Abstract: BEST TOOL: The Mini Mental State Examination (MMSE) is a tool that can be used to systematically and thoroughly assess mental status. It is an 11-question measure that tests five areas of cognitive function: orientation, registration, attention and calculation, recall, and language. The maximum score is 30. A score of 23 or lower is indicative of cognitive impairment. The MMSE takes only 5-10 minutes to administer and is therefore practical to use repeatedly and routinely. TARGET POPULATION: The MMSE is effective as a screening tool for cognitive impairment with older, community dwelling, hospitalized and institutionalized adults. Assessment of an older adult’s cognitive function is best achieved when it is done routinely, systematically and thoroughly. VALIDITY/RELIABILITY: Since its creation in 1975, the MMSE has been validated and extensively used in both clinical practice and research.

Abstract: WHY: Sleep is a necessary part of life However, normal aging changes, medical problems, psychiatric problems, and psychosocial issues can alter the pattern and quality of sleep as one grows older, and thus affect the quality of life in the older adult Assessment of sleep patterns enables the nurse to intervene immediately by implementing interventions with the client, or by referring the client for further assessment BEST TOOL: The Pittsburgh Sleep Quality Index (PSQI) is an effective instrument used to measure the quality and patterns of sleep in the older adult It differentiates “poor” from “good” sleep by measuring seven areas: subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleeping medication, and daytime dysfunction over the last month The client self-rates each of these seven areas of sleep Scoring of answers is based on a 0 to 3 scale, whereby 3 reflects the negative extreme on the Likert Scale A global sum of “5” or greater indicates a “poor” sleeper Although there are several questions that request the evaluation of the client’s bedmate or roommate, these are not scored (not reflected in attached instrument) Refer to “More on the Topic”, Buysse et al, 1989, for these questions TARGET POPULATION: The PSQI can be used for both an initial assessment and ongoing comparative measurements with older adults across all health care settings VALIDITY/RELIABILITY: The PSQI has internal consistency and a reliability coefficient (Cronbach’s alpha) of 083 for its seven components Numerous studies using the PSQI have supported high validity and reliability STRENGTHS AND LIMITATIONS: The PSQI is a subjective measure of sleep Self-reporting by clients can empower the client, but can reflect inaccurate information if the client has difficulty understanding what is written, or can not see or physically write out responses Moreover, the scale is presented in English The scale can be adapted to enable the client to respond verbally to items on the scale by having the nurse read the statements to the client

Abstract: 1 Resilience, a personality characteristic that moderates the negative effects of stress and promotes adaptation, has been associated with better health in prior studies. 2 Successful aging can be defined as the enjoyment of health and vigor of the mind, body, and spirit into middle age and beyond. 3 Individuals with lower incomes may be less likely to achieve successful aging because of a higher prevalence of health risk factors. 4 Resilience appears to be positively and significantly associated with indicators of successful aging regardless of income.

Abstract: The purpose of this study was to examine the effects of recorded, preferred music in decreasing occurrences of aggressive behavior among individuals with Alzheimer's type dementia during bathing episodes. Eighteen older adults, age 55 to 95, with severe levels of cognitive impairment, participated in the study. They were randomly scheduled for observation during bath time under either a control (no music) condition or an experimental condition in which recorded selections of preferred music were played via audiotape recorder during the bathing episode. Following a 2-week (10 episode) observation period, conditions were reversed. A total of 20 observations were recorded for each individual. Results indicated that during the music condition, decreases occurred in 12 of 15 identified aggressive behaviors. Decreases were significant (p < 0.05) for the total number of observed behaviors and for hitting behaviors. During the music condition, caregivers frequently reported improved affect and a general increase in cooperation with the bathing task. The implications of these findings for improving the overall quality of care for severely cognitively impaired older adults are discussed.
