An In-Vivo Study on Intra-Day Variations in Vascular Stiffness using ARTSENS Pen

Abstract: This work presents investigations on intraday variations in arterial stiffness. For this purpose, an in-vivo study was conducted on five subjects over a duration of five consecutive days. Five stiffness index ($\beta $) measurements were obtained per day for each individual. Our clinically validated ARTSENS device was used to perform fully automated reliable stiffness measurements on the carotid artery. For each measurement, two trials were performed and averaged. These trials were observed to be repeatable with the coefficient of variation $< 0.72$%. For each day, one measurement that was performed immediately after the lunch was subject to intervention due to the consumed food, which significantly $( \mathrm {p}< 0.001)$ deviated from the mean baseline $\beta $ of the day. Such significant deviations were not observed for the rest measurements that were performed in the absence of an intervention. Two subjects who consumed caffeinated beverages during the lunch exhibited an increment in $\beta $ measurement (taken immediately after lunch) as compared to mean baseline $\beta $ of the day. Further, there was no significant (p $=$ 0.97) difference between the mean baseline $\beta $ measured over a day and the mean baseline $\beta $ measured over the entire course of the study. Results obtained from the present study demonstrated that the arterial stiffness does not vary significantly over a short period but varies progressively. However, significant temporary variations in stiffness could be observed due to dietary interventions.

Abstract: Caffeine acutely increases blood pressure and peripheral vascular resistance, in part because of sympathetic stimulation. Its effects on large artery properties are largely unknown. In a double-blind crossover study, 7 healthy subjects 26+/-2.6 years of age (mean+/-SEM) were studied for 90 minutes while in the supine position on 2 occasions separated by a week in random order after ingestion of 250 mL caffeinated (150 mg) and decaffeinated (<2 mg) coffee. Compared with baseline, arterial stiffness measured by carotid femoral pulse wave velocity increased progressively from 7.2+/-0.41 to 8.0+/-0.6 m/s (P<0.05) at 90 minutes after caffeine intake, an effect that may be independent of changes in blood pressure. In addition, arterial wave reflection, measured by applanation tonometry from the aortic pressure waveform, also increased from -5.7+/-7.6% to 5.28%+/-5.6 (P<0.01). No such changes were seen with decaffeinated coffee intake. Although the integral of the brachial systolic and diastolic blood pressure values over the 90 minutes was larger (P<0.05) after caffeinated than decaffeinated coffee intake, the effect on aortic systolic and diastolic blood pressures was more pronounced (P<0.05) than on the brachial artery. These results show a significant effect of caffeine intake on arterial tone and function and suggest that caffeine acutely increases arterial stiffness.

Abstract: Vascular stiffness is an indicator of cardiovascular health, with carotid artery stiffness having established correlation to coronary heart disease and utility in cardiovascular diagnosis and screening. State of art equipment for stiffness evaluation are expensive, require expertise to operate and not amenable for field deployment. In this context, we developed ARTerial Stiffness Evaluation for Noninvasive Screening (ARTSENS), a device for image free, noninvasive, automated evaluation of vascular stiffness amenable for field use. ARTSENS has a frugal hardware design, utilizing a single ultrasound transducer to interrogate the carotid artery, integrated with robust algorithms that extract arterial dimensions and compute clinically accepted measures of arterial stiffness. The ability of ARTSENS to measure vascular stiffness in vivo was validated by performing measurements on 125 subjects. The accuracy of results was verified with the state-of-the-art ultrasound imaging-based echo-tracking system. The relation between arterial stiffness measurements performed in sitting posture for ARTSENS measurement and sitting/supine postures for imaging system was also investigated to examine feasibility of performing ARTSENS measurements in the sitting posture for field deployment. This paper verified the feasibility of the novel ARTSENS device in performing accurate in vivo measurements of arterial stiffness. As a portable device that performs automated measurement of carotid artery stiffness with minimal operator input, ARTSENS has strong potential for use in large-scale screening.
