Abstract: An electrosurgical tubular trocar system (10) has a hollow tube (13) substantially longer than its diameter. The tube (13) is shaped for insertion in a direction generally along its axis through tissue of a human or animal body. Distal and proximal ends (17 and 18) on the tube (13) enter and remain outside the tissue, respectively. A tip (19) on the distal end (17) punctures tissue of a human or animal. An insulating portion (20) of high dielectric material extends along the tube (13) between the distal and proximal ends (17 and 18). An electrode (14) on the insulating portion (20) extends from the proximal end (18) to the tip (19) to transmit radio frequency energy. A tip point (19) at an acute angle to the axis lessens the initial force necessary for entry of the tube (13). The return path (16) is a conductor (23) on the insulating portion (20) for bipolar cutting across a gap (25). An alternate system may have the return path (16) as a conductive pad (15) in contact with the tissue as a monopolar circuit. The tube (13) may be in fluid communication for flow.

The staple fastening assembly (8) is adapted to allow unobstructed access towards the concave inner faces of the cartridge (10) and the anvil (22). The cartridge (10) can be moved towards the anvil (22) from a spaced position for positioning tissue therebetween to a closed position for clamping the tissue. Preferably, a knife is contained within the cartridge (10) and is positioned such that there is at least one row of staples on at least one side of the knife.

The surgical instrument can further comprise a cutting member configured to incise tissue positioned captured between the staple cartridge and the anvil and, in addition, means for stopping the cutting member prior to a distal end datum, wherein the distal end datum can be defined by the distal-most staple cavity in the staple cartridge. In such embodiments, the incision within the tissue may not extend beyond the portion of the tissue that has been stapled.

Abstract: In an electrosurgical generator for generating radio frequency power, the generator includes a radio frequency output stage having two or more output connections, and a power supply coupled to the output stage for supplying power to the output stage. A controller is operable to cause the generator to supply a blended output signal alternating constantly between a first output signal across the output connections in which the radio frequency output voltage developed across the output connections is limited to at least a first predetermined threshold value for cutting or vaporisation of tissue, and a second output signal across the output connections in which the radio frequency output voltage developed across the output connections is limited to a second threshold value for coagulation. There is also provided adjustment means, operable by a user of the electrosurgical generator, for changing between various preset settings for the blended signal, the preset settings each having a predetermined duty cycle of the blended signal that is limited to the first threshold value for cutting or vaporisation, and a predetermined duty cycle of the blended signal that is limited to the second threshold value for coagulation.

Abstract: The method for resolving atherosclerotic plaque buildup and/or eroding unwanted tissue in a blood vessel includes the steps of: inserting an electrode in and along the lumen of a blood vessel; manually manipulating said electrode through the blood vessel; positioning said electrode proximate to atherosclerotic plaque buildup site or unwanted tissue site in the blood vessel; supplying a predetermined high-frequency, high-voltage electrical current to said electrode; maintaining said predetermined current for a predetermined time period; and sensing from time to time, the amount of plaque or unwanted tissue at the site. The electrosurgical plaque-resolving or tissue-eroding device is adapted to be inserted within and along the lumen of a blood vessel and manipulated therethrough to a desired position where the device is operated to resolve atherosclerotic plaque buildup or erode tissue in the blood vessel according to the method to re-establish desired blood flow through the blood vessel or to remove tissue therefrom. The device comprises an elongate flexible hollow tubular body having a distal end and a proximal end. A hollow tip member is mounted at the distal end of the flexible hollow tubular body and an electrode is associated with the hollow tip member for resolving plaque or eroding tissue. A power supply circuit for supplying a high-frequency, high-voltage electrical current to the electrode is coupled to the electrode. A mechanism for sensing, from time to time, the amount of plaque or tissue at the site in the blood vessel is also provided with the device.

Abstract: An apparatus (10) for forming and enlarging percutaneous penetrations comprises an elongate dilation tube (14) which receives an elongate expansion member (12) in an axial lumen thereof. The dilation tube is radially expandable from a small diameter configuration to a larger diameter configuration. The dilation tube is percutaneously introduced to a desired target site within a patient's body and thereafter radially expanded by axial insertion of the expansion member through the axial lumen of the tube. The tube prevents the application of axial forces on the surrounding tissue as the expansion member is introduced. The expansion member may include a separate outer sleeve member (30) which remains in place to maintain the enlarged access channel. Optionally, an inflatable balloon (46) at the distal end of the dilation tube may be provided to enhance retention of the device and sealing of the penetration.

Abstract: A trocar sleeve characterized by an annular flange adjacent a proximal end, a disc-shaped annular stop spaced from the annular flange in a distal direction and helically extending thread extending from said annular disc-shaped stop toward the distal end to prevent the removal of the sleeve from a puncture. The sleeve is used in conjunction with the trocar, which is provided with a disc-shaped annular stop between its ends, which engages the proximal end of the sleeve as the trocar is inserted therethrough and with cutting edges of the trocar being exposed to the distal end of the sleeve. The sleeve and trocar form a device which is used to form a puncture in the wall of a patient and then, after the distal end of the sleeve has been inserted in the puncture, the sleeve is rotated to thread the sleeve into the puncture to a position to prevent its removal. The sleeve is particular useful for the insertion of various instruments, such as endoscopes, to perform internal surgery and is, preferably, provided with an insulating inner sleeve layer.

Abstract: An epidural needle and method of use are disclosed involving a needle shaft having a first end and a second end with an axial channel extending therebetween. The first end of the needle shaft includes an inclined surface and a rounded and blunted point. The second end of the needle includes an attachment means. A solid rod having an inclined surface and a rounded and blunted point is receivable into the axial channel of the needle shaft. When the solid rod is positioned within the axial channel of the needle shaft, the point of the solid rod and the needle together form a unitary tip having an inclined portion and a blunted portion. The blunted portion forms an end face disposed at an angle of about 80 to 100 degrees relative a longitudinal axis of the needle. The unitary tip avoids nicking, piercing, severing, or perforating the veins, arteries, nerves and dura of the patient in traversing the spinal ligaments and in entering a space proximate the dura matter of a spinal cord of a patient to provide liquid communication with the space when the solid rod is withdrawn from the axial channel of the needle.
