Abstract: The acid-etch technique for bonding composite resins to enamel has revolutionized the practice of restorative dentistry. The ability of clinicians to bond restorative materials to enamel has fundamentally changed such diverse areas as cavity preparation, caries prevention, and esthetic treatment options. Although bonding of resin to dentin has proved to be a difficult challenge, ongoing advances are improving the reliability and predictability of dentinal adhesion. The purpose of this article is to provide a brief history of enamel and dentinal bonding, as well as an overview of the current state of the art.

Abstract: The permeability of dentin to adhesive agents is of crucial importance in obtaining good dentinal bonding. In those systems that remove the smear layer, the opportunity exists for resin to infiltrate both tubules and intertubular dentin. Resin penetration into tubules can effectively seal the tubules and can contribute to bond strength if the resin bonds to the tubule wall. Resin infiltration into intertubular dentin can only occur if the mineral phase of dentin is removed by acidic conditioners or chelators. This is more easily accomplished in fractured dentin than in smear layer-covered dentin because of the residual collagen debris that remains on the surface following acid etching of smear layers. The channels for resin infiltration are the perifibrillar spaces created around the collagen fibers of dentin following removal of apatite mineral by acids. The diffusion of adhesive resins through these narrow, tortuous, long channels in 1 to 2 minutes offers a number of challenges that require further research.

Abstract: On May 24-25, 2002, a symposium was held at McGill University in Montreal, Quebec, Canada, during which scientists and expert clinicians presented 75 papers on the efficacy of overdentures for the treatment of edentulous patients. Strong emphasis was given to evidence from randomized controlled trials in which mandibular two-implant overdentures were compared to conventional dentures. A draft consensus statement was circulated to all presenters, as well as to subjects who participated in some of the clinical trials and other edentulous individuals who attended the symposium. The statoment was modified during the meeting in tight of their comments. We hope that the final version of the consensus statement will serve as a guideline for ctinioians and patients, and that it will stimulate discussion within and between professional organizations, heaith authorities, and thirdparty payers.

Abstract: This article reviews the literature on the use of hydrogen peroxide in three professionally administered bleaching techniques from historical, technique, and safety viewpoints. Safety over time, absolute safety, and relative safety of nonvital bleaching, in-office vital bleaching, nightguard vital bleaching, and over-the-counter bleaching kits are compared. The advantages and disadvantages of different bleaching options, as well as indications for individual or combined use of the techniques, are discussed. In addition, specific indications for the use of the nightguard vital bleaching technique are presented.
