Abstract: Objectives:To investigate the structur and function of heart in COPD patients with normal PaO2 and without signs of RV failure.Methods:The RV, left ventricular(LV) structure and function of 25 clinically stable COPD patients (FEV1,1.23 ±0.52 L/s; PaO2, 82±10 mm Hg) and 26 age-matched health person were detected by Doppler chocardiography.Pulmonary artery pressure (PAP) was detected by Doppler chocardiography.Results:The RVDD of COPD group and control group was 19±3mm and 23±2mm (P0.01), respectively.RVFT of COPD group and control group was 4±0.9 and 3±0.8 (P0.05) , respectively;E/A is 1.2±0.9,1.5±0.4 (P0.05)and RVEF was 56±12 and 60±11 (P0.05) , respectively.The LVDD of COPD group and control group was 48.0±5.5 and 46.2 ±3.4(P0.05) , respectively; LVFT of COPD group and control group was 10.0±0.8 and 10.3±0.7(P0.05) , respectively; E/A:1.2±0.9,1.5±0.4 (P0.05), The maximum rate of early diastolic filling fraction (PRF):2.83±0.43,2.81±0.45 (P0.05); LVEF was 53±7,62±14 (P0.05); EF slope rate 55.5±11.7,54.5±12.1 (P0.05); PAP was not elevated in the COPD group and control group.Conclusion:Concentric RV hypertrophy is the earliest sign of RV pressure overload in patients with COPD.This structural adaptation of the heart does not alterRV and LV systolic function.

Abstract: Objective:To assess the association between serum ferritin levels and parent reports of sleep disturbances in a sample of children with attention-deficit/hyperactivity disorder(ADHD)Methods:Sixty-two consecutively referred children(6-14 years) with ADHD diagnosed according to DSM-IV criteriaParents filled out the Sleep Disturbance Scale for Children(SDSC) and serum ferritin levels were determinedResults:Compared to children with serum ferritin levels ≥45μg/L,those with serum ferritin levels 45μg/L had significantly higher scores on the SDSC subscale "Sleep wake transition disorders"(P005),as well as significantly higher scores on the total scores of SDSCThe mean scores on the other SDSC subscales did not significantly differ between children with serum ferritin ≥45 and 45μg/LSerum ferritin levels were inversely correlated to SWTD scores(P005)Conclusions:Serum ferritin levels 45μg/L might indicate a risk for sleep wake transition disorders,including abnormal sleep movements,in children with ADHD

Abstract: Pain is one of the most common symptoms of patients,and help patients as far as possible to reduce the pain is the duty of medical personnel.With the transformation of the pain management mode,it is advocated that the medical person not only control the patients' pain,but also evaluate them comprehensively.Pain assessment can provide evidence for treatment,which is the first step of pain management.As clinical working persons,the nurse contacts with patients commonly and could be able to learn how the patients feel about pain,so they should have the capability to assess pain accurately.However pain is a subjective feeling of humans,it is rather difficult to evaluate objectively.At present,hospital management is still lack of scientific and effective standards for pain assessment.This review summarized the significance of pain assessment,assessment methods,frequency,record and matters need attention,and the aim of which is to provide the basis for clinical nursing staff assessing pain effectively,to promote the development of the pain nursing care and to reduce patients' pain.

Abstract: NPT(negative pressure therapy)which also can be called VAC(vacuum-assisted closure)is a treatment which can promote wound healing.It can not only be used to one stage treatment of chronic and complex wounds but also can assist to temporary close the wound and to the preparatory work of pre-operation wound area such as dermatoplasty. Although the physiological basis of VAC has been not fully expounded, the clinical effects have been proved.We analysed the relevant researches that based on randomized clinical trial, VAC and topical negative pressure which are chosen from Pubmed-medline and assessed the scientific data which derived from animal experiment, randomized clinical trial and observing in clinical application. According to the data, NPT has remarkable effects in inducing wound healing especially in early stage of wound healing. Besides, NPT also has positive effect in anti-infective therapy. Experimental data have presented that VAC has positive effect in promoting wound healing. But the functional mechanism of it has not been clarified because of evidence-based medicine data could not explain the positive effects of clinical treatment. Although there still need further random and blind method to explain its effects, we deem that reasonable using VAC especially using by prectised surgeon is a commendable treatment to promoting wound healing.

Abstract: Osteoarthritis(OA) is associated with cartilage destruction, subchondral bone remodeling and inflammation of the synovial membrane. However, this debilitating disease are poorly understood. Secreted inflammatory molecules, such as proinflammatory cytokines, are among the critical mediators of the disturbed processes implicated in OA pathophysiology. This Review discusses the current knowledge regarding the role of proinflammatory cytokines in the pathophysiology of OA and addresses the potential of anti-cytokine therapy in the treatment of this disease.
