Abstract: Objective—To review the literature on the benefits and disadvantages of clinical and medical audit, and to assess the main facilitators and barriers to conducting the audit process. Design—A comprehensive literature review was undertaken through a thorough review of Medline and CINAHL databases using the keywords of “audit”, “audit of audits”, and “evaluation of audits” and a handsearch of the indexes of relevant journals for key papers. Results—Findings from 93 publications were reviewed. These ranged from single case studies of individual audit projects through retrospective reviews of departmental audit programmes to studies of interface projects between primary and secondary care. The studies reviewed incorporated the experiences of a wide variety of clinicians, from medical consultants to professionals allied to medicine and from those involved in unidisciplinary and multidisciplinary ventures. Perceived benefits of audit included improved communication among colleagues and other professional groups, improved patient care, increased professional satisfaction, and better administration. Some disadvantages of audit were perceived as diminished clinical ownership, fear of litigation, hierarchical and territorial suspicions, and professional isolation.The main barriers to clinical audit can be classified under five main headings.These are lack of resources, lack of expertise or advice in project design and analysis,problems between groups and group members, lack of an overall plan for audit, and organisational impediments. Key facilitating factors to audit were also identified: they included modern medical records systems, eVective training, dedicated staV, protected time, structured programmes, and a shared dialogue between purchasers and providers. Conclusions—Clinical audit can be a valuable assistance to any programme which aims to improve the quality of health care and its delivery. Yet without a coherent strategy aimed at nurturing eVective audits, valuable opportunities will be lost. Paying careful attention to the professional attitudes highlighted in this review may help audit to deliver on some of its promise.

Abstract: Background— Fewer than half of patients with ST-elevation acute myocardial infarction (STEMI) are treated within guideline-recommended door-to-balloon times; however, little information is available about the approaches used by hospitals that have been successful in improving door-to-balloon times to meet guidelines. We sought to characterize experiences of hospitals with outstanding improvement in door-to-balloon time during 1999–2002. Methods and Results— We performed a qualitative study using in-depth interviews (n=122) with clinical and administrative staff at 11 hospitals that were participating with the National Registry of Myocardial Infarction and had median door-to-balloon times of ≤90 minutes during 2001–2002, representing substantial improvement since 1999. Abstract: We investigated the explicit and implicit ways in which 4 fifth-grade teachers communicated an emphasis on mastery and performance goal orientations to their students. We used survey data about perceptions of the classroom mastery and performance goal structures from 223 students in 10 classes to identify 4 classrooms with significantly different motivational profiles. We then used observational data to describe teachers' talk and practices regarding tasks, authority, recognition, grouping, evaluation, time, social interactions, and help-seeking in those classes. We found that teachers perceived as having a high mastery focus spoke about learning as an active process, and this was reflected in their practices. They required involvement from all students, emphasized effort, and encouraged student interaction. Those teachers also exhibited social and affective support for, and concern about, students' learning and progress. These practices were not observed in low mastery-focused classes. Abstract: Current policies place unprecedented demands on districts to use evidence to guide their educational improvement efforts. How districts respond is likely to be influenced by how individuals in the district conceptualize what it means to use evidence in their ongoing work. This study draws on sensemaking and institutional theory to investigate how individuals in one urban school district conceive of evidence‐based practice. The study develops grounded typologies that describe the ways that individuals conceptualize high‐quality evidence, appropriate evidence use, and high‐quality research. It then explains variation in conceptions, pointing to the ways organizational responsibilities and reform history shape how individuals come to understand evidence‐based practice. The article closes by suggesting implications for district response to federal policy demands for evidence‐based practice.

Abstract: A significant number of security breaches result from employees' failure to comply with security policies. Many organizations have tried to change or influence security behaviour, but found it a major challenge. Drawing on previous research on usable security and economics of security, we propose a new approach to managing employee security behaviour. We conducted interviews with 17 employees from two major commercial organizations, asking why they do or don't comply with security policies. Our results show that key factors in the compliance decision are the actual and anticipated cost and benefits of compliance to the individual employee, and perceived cost and benefits to the organization. We present a new paradigm -- the Compliance Budget - as a means of understanding how individuals perceive the costs and benefits of compliance with organisational security goals, and identify a range of approaches that security managers can use to influence employee's perceptions (which, in turn, influence security behaviour). The Compliance Budget should be understood and managed in the same way as any financial budget, as compliance directly affects, and can place a cap on, effectiveness of organisational security measures.

Abstract: The use of reliability and validity are common in quantitative research and now it is reconsidered in the qualitative research paradigm. Since reliability and validity are rooted in positivist perspective then they should be redefined for their use in a naturalistic approach. Like reliability and validity as used in quantitative research are providing springboard to examine what these two terms mean in the qualitative research paradigm, triangulation as used in quantitative research to test the reliability and validity can also illuminate some ways to test or maximize the validity and reliability of a qualitative study. Therefore, reliability, validity and triangulation, if they are relevant research concepts, particularly from a qualitative point of view, have to be redefined in order to reflect the multiple ways of establishing truth. Key words: Reliability, Validity, Triangulation, Construct, Qualitative, and Quantitative This article discusses the use of reliability and validity in the qualitative research paradigm. First, the meanings of quantitative and qualitative research are discussed. Secondly, reliability and validity as used in quantitative research are discussed as a way of providing a springboard to examining what these two terms mean and how they can be tested in the qualitative research paradigm. This paper concludes by drawing upon the use of triangulation in the two paradigms (quantitative and qualitative) to show how the changes have influenced our understanding of reliability, validity and triangulation in qualitative studies.

Abstract: Theme identification is one of the most fundamental tasks in qualitative research. It also is one of the most mysterious. Explicit descriptions of theme discovery are rarely found in articles and reports, and when they are, they are often relegated to appendices or footnotes. Techniques are shared among small groups of social scientists, but sharing is impeded by disciplinary or epistemological boundaries. The techniques described here are drawn from across epistemological and disciplinary boundaries. They include both observational and manipulative techniques and range from quick word counts to laborious, in-depth, line-by-line scrutiny. Techniques are compared on six dimensions: (1) appropriateness for data types, (2) required labor, (3) required expertise, (4) stage of analysis, (5) number and types of themes to be generated, and (6) issues of reliability and validity.

Abstract: This chapter is about methods for managing and analyzing qualitative data. By qualitative data the authors mean text: newspapers, movies, sitcoms, e-mail traffic, folktales, life histories. They also mean narratives--narratives about getting divorced, about being sick, about surviving hand-to-hand combat, about selling sex, about trying to quit smoking. In fact, most of the archaeologically recoverable information about human thought and human behavior is text, the good stuff of social science.
