate renal calyx to be punctured. A Lawson retrograde nephrostomy puncture wire (Lawson Retrograde Nephrostomy Wire Puncture Set; COOK Urological) is carefully set into the flexible URS. The flexible ureteroscope approaches the desired renal calyx again, and the route from t...

TL;DR: Hemium laser enucleation of the prostate (HoLEP) is a novel surgical alternative for the treatment of bladder outlet obstruction caused by benign prostatic growth and is associated with less morbidity than conventional surgery.
Abstract: Introduction: Holmium laser enucleation of the prostate (HoLEP) is a novel surgical alternative for the treatment of bladder outlet obstruction (BOO) caused by benign prostatic growth. The results of randomized studies and case series have confirmed that HoLEP achieves immediate relief of BOO. The improvement in symptoms and flow parameters is maintained throughout the medium term and is associated with less morbidity than conventional surgery. However, the shortage of urologists with relevant experience and the approach's technical difficulty have limited its use. Materials and Methods: In this video, we describe in detail the technique that we use in our center to perform HoLEP. We describe our modifications to the surgical technique from the original description by Gilling,1 which are the results of our experience after more than 500 procedures.2 We emphasize the technical details that facilitate surgery and avoid complications. Results and Conclusions: HoLEP is a reproducible procedure that h...

TL;DR: A 5-minute 43-second video of laparoscopic bladder diverticulectomy for paraureteric diverticula in a 2-year-old boy is presented.
Abstract: Introduction: Paraureteric diverticulum results from protrusion of mucosa through the detrusor muscle fibers at the ureteric hiatus and demands attention in view of recurrent urinary infection, ureteral obstruction, bladder outlet obstruction, or vesicoureteral reflux. Among the various treatment modalities, laparoscopic approach has been appraised for excellent morbidity profile. Only limited cases of laparoscopic bladder diverticulectomy have been reported thus far.1 We present a 5-minute 43-second video of laparoscopic bladder diverticulectomy for paraureteric diverticula in a 2-year-old boy. Methods: Patients were selected after detailed evaluation, including presenting complaints, clinical parameters, and blood profile. The imaging protocol included ultrasonogram (USG), voiding cystourethrogram (VCUG), computed tomogram urography, or magnetic resonance urogram. Renogram was performed to ensure salvagability of the pathological renal unit. Preoperative cystoscopy was performed in all cases. P...

TL;DR: A video of the three-trocar technique step by step, with several tips and tricks to improve the performance is presented, with special caution to avoid incorporating the bowel into the retraction devices.
Abstract: Introduction: Laparoscopic pyeloplasty (LP) is the first-line option for management of ureteropelvic junction obstruction (UPJO).1,2 Initially, this procedure was described using four or five ports; however, newer approaches use three-trocars aiming for minimal invasion,3,4 which may complicate the procedure. We present a video of the three-trocar technique step by step, with several tips and tricks to improve the performance. Patients and Methods: Seven patients (mean 44 years old range: 31–58) underwent Anderson-Hynes three-trocar LP for symptomatic UPJO. The three-port transperitoneal technique was done. A 10-mm trocar was placed directly through the umbilicus and two 5-mm trocars in a triangular arrangement. To improve exposure of the UPJ and reduce the dissection time, the surrounding tissues may be retracted with clips or sutures applying the puppet maneuver, with special caution to avoid incorporating the bowel into the retraction devices. The proximal ureter may be lifted with similar man...
