Abstract: �� Purpose: The aim of this study was to develop and validate identification criteria for the cases of osteoporotic hip fracture using the National Health Insurance Database, and we calculated the incidence rate of osteoporotic hip fracture in the Korean population using these criteria. Materials and Methods: The criteria to identify osteoporotic hip fracture using a diagnosis code, a procedure code, the type and number of medical service usages and the patients' ages were developed via discussions among experts. These criteria were validated by using a hip fracture cohort in which all the genuine osteoporotic hip fracture patients in the Jeju area were identified by reviewing the registered medical records and radiographs. By applying the identification criteria to the National Health Insurance Database that was accumulated until June 2009, we calculated the incidence rate of osteoporotic hip fracture in patients between 50 and 100 years of age in 2008. Results: The identification criteria had a sensitivity of 93.1% and a positive predictive value of 77.4%. A total of 20,432 osteoporotic hip fracture cases were identified in 2008, and the incidence rate was 15.7 cases per 10,000 persons. The osteoporotic hip fracture incidence rate for females was 20.7 (per 10,000persons), and this was 2.1 times higher than that for the males (9.8case per 10,000persons), and the rates were increased with age. Conclusion: The incidence of osteoporotic hip fracture that occurred on a nationwide scale can be more precisely estimated by using the National Health Insurance Database with its comprehensive information on the overall details of treatment as well as the diagnosis codes, and so the incidence of osteoporotic hip fracture can be reliably calculated for each year. The results from this research could be used as evidence in a hip fracture management plan for establishing Korean Health policy.

Abstract: 인체의 관절 중 견관절에 이어 두 번째로 운동 범위가 큰 고관절은, 비구와 대퇴골 근위부로 이루어지고 체중 부 하와 체중 전달의 기능상 관절의 운동 범위보다는 관절의 안정성이 더 중요한, 다축성(multiaxial) 볼-소케트 형의 활막 관절(synovial joint)이다. 구형의 대퇴골 두와 컵 모양의 비구가 밀접하게 맞고 관절 막과 주위 근육과 인대 에 보강되어 관절의 안정성이 매우 크며 관절 운동시 이러 한 구조물들이 조화롭게 협동작용을 한다. 고관절에 미치 는 체중의 3배에서 5배에 이르는 하중에도 수동 및 능동 적 관절 운동 범위내에서 정적인 평형을 유지한다. 일생동 안 반복되는 수 많은 작은 외상에도 고관절은 대부분 효과 적이고 편안한 관절 역할을 수행한다. 정상 고관절의 해부 학에 대한 이해는 환자의 질병과 손상을 진단하고 치료하 는데 매우 중요한 바탕이 된다. 고관절부의 질환과 외상은 점진적으로 증가하는 추세이 며, 특히 인공관절 치환술은 그 수요가 증가하여 정형외과 과학내에서 학문적 비중이 커지고 있다. 고관절의 생체역 학적 지식의 이해는 여러 고관절 질환의 진단과 치료 접근 에 필수적일뿐만 아니라 관절 기능의 평가, 치료 프로그램 과 새로운 술기 및 최신 인공관절 기기의 개발에 결정적인 단초를 제공하는 매우 중요한 역할을 하고 있다. 또한 고 관절 주위조직의 손상기전과 퇴행성 고관절 질환의 발생 기전을 파악하는데 있어서도 생체 역학적 원리의 이해는 필수적이다. 고관절의 생체역학 연구는 근육의 기능적 역할, 보행, 퇴행성 관절의 병리와 절골술 및 인공 고관절 치환술 등의 분야에서 많은 발전이 이루어 지고있다. 이를 통해 체중이 Submitted: April 23, 2009 1st revision: May 25, 2009 2st revision: June 1, 2009 3st revision: June 9, 2009 4st revision: June 10, 2009 Final acceptance: June 11, 2009
