Abstract: Exposure to toxic metals remains a widespread occupational and environmental problem in world. There have been a number of reports in the recent past suggesting an incidence of childhood lead poisoning and chronic arsenic poisoning due to contaminated drinking water in many areas of West Bengal in India and Bangladesh has become a national calamity. Low level metal exposure in humans is caused by air, food and water intake. Lead and arsenic generally interferes with a number of body functions such as the central nervous system (CNS), the haematopoietic system, liver and kidneys. Over the past few decades there has been growing awareness and concern that the toxic biochemical and functional effects are occurring at a lower level of metal exposure than those that produce overt clinical and pathological signs and symptoms. Despite many years of research, we are still far from an effective treatment of chronic plumbism and arsenicosis. Medical treatment of acute and chronic lead and arsenic toxicity is furnished by chelating agents. Chelating agents are organic compounds capable of linking together metal ions to form complex ring-like structures called chelates. They have been used clinically as antidotes for acute and chronic poisoning. 2, 3-dimercaprol (BAL) has long been the mainstay of chelation therapy for lead or arsenic poisoning. Meso 2, 3, -dimercaptosuccinic acid (DMSA) has been tried successfully in animals as well as in a few cases of human lead and arsenic poisoning. DMSA could be a safe and effective method for treating lead or arsenic poisoning, but one of the major disadvantages of chelation with DMSA has been its inability to remove lead from the intracellular sites because of its lipophobic nature. Further, it does not provide protection in terms of clinical/ biochemical recovery. A new trend in chelation therapy is to use combined treatment. This includes the use of structurally different chelators or a combination of an adjuvant and a chelator to provide better clinical/biochemical recovery in addition to lead mobilization. The present review article attempts to provide update information about the current strategies being adopted for a safe, effective and specific treatment for two major toxic metals or metalloid.

Abstract: We conducted this study to examine the relationship between shift work duration and the metabolic risk factors of cardiovascular disease among shift workers. The study population consisted of 226 female hospital nurses and 134 male workers at a firm manufacturing diapers and feminine hygiene materials, whose mean ages were 28.5 yr for the nurses and 29.1 yr for the male workers. The fasting blood sugar level, serum cholesterol, blood pressure, height and weight, waist and hip circumferences (only for the nurses), and numbers of walks during work (as a measure of physical activity) were measured. Using the Karasek's job contents questionnaire, job stress was assessed. Information about the years of work, shift work duration, past medical and behavioral history, including smoking, was obtained by a self-administered questionnaire. With definitions of hypertension as systolic blood pressure (SBP) > or =160 or diastolic blood pressure (DBP) > or =90 mmHg occurring at least once, hypercholesterolemia as serum total cholesterol > or =240 mg/dl, obesity as body mass index (BMI) > or =25 kg/m(2) and as waist to hip ratio (WHR) > or =0.85, we examined the prevalences of metabolic risk factors among subjects. Regression analyses to show the relationships between shift work duration and metabolic risk factors were performed using simple and multivariate models stratified by age, and adjusted for smoking, drinking, job strain and physical activity. Duration of shift work was significantly associated with SBP or cholesterol level among male workers aged 30 or more. Among female nurses, it was inversely associated with DBP (in those who were below 30 yr old) and cholesterol (in those who were aged 30 or more). BMI was non-significantly associated with the duration of shift work in both male workers and female nurses who were 30 yr old or more. WHR in female nurses increased slightly according to increasing duration of shift work. Fasting blood sugar was not significantly associated with the duration of shift work in either sex regardless of age-group. These results suggest an association between shift work duration and the metabolic risk factors of cardiovascular disease.

Abstract: Nurses are known to be exposed to occupational stress. However, occupational stress is not well documented for nurses in psychiatric institutions in Taiwan. A cross-sectional study was conducted to explore the work-related stress and risk factors of nurses in psychiatric institutions in Taiwan. A structured questionnaire was distributed to nurses at five state-owned psychiatric hospitals in Taiwan in 2001. Demographic information, working environment, and personal health status were inquired. Occupational stress was assessed based on the Chinese version of Job Content Questionnaire (JCQ). General health status and mental health were evaluated by the International Quality of Life Assessment Short Form-36 (IQOLA SF-36). A total of 573 questionnaires were disseminated to nurses and 518 (90.4%) were satisfactorily completed by nurses, including 408 female full-time nurses who had been in their current work for more than 6 months. In the past one month, 17.2% of nurses reported being under significant stress often or always. Assault episodes were reported by 45.1% of nurses in the past 6 months. Among the nurses, 16.9%, 25.2%, 50.0%, and 7.8% belong to the "High strain", "Low strain", "Active", and "Passive" groups, respectively. Perceived occupational stress was associated with young age, widowed/divorced/separated marital status, high psychological demand, low workplace support, and threat of assault at work. Lower general health score was associated with low job control, high psychological demand, and perceived occupational stress. A lower mental health score was associated with low job control, high psychological demand, low workplace support, and perceived occupational stress. We concluded that nurses in psychiatric institutions are under significant stress related to work factors.

Abstract: This paper investigates levels of engagement in physical activity (PA) among the total German working population and for specific subgroups. The first national health survey for the Federal Republic of Germany was conducted from October 1997 to March 1999. The following study is based on a representative net sample of 3,323 employed persons aged 18 to 69. Bivariate methods and multiple logistic regression analyses were used to investigate the relationship between PA and workplace and occupational factors in addition to social and lifestyle-specific correlates. Four out of 10 gainfully employed persons (39.2%) do not engage in sport. Those with physically strenuous jobs and frequent overtime work are significantly less likely to engage in leisure-time PA. Non-manual workers, and younger, unmarried workers are particularly likely to have an active lifestyle. Our study population did not correspond to the popular image of the recreational athlete as an abstinent, "ascetic" individual: The subgroups of non-smokers and teetotalers contained significantly fewer athletes than the corresponding reference groups. The present paper is the first to publish representative data on PA in the working population since German reunification in 1990. The data show that workers with a high risk of morbidity are those least likely to engage in leisure-time PA (manual workers with below-average educational qualifications from lower socioeconomic groups). The significant accumulation of socially depriving living conditions and lifestyle deficits among inactive subjects shows that one-off preventive measures intended to motivate sporting activity are likely to be ineffective in these subgroups of the population. We therefore advocate continuous exercise programs near the workplace involving exercise training suited to the particular occupation, dietary advice, relaxation techniques and occupational medical care.

Abstract: This study was designed to assess the after effects of sarin exposure on the nervous system in victims of the Tokyo Subway Sarin Attack, which occurred on 20 March, 1995. We performed a similar study 3 yr after the disaster. This time, we newly enrolled 36 staff of the Teito Rapid Transit Authority (Tokyo Eidan subway) to assess the 7 yr after effects on the nervous system, and merged previous data including unpublished data to enhance statistical power. New subjects consisted of 23 male exposed subjects and 13 referent subjects matched for age and working types. Neurobehavioral tests for psychomotor function and memory, stabilometry, and Benton visual retention test were performed. As reported previously, the exposed group performed significantly less well in the psychomotor function test (tapping) than the referent group (117.8 +/- 1.2 vs. 105.6 +/- 1.2 msec). Using merged data, this phenomenon was also observed in a dose-dependent manner and the exposed group performed significantly less well in the backward digit span test (4.47 +/- 1.17 vs. 5.11 +/- 1.65 digits). These results indicate that chronic decline of psychomotor function and memory function still exist 7 yr after the sarin exposure.

Abstract: With the rapidly increasing number of work-related suicides in Japan (Karojisatsu, in Japanese), both applications for worker's compensation insurance and civil suits are proliferating. The phenomenon of work-related suicide is examined along with the process and related factors. With informed consent from bereaved families, two certified psychiatrists independently reviewed and summarized 22 insurance and legal reports filed by psychiatrists on employee suicides that were related to heavy workloads. A clinical epidemiologist participated in discussions with psychiatrists to reach a consensus concerning the cause of the suicides. Only one case involved a female. Seventeen had experienced personnel changes, such as a promotion or transfer. Low social support was recognized in 18, high psychological demand in 18, low decision latitude in 17, and long working hours in 19 cases (more than 11 hours per day for 3 months or more, and without a day off in 9). The subjects had depressive episodes by the ICD-10 criteria and showed suicidal signs. Ten of them saw a general practitioner because of unspecified somatic complaints, but no effective measures were taken. None of them had a history of psychiatric consultation or had received mental health education dealing with job stress management. Although causality cannot be made from this case series report, we hypothesize that long working hours, heavy workloads, and low social support may cause depression, which can lead to suicide. Appropriate countermeasures are urgently needed and the present findings suggest some of them are possible.

Abstract: A clustering of insulin resistance, hypertension and dyslipidemia has been labeled as the metabolic syndrome. Asians have a lower frequency of obesity than do Caucasians, but have an increasing tendency toward metabolic syndrome. Most data on metabolic syndrome are based on studies from Western countries with only limited information derived from Asian populations. We conducted a cross-sectional study of individuals aged 30-60 yr in workplace settings. We examined and analyzed the health data of 1,384 Japanese, Koreans and Mongolians for metabolic syndrome based on the modified definitions of the working definition proposed by the Third Report of the National Cholesterol Educational Program Expert Panel on Detection, Evaluation, and Treatment of High Blood Cholesterol in Adults (ATP III definition). The prevalence of metabolic syndrome using the ATP III-BMI30 and ATP III-BMI25 definitions was 7% and 12% for Japanese, 7% and 13% for Koreans, and 12% and 16% for Mongolians, respectively. With the exception of obesity, the prevalences of individual metabolic abnormalities within each of the three Asian groups were similar to each other and to reported rates of prevalence in the U.S.A. Nevertheless, the values of sensitivity and specificity by the metabolic syndrome definitions are remarkably different relative to ethnicity. A universal metabolic syndrome definition is inappropriate for comparisons of metabolic syndrome among Asian ethnic groups. We believe that the ATP III-BMI25 definition is suitable for the determination of metabolic syndrome among Japanese and Koreans, and that the ATP III-BMI30 is more appropriate for Mongolians.

Abstract: This cross-sectional study on a random sample of 1,500 ambulance personnel investigated the relationships between self-reported work-related physical and psychosocial factors, worry about work conditions, and musculoskeletal disorders among female and male ambulance personnel. Three different outcomes, complaints, activity limitation, and sick leave, for the neck-shoulder and low-back region, respectively, were chosen. Among the female personnel, physical demands was significantly associated with activity limitation in the neck-shoulder (OR 4.13) and low-back region (OR 2.17), and psychological demands with neck-shoulder (OR 2.37) and low-back (OR 2.28) complaints. Among the male personnel, physical demands was significantly associated with low-back complaints (OR 1.41) and activity limitation (OR 1.62). Psychological demands and lack of social support were significantly associated with neck-shoulder complaints (OR 1.86 and OR 1.58, respectively) and activity limitation (OR 3.46 and OR 1.71) as well as activity limitation due to low-back complaints (OR 2.22 and OR 1.63). Worry about work conditions was independently associated with activity limitation due to low-back complaints among the female (OR 5.28), and to both neck-shoulder and low-back complaints (OR 1.79 and OR 2.04, respectively) and activity limitation (OR 2.32 and OR 1.95) among the male personnel. In conclusion, the association patterns between physical and psychological demands and MSDs suggest opportunities for intervention.

Abstract: This study was aimed at understanding the relationship among menstrual pattern, dysmenorrhea, life style and working conditions in nurses. The nurses were randomly selected from a medical center in Northern Taiwan. Each subject completed daily records including life and working conditions during the study period. The study showed that there were statistically significant differences in work years, daily working hours and type of work shift among nurses that worked at different units in the hospital. In the perceived regular cycle group, nurses that worked the night shift only exhibited the shortest menstrual cycles, less than 25 d. There was a significant difference (p<0.05) among the nurses' menstrual cycles. Many life factors (such as passive tobacco smoke exposure, perceived life satisfaction and perceived life stress) and working factors (such as work years, perceived work satisfaction and perceived work stress) were not significantly related to menstrual cycle regularity. In addition, 30% of the nurses complained of dysmenorrhea. Some factors including age, marital status and perceived life satisfaction were significantly related to dysmenorrhea. However, other life factors (such as passive tobacco smoke exposure, smoking, coffee, alcohol, cold drink habits, exercise and perceived life stress) and working factors (such as working places, type of work shift, daily work hours, perceived work satisfaction and perceived work stress) showed no correlation with dysmenorrhea. This study indicates that women should pay attention to their menstrual function and dysmenorrhea phenomenon.

Abstract: To clarify the association between oxidative DNA damage and the neurotoxicity of arsenic, the formation of 8-hydroxy-2'-deoxyguanosine (8-OHdG) as an index of oxidative DNA damage in the brain was examined in mice fed with drinking water containing 1 or 2 ppm arsenic, using an HPLC-electrochemical detector and immunohistochemical method. 8-OHdG levels were significantly increased in the brain of mice given arsenic and its immunoreactivity was distributed in the cerebral and cerebellar cortexes. Cerebral cortex neurons and Purkinje cells in the cerebellar cortex showed degenerative changes in accordance with the distribution of 8-OHdG immunoreactivity. The levels of arsenic in this study were lower than those reported in epidemiological studies. Thus, we conclude that environmentally relevant levels of arsenic induce pathological changes through oxidative DNA damage in the brain tissues in vivo and that cerebral and cerebellar cortex neurons seem to be the major targets of arsenic neurotoxicity.

Abstract: In Japan, Karoshi, which means "death from overwork", has become a focus of social concern. However, no previous study has examined long working hours and job strain simultaneously in relation to incidence of cardiovascular events (CVE) in Japanese workers. We prospectively evaluated the relation between job strain or long working hours and risk of CVE in treated hypertensives during the years 1994-2000. In this study, we followed a sample of 1,615 participants (908 men and 707 women) aged 40-65 yr who were working more than 5 working hours per day. Participants completed questions about work-related conditions and lifestyle variables at baseline, and were free of diagnosed cardiovascular disease, stroke or cancer. Job strain (the combination of job demands and job control) was assessed using a simple questionnaire developed with reference to Karasek's model. The main outcome measure was incidence of CVE. During the mean follow-up period of 5.6 yr, we documented 38 cases of initial CVE. Analyses were conducted using the Cox proportional hazard model. We found a significant association between incidence of CVE and job strain categories, but not long working hours. Multivariate relative risks for CVE in active jobs and high strain jobs overall were 2.89 (95% CI: 1.33-6.28) and 2.45 (95% CI: 0.87-6.93); for men 2.94 (95% CI: 1.29-6.73) and 1.86 (95% CI: 0.51-6.75), and for women 3.97 (95% CI: 0.34-46.88) and 9.05 (95% CI: 1.17-69.86), respectively. In conclusion, active jobs and high strain jobs were associated with increased risk of CVE for treated hypertensive workers.

Abstract: The concept of biological monitoring (BM) has gained the special interest of individual scientists and international organizations. Today, when analytical problems have almost ceased due to new laboratory techniques and quality assurance systems, the methods for interpretation of results have become the most important issue. There are important discrepancies regarding the role of biological monitoring of occupational exposure between Europe and the United States. BM has been an important tool of medical health surveillance in the European countries. In the United States it belongs rather to the field of occupational hygiene. It seems that both the approaches can be accepted. More attention should be paid to the development of the truly health-based biomarkers of exposure based on the dose-effect and dose-response relationships. New areas of application of BM of occupational exposure include determination of DNA and protein adducts, unchanged volatile organic compounds in urine, monitoring of exposure to pesticides, antineoplastic drugs, hard metals, and polycyclic aromatic hydrocarbons. In the general environment BM is the most valuable tool for acquiring knowledge of current levels of internal exposure to xenobiotics, identifying the hot spots and developments in trends of exposure. BM can provide policy makers with more accurate information on the control measures undertaken. At present, the main areas include heavy metals, persistent organic pollutants and pesticides. BM of chemical exposure has become increasingly important in the assessment of the health risk in occupational and environmental medicine. Therefore it would be worthwhile to include BM in the curricula for the training of occupational hygienists.

Abstract: A total of sixty-one subjects occupationally exposed to chromium in an industry which manufactures chromium sulphate and fifteen control subjects from a nearby industry which does not manufacture any chromium related compounds were studied. The history of each subject was recorded on pre-designed form through interview and a routine medical examination was carried out. Blood samples (5-6 ml) were collected for the estimation of chromium and semen samples were collected for semen analysis and the determination of copper and zinc levels in the seminal plasma. Clinical examination revealed nasal perforation in 10 subjects (out of 61) in the exposed group as compared to none in the control group. A significantly higher level of chromium was observed in the blood of the exposed workers as compared to the control. The concentration of zinc in seminal plasma was lower while the level of copper was higher in the exposed group as compared to the control. However, these changes were not statistically significant. Statistically significant higher numbers of morphologically abnormal sperms were noticed in the exposed group with respect to the control. Further analysis of the data indicated that about 53% of the exposed subjects showed less than 30% normal forms as compared to 10% in control subjects. However, no significant alterations in semen volume, liquefaction time, mean pH value, sperm viability, concentration or motility, were noticed between chromium exposed and unexposed workers. The data also indicates that exposure to chromium has some effect on human sperm as a significant positive correlation (r=0.301) was observed between percentages of abnormal sperm morphology and blood chromium levels (p=0.016) after pooling all the data of the control and exposure groups.

Abstract: Burnout is construed as a phenomenon occurring among professionals who work with people , and as a result raises serious problems for human service workers, especially hospital nurses. Nurses’ burnout, known to be related to age and working years, can have an adverse impact on nurses’ development and the quality of care in medical institutes . In the Maslach Burnout Inventory—General Survey (MBI-GS) which measures burnout in general professions, burnout consists of three factors: exhaustion, cynicism, and reduced professional efficacy. Exhaustion refers to fatigue, cynicism refers to indifference or a distant attitude toward work, professional efficacy refers to both social and non-social accomplishment at work. Tao and Kudo, in a cross-sectional study, suggested that nurses’ burnout would result in their intention of leaving. A few other studies which have investigated Japanese nurses’ turnover by cross-sectional study have indicated that the reasons for their turnover were low supervisory support, health problems, inadequate career development, low job satisfaction, familial issues, poor working conditions, low job control, high job demand, and depressive status. We have investigated the factors associated with nurses’ turnover in order to understand the factors contributing to the turnover of high-skilled nurses with the objective of maintaining the quality of nursing in medical institutes. With reference to the indication of Tao et al., we hypothesized that Japanese nurses’ turnover was associated with burnout, and we explored the relationship between turnover and burnout by a follow-up study in order to certify our hypothesis.

Abstract: Prevalence of upper extremity disorders and their associations with psychosocial factors in the workplace have received more attention recently. A national survey of cross-sectional design was performed to determine the prevalence rates of upper extremity disorders among different industries. Trained interviewers administered questionnaires to 17,669 workers and data on musculoskeletal complaints were obtained along with information on risk factors. Overall the 1-year prevalence of neck (14.8%), shoulder (16.6%), and hand (12.4%) disorders were higher than those of the upper back (7.1%) and elbow (8.3%) among those who sought medical treatment due to the complaint. Workers in construction and agriculture-related industries showed a higher prevalence of upper extremity disorders. After multiple logistic regression adjusted for age, education, and employment duration, we found job content, physical working condition, a harmonious interpersonal relationship at the workplace and organizational problems were significant determinants of upper extremity disorders in manufacturing and service industries. Male workers in manufacturing industries showed more concern about physical working conditions while female workers in public administration emphasized problems of job content and interpersonal relationships. We concluded that these factors were major job stressors contributing to musculoskeletal pain of the upper extremity.

Abstract: Single-session supervisory education was developed in conjunction with the Japanese national guidelines for the promotion of employee mental health. Totally 267 voluntary supervisors in a prefectural office were presented with comprehensive information on the role they had to fulfill to promote mental health in the workplace. Totally 864 office employees (53%) were evaluated to determine whether education had had an effect on their psychological distress and job performance. The findings from the departments in which more than one-third of the supervisors had attended education were compared to those from the departments in which no more than one-third of the supervisors had attended education. Three months after the education, the levels of psychological distress and, to a lesser extent, self-reported job performance improved among employees in the departments in which at least one-third of the supervisors attended the education compared to those from departments with lower attendance rates of the supervisors. For the psychological outcome, the positive educational effect was supported by statistically significant interaction terms between time and department category with adjustment for the confounders. Favorable changes were noted among supervisors who received the education in knowledge, attitude, and behavior regarding mental health practices. The behavioral changes were related to decreasing workplace problems and referral of employees to the liaison office and associated medical institutions. Despite several limitations, the findings suggest that providing supervisors with appropriate information has a positive effect on employee psychological well-being.

Abstract: Silicosis is a fibrotic disease of the lungs caused by inhalation, retention, and pulmonary reaction to crystalline silica. Crystalline silica exposure is widespread, and silica sand is an inexpensive and versatile component of many manufacturing processes. Its use and the incidence of silicosis are increased in industrial operations by the mechanization and the use of sandblasting, drilling, pulverizing, cutting, grinding tools and other pneumatic equipment. Sandblasting involves forcefully projecting a stream of abrasive particles onto a surface, usually with compressed air or steam. Mostly, silica sand is forced by compressed air onto the target surface. It has been commonly used in abrading metal or glass. Recently, it has been widely and increasingly used in sandblasting of jeans in Turkey. In this report, we present two concomitant cases of silicosis occurring in the same workplace, located in Istanbul, and caused by sandblasting of jeans. To our knowledge, there is no previous description of silicosis in such an occupation.

Abstract: We assessed the relationship between shift work and job stress. The target subjects were 4,962 male workers (3,078 day workers and 1,884 shift workers) aged 18 to 60 yr who work in a Japanese steel company. We used the "Brief Job Stress Questionnaire", which was developed by a research group organized by the Japanese Ministry of Labour. We evaluated the effect of shift work on job stress using logistic regression analysis including age, lifestyle factors, work conditions, marital status, and living arrangements in the model. Job schedule type was significantly associated with job control, with an odds ratio of 2.22 for shift workers compared to day workers. The logistic regression analysis revealed that the odd ratios for having one or more stressor items in an unfavorable condition were significantly higher for shift workers compared to day workers. Increase in the amount of overtime and decrease in the number of holidays led to a significant deterioration in job stress. Our study reveals that the 3-shift system of employment increases work-related stress, and that job control is low among shift workers. To reduce job stress in this occupational population, a reduction in the amount of overtime and an increase in the number of holidays seem to be useful interventions.

Abstract: The present study investigated the effects of job stress on cellular immune function, such as NK cell activity and NK cell subsets. The participants were 61 female nurses aged 23-59, who worked in a public psychiatric hospital in Ishikawa, Japan. Each subject completed the Nursing Job Stressor Scale (NJSS) and their NK cell activity and lymphocyte surface antigens (CD16+56+) were evaluated as immune system parameters. The NJSS has seven subscales: conflict with other nursing staff, nursing role conflict, conflict with physicians or autonomy, conflict with death or dying, quantitative work load, qualitative work load and conflict with patients. Factors influencing NK cell activity, and the proportion and cell counts of CD16+56+ lymphocytes were evaluated. Increase in quantitative work load significantly decreased NK cell activity. Conversely, no linear relationship was observed between qualitative work load and immunological variables, with the highest percentage of CD16+56+ lymphocytes observed among participants in the medium work load group. The other five NJSS subscales did not relate to immune parameters. In conclusion, the results suggest that perceived job strains, particularly quantitative work load, decreased NK cell function.

Abstract: This study examined effects of web-based psychoeducation on self-efficacy, problem solving behavior, stress responses and job satisfaction. The program was based on social cognitive theory and was primarily aimed at increasing knowledge of stress, self-efficacy, and the use of problem solving behavior. A total of 225 employees were invited to participate in the study. Participants were assigned to an intervention (n=112) or waiting list control group (n=113). Those in the intervention group were asked to access the website and to complete learning within 1 month. To investigate the intervention effect, the change score in the outcome variable was calculated by subtracting the score at pre-intervention from that at post-intervention (1 week after completion of the learning period). Then, the difference in the scores between groups was examined using analyses of covariance (ANCOVA) with the pre-intervention score as the covariate. We detected a marginally significant difference between groups in changes in job satisfaction from pre- to post-intervention (p=0.081). Participants in the intervention group showed an increase in job satisfaction score, while those in the waiting list control group showed a decrease in it. No significant differences were detected between groups in the other indicators (p>.10). We detected significant or marginally significant intervention effects on self-efficacy, problem solving behavior, stress responses, and job satisfaction among males and younger individuals, and those who had initially higher job strain and higher interest in managing stress. Further research is required with longer time periods (controlled follow-up) and a broader sample to fully determine the intervention effect.

Abstract: On May 13, 2000, a firework depot exploded in a residential area of the city of Enschede, The Netherlands. Many disaster workers responded, including volunteer firefighters, a group that has received little attention in disaster research. This study examined the presence of health problems in volunteer firefighters who were involved in disaster work, three years after the disaster. Furthermore, it was investigated whether demographic characteristics and disaster exposure predicted health problems. The study population consisted of 246 volunteer firefighters who were deployed in disaster work and 71 non-deployed controls. These firefighters completed a questionnaire which inquired about their perceived health and health change, physical symptoms, post-traumatic stress, mental health problems, and health care utilization. Three years after the disaster, no health differences emerged between deployed and non-deployed firefighters. Good health and health improvement over the previous year were reported. Respondents who encountered more distressing experiences during disaster work or carried out more direct disaster-related recovery tasks reported more mental health problems and health care utilization. However, the most reliable predictors of health problems were distressing experiences unrelated to the disaster in personal life or during work. Three years after the disaster, the health of volunteer firefighters involved in the disaster work was not much impaired, possibly because aftercare was available and utilized. Nevertheless, disaster exposure was associated with elevated post-traumatic stress symptoms and mental health care utilization. Health care workers should direct specific attention to the treatment and prevention of post-traumatic stress symptoms in cases of major accidents or disasters.

Abstract: Welding is a process during which fumes, gases, electromagnetic radiation and noise are emitted as by-products. Metal oxide particles are particularly hazardous components of welding fumes. Welding has been found to be associated with respiratory symptoms and our objective in the present study was to study the effects of welding on pulmonary function and serum oxidant-antioxidant status. Fifty-one welding workers and 31 control subjects were recruited. Face to face interviews were conducted using the respiratory illness questionnaire adapted from the American Thoracic Society with the addition of demographic characteristics, work history and working conditions. Additionally physical examinations and spirometric measurements were performed at workplaces. Thiobarbituric acid reactive substances (TBARS), protein carbonyls, protein sulfhydryls (SH) and erythrocyte reduced glutathione (GSH) levels were measured to evaluate oxidant-antioxidant status in 34 welding workers and in 20 control subjects. No statistically significant differences were observed in age, height, weight, body mass index (BMI), smoking status and annual working durations between welding workers and controls. Coughing, sputting and wheezing were significantly higher in welding workers (p<0.05). When adjusted for age, BMI and smoking status in logistic regression, welding work showed a significant risk for chronic bronchitis (OR: 4.78, 95%CI: 1.30-17.54). Forced expiratory volume in one second (FEV(1))/forced vital capacity (FVC) and four parameters of forced expiratory flow (FEF: FEF(25), FEF(50), FEF(75), FEF(25-75)) levels measured in the welding workers were significantly lower than those in the control group (p<0.05). Serum TBARS and protein carbonyl levels were higher in welding workers than those in controls (p<0.001, p<0.05, respectively). On the other hand, total protein SH groups and GSH levels were significantly lower in welders than those in controls (p<0.05, p<0.001, respectively). Pulmonary function tests and oxidant-antioxidant status were found to be negatively affected in welding workers chronically exposed to welding fumes and gases. Preventive measures should be taken to improve the health status of these workers.

Abstract: A Cross-sectional Study of Alcohol Drinking and Health-related Quality of Life among Male Workers in Japan: Isao SAITO, et al. Department of Public Health, Nara Medical University— Background: Although light and moderate alcohol drinkers are likely to have better subjective health, the sub-scales for subjective health have not been well documented. Methods: We studied 4,521 male workers aged 25 yr and older with no history of cancer or cardiovascular disease, in 12 occupational groups in Japan. Data were from the High-risk and Population Strategy for Occupational Health Promotion Study (HIPOP-OHP). Drinking status was classified according to daily alcohol intake or frequency of drinking. We assessed the health-related quality of life (HRQOL) based on scores for five scales of the SF-36. Results: Decreased odds ratios of sub-optimal HRQOL conditions, defined as less than the median SF-36 scores, for Role-Physical and General Health were found among persons who consumed 1.0 to 22.9 g/d of alcohol. Odds ratios for sub-optimal Vitality conditions were lowered according to increased levels of alcohol intake. Role-Emotional scores were not associated with alcohol drinking. People who drank 5 to 6 d/wk had higher levels of Role-Physical and Vitality, and those who drank 1 to 2 d/wk had better Vitality and Mental Health scores than non-drinkers. When adjusted for age, marital status, working hours, physical activity at work, self-reported job stress, smoking, regular exercise, hypertension, hyperlipidemia, and diabetes, the associations were almost unchanged except for General Health. Conclusions: Associations of drinking patterns with subjective health varied in five sub-scales of the SF-36. Overall, alcohol drinkers rated their health as good in comparison with non-drinkers. Abstract: This study sought to determine the relationship of job type differences to burnout level, the details of job characteristics for each job type, and the association between burnout and job factors in 189 social workers at all social welfare offices in a prefecture in Japan. Among the three job types, 32.9% of social workers involved with public assistance, 29.0% of social workers involved with public assistance, the elderly, the disabled and single mothers, and 15.2% of social workers involved with the elderly, the disabled and single mothers were scored in the "high burnout" category (p<.05). Job type differed significantly with respect to the job characteristics of percentage of time spent on home visits per typical working day, job satisfaction, aversion to the job, and social support. The job type of public assistance work had a higher percentage of time spent on home visits per typical working day, aversion to the job, lower job satisfaction, and less social support than the job type involving no public assistance work. Multiple regression analyses showed the associations between job factors and burnout for each job type. Aversion to the job had a primary positive association with burnout for all social worker job types. Social support had a negative association with burnout in social workers whose clients included public assistance cases. The number of years in social work had a negative association with burnout, while percentage of time spent doing interviews per typical working day had a positive association with burnout in social workers who were involved with public assistance, the elderly, the disabled and single mothers. These results suggest that the job type of public assistance work may carry a higher risk of burnout than job types involving no public assistance work. To ameliorate this risk, it was thought to be important to improve aversion to the job as well as having a social support network for public assistance social workers.

Abstract: Environmental and biological monitoring of lead, cadmium and chromium levels in spray painters is reported. All of the study subjects worked in automobile body repair shops that had no standard spraying room. They were divided into 2 groups, those who wore an aerosol-removing respirator while spraying (n=20) and those who did not wear the respirator (n=50). Air in the breathing zone of each subject was sampled and analyzed for lead, cadmium and chromium levels. The subjects' blood lead levels and urinary cadmium and chromium levels were also measured along with those of a control group. The mean environmental and biological levels of these metals between the two groups of the painters were not significantly different (p>0.05). However, the biological levels of the metals were significantly higher in the study groups than in the control group (p<0.01). On-site observations revealed that improper use of an aerosol-removing respirator, lack of an isolated spraying room and poor personal hygiene habits caused the failure to prevent heavy metal exposure among the automobile spray painters.

Abstract: Few studies have focused on the health effects of vibrating tools on workers in the tropical area. Work conditions and health effects related to rock drill operation were studied in 102 quarry workers, including 73 rock drill operators in Vietnam. We aimed to clarify (1) risk of vibration exposure, (2) occurrence of vibration-induced white finger (VWF), and (3) characteristics of hand-arm vibration syndrome (HAVS). Total weighted r.m.s. acceleration of the Chinese -or Russian-made rock drills, was 45-55 m/s(2). According to work observation studies, daily exposure time to vibration was 160-210 min. ISO5349 predicted that this exposure level would be associated with a high risk of HAVS in workers. We found no clear evidence of VWF. There may be several reasons why no worker exhibited VWF: (1) warmer work conditions, (2) younger and less experienced workers, (3) seasonal changes in work operations, and (4) healthy worker effect. On the other hand, 5-10% of rock drill operators might be suffering from moderate HAVS which was sensori-neural type dominant. There may be some characteristic features of HAVS among quarry workers in the tropical area.

Abstract: The objective of this study was to explore the associations between two major job stress models (job strain and effort-reward imbalance) and coronary heart disease (CHD) risk factors (blood pressure; total, high- (HDL) and low-density lipoprotein (LDL) cholesterol; and triglycerides) in Japanese part-time female employees of a retail company. The study population was either 35 yr old or between 40 and 63 yr old. Data collection was carried out in 2002; a total of 1,401 subjects participated in a medical examination and completed a questionnaire. After adjusting for other covariates (age, relative weight, tobacco use, alcohol consumption, lack of exercise, education, marital status, history of child bearing, medical treatment for disease, and occupation), a significant association was found between the effort-reward imbalance, a "high-cost and low-gain" condition at work, and a high prevalence of low HDL cholesterol (Odds ratio = 4.4). A weak but unexpected association was found between job strain and low prevalence of low HDL cholesterol. In explanatory analysis with individual components of the two models, associations were evident between high extrinsic effort and high prevalence of low HDL cholesterol and low prevalence of high triglyceride, high job control and low prevalence of high systolic blood pressure, and high job demands and low prevalence of high systolic and diastolic blood pressure. In this cross-sectional study of Japanese part-time working women, a significant association was found between effort-reward imbalance and unfavorable HDL cholesterol profiles. The findings did not support the hypothesis that job strain is associated with CHD risk factors.

Abstract: The extensive use of multiple organic solvents in offset lithographic printing causing high emissions of volatile organic compounds (VOCs) indeed poses a serious risk to printing workers' health. In this study, indoor air quality (IAQ) assessments were carried out in seven printing plants and the main objectives were to understand the effect of VOC emissions on IAQ and develop effective mitigation measures to protect workers. The thorough gas chromatography/mass spectrometry (GC/MS) measurements showed that although a variety of VOCs were presented in the indoor air, none of them was found close to individual 8-h time-weighted average (TWA) of the occupational exposure limit (OEL). The additive effect was also found below the critical value of unity. However, short-term personal exposure to total volatile organic compounds (TVOCs) was exceedingly high when a print worker carried out blanket and ink roller cleaning procedures. Therefore, the occupational health risk was mainly due to repeated short-term exposures during intermittent VOC-emitting procedures rather than long-term exposure to background VOCs. Push-pull local exhaust ventilation (LEV) was identified as an effective mitigation measure. Computational fluid dynamics (CFD) analysis was conducted to study the push-pull LEV operation. It was found that there existed a threshold LEV air flow rate for an abrupt reduction in the worker's exposure to VOCs. The reduction was less sensitive when the LEV airflow was further increased beyond the threshold. These phenomena, consistent with experimental results reported by other investigators, were explained by detailed CFD analysis showing the competition between the general ventilation and the push-pull LEV to become the dominating driving force for the resultant local flow pattern.

Abstract: Sleep disturbance as a pervasive health problem can directly affect the physical and psychological well-being of individuals. Factors that positively relate to sleep quality can therefore improve healthy functioning. We examined whether leisure time activities are associated with sleep quality in two culturally different samples of civil servants. In this cross-sectional study we evaluated 1,682 Japanese, in Toyama prefecture (T) city, and 6,914 British civil servants from the Whitehall II study undertaken in London. The Japanese version of Pittsburgh sleep quality index (PSQI-J) was used in T city and Jenkins' sleep problem scale was used in the Whitehall II study. Setting a validated cut-off point of 5.5 for the PSQI-J global score and the upper tertile point for the Jenkins' sleep problem scale, we conducted logistic regression analysis to assess the association between leisure time activities and sleep quality. In both populations, those who participated in voluntary activities in clubs or organizations were significantly less likely to have poor sleep quality with Odds ratios (OR) and 95% confidence intervals (95%CI) of 0.73 (95%CI; 0.56-0.97) and 0.85 (95%CI; 0.76-0.95) in Japanese and British civil servants, respectively. Similar findings were apparent for visiting friends and relatives (ORs 0.60 (95%CI; 0.46-0.80) and 0.71 (95%CI; 0.56-0.90) for Japanese and British subjects, respectively). Our findings suggest that engagement in social leisure activities is associated with better sleep quality and consequently better general well-being.

Abstract: Occupational asthma has been found to be associated with lowered socio-economic outcomes, an increased unemployment rate and a decreased quality of life. The compensation of occupational diseases is comparatively favourable in Finland. Our aim was to follow-up the working status and life satisfaction of patients with diisocyanate-induced asthma in 245 cases diagnosed during 1976-1992. A questionnaire was sent out on average 10 (3-19) yr after the diagnosis to the surviving 235 patients. The questionnaire was validated by re-examining 91 of them clinically, and with spirometry, histamine challenge test and peak flow surveillance. Of the 213 responding patients, 14% were unemployed, and for 50% of them unemployment was caused by asthma. Unemployment was associated with nocturnal asthma symptoms (OR 10.93; CI 2.69-44.452), increased PEF variability (OR 8.46; CI 1.52-46.97) and with the use of short-acting beta-sympathomimetic medication (p=0.045). Satisfaction with life was associated with present working (OR 3.50; CI 1.73-7.06) and with good condition of asthma as assessed by the use of asthma medication (OR 0.49; CI 0.27-0.89) and objective measurements of the asthma condition, e.g. PEF variability (OR 0.21; CI 0.08-0.59). As a conclusion, unemployment was remarkably low as compared with earlier reports and related to the period of simultaneous economic recession. Unemployment, as well as dissatisfaction with life were associated with poor conditions of asthma. Unemployment was associated with improper asthma care favouring the use of short-acting beta-sympathomimetic medication. Proper follow-up of asthma is essential for minimizing the social complaints of occupational asthma, as well as for enhancing life satisfaction.
