Classification of eczemas: an approach using pathogenetic criteria

Abstract: Based on pathogenetic categories, a consistent classification of eczemas is proposed. In order to avoid perpetual confusion, the terms endogenous and exogenous should no longer be used for classification. The need for some neologisms is explained together with the meaning of these new terms. Two main groups are topical eczemas and hematogenous eczemas. Each group comprises three types, resulting in a total of six different type of eczemas. Each group comprises three types, resulting in a total of six different types of eczema. Mit einem Überblick über gebräuchliche Hautirritabilitätstests und hautphysiologische Messmethoden soll der Leser diagnostische Methoden kennenlernen. Die Notwendigkeit einer engen, motivierenden Führung des Patienten wird herausgearbeitet, da neben der suffizienten dermatologischen Lokaltherapie eine längerfristige Besserung nur durch eine Verhaltensänderungen des Patienten erreicht werden kann. Erläuterungen zur Prophylaxe sollen eine frühzeitige Risikoeinschätzung ermöglichen und zur Beratung ermutigen.

Abstract: The life style and demographic structure of Libyan society is changing, and this could affect the epidemiology of certain diseases, including eczema. The aim of this study was to assess the burden of eczema among a selected patient population in the Zliten area in the northwest of Libya. We conducted a retrospective study by reviewing case notes and records in public and private dermatology practices in the Zliten area. The frequency of eczema among patients attending dermatology clinics in the Zliten area was 12.8%, and the male to female ratio was almost 1:1. The most affected age group among patients was 20-29 years. Eczema represented a larger proportion of dermatologic conditions during spring and summer. Abstract: Eczematous dermatitis is very common in our country and all over the world. The aim of current study was to assess the burden, frequency and pattern of different eczema at outpatient department of Faridpur Medical College Hospital in the year 2009. Retrospective data were collected from medical records of patient. Patients of all ages & both sexes attending at the dermatology outpatient department of Faridpur Medical College Hospital in the year 2009 suffering from different eczema were included in this study. The frequency of eczema among patients attending at outpatient department of Faridpur Medical College Hospital was 12.8%, and the male to female ratio was almost 1:1. The most affected age group among patients was 0-5 years. Of all cases of eczema, 77.05% were endogenous and 22.52% were exogenous. The most common type of eczema was seborrhoeic dermatitis (27.17% of all cases), followed by contact dermatitis (20.00%) and Hand eczema (14.18%). Seborrhoeic dermatitis is commonest in young adult (21- 45 yrs), whereas atopic dermatitis is common in infant and children (0-10 yrs). Eczema is a public health problem in Faridpur, Bangladesh, and this necessitates prospective studies to determine its incidence and prevalence. Abstract: .. Objectives: To determine the frequency of different types of eczema in patients presenting in a tertiary care hospital. Study Design: Cross sectional descriptive type of study. Setting: Hamdard University Hospital Karachi. Period: January 2013 till December 2013. Patients and Methods: All new cases of any type of eczema were registered by nonprobability convenient sampling. Total 192 patients of both genders and all ages were enrolled after taking informed consent. Diagnosis was made on detailed history and clinical examination by consultant dermatologist. All specific and relevant investigations were done according to requirement. Patients were divided in two groups, exogenous eczema and endogenous eczema, for convenience. Data was analyzed by the latest version of SPSS. Results: There were 192 patients, out of which 52.08% were males and 47.91% females. Out of the total presented patients 50% had endogenous eczema, 43.37% had exogenous eczema and 25.62% had unspecified eczema. In endogenous group 11.45% had hand dermatitis followed by seborrhoeic dermatitis, atopic dermatitis and asteatotic eczema in descending frequency of presentation. Pityriasis alba, discoid eczema and pompholyx were seen less frequent. In exogenous eczema allergic contact dermatitis was seen in 14.06% patients, while irritant contact dermatitis in 11.45% patients. Infective eczema and photo-allergic eczema were seen in small number of patients. Conclusion: It was found in this study that allergic contact dermatitis was most common type of eczema followed by other types. Whatever the type is, eczema can be treated, symptoms can be relieved and recurrences can be prevented by appropriate therapy. All types of eczema affect patient’s quality of life adversely. However, they can be prevented by educating patients and their relatives to take adequate safety measures like using good quality emollients prophylactically.
