Développement et validité d'un questionnaire de qualité de vie spécifique de l'arthrose de hanche et de genou : l'AMIQUAL (Arthrose des Membres Inférieurs et Qualité de vie)

TL;DR: TheAMIQUAL instrument as discussed by the authors is a QV specifique de l'arthrose du genou et de la hanche, which is the state-of-the-art instrument for qualitative verification.

TL;DR: A semantically equivalent translation has been developed with cultural adaptation of OAKHQoL and it is quite reliable and a valid measure of the effect of osteoarthritis on the quality of life on Moroccan patients.
Abstract: The aim of this study was to adapt the knee and hip osteoarthritis quality of life questionnaire (OAKHQoL) into Moroccan Arabic and to determine its psychometric properties. After translation, back-translation and pretesting, the translated version was submitted to an expert committee. The psychometric properties were tested on patients with hip or knee osteoarthritis. Internal consistency was tested using Cronbach’s alpha coefficient (α), and the test–retest reliability using intraclass correlation coefficients (ICC). Construct validity was assessed by examining item-convergent and divergent validity and by comparing the average scores between age groups and walk perimeter groups. The study was conducted on 131 patients (115 with osteoarthritis of the knee and 16 with osteoarthritis of the hip). The “physical activities” (α = 0.93), “mental health” (α = 0.84) and “pain” (α = 0.88) dimensions of the Arabic version were internally reliable. The ICC were adequate to good; 0.83 for “physical activities”, 0.65 for “mental health” and 0.70 for “pain” dimensions. The instrument demonstrated good construct validity; all items exceeded the 0.4 criterion for convergent validity, except items 13 and 41 and most of the correlations between items and their own scale were significantly higher than their correlations with other scales. A semantically equivalent translation has been developed with cultural adaptation of OAKHQoL. It is quite reliable and a valid measure of the effect of osteoarthritis on the quality of life on Moroccan patients.

TL;DR: Although there was a correlation between clinical signs of osteoarthritis and radiographic osteoartritis, clinical signs were often present without radiographic evidence and moderate and severe radiographic arthritis wasoften present without clinical signs.
Abstract: The prevalence of joint complaints and clinical and radiological findings of osteoarthritis in wrist, hand, and knee joints was studied in representative population subsamples of 79 and 85 year olds. Joint complaints, clinical findings of osteoarthritis, and radiographic osteoarthritis were more common in women. Age related differences in the prevalence of osteoarthritis were not found. Although there was a correlation between clinical signs of osteoarthritis and radiographic osteoarthritis, clinical signs were often present without radiographic evidence and moderate and severe radiographic osteoarthritis was often present without clinical signs.

TL;DR: The most common and significant musculoskeletal disorders of adults are osteoporosis and associated fractures, low back pain, and the arthritic disor ders.
Abstract: Disorders of the musculoskeletal system are common, are responsible for a considerable amount of disability, impairment, and handicap, and are associ ated with high economic costs. Almost half (47.8%) of the adults examined in the National Health and Nutrition Examination Survey I (NHANES I) in the United States had either a history of chronic musculoskeletal symptoms or a musculoskeletal abnormality noted by the examining physician (32). Muscu loskeletal impairments (chronic or permanent defects representing a decrease or loss of ability to perform various functions) were found in about 10% of the population of the United States in the Health Interview Survey of the National Center for Health Statistics. More people (an estimated 12 million in 1984) have their activity limited by musculoskeletal conditions than by any other disease category. The total economic cost of musculoskeletal conditions was estimated to be over $59 billion in 1982, second only to disease of the circulatory system (63). The most common and significant musculoskeletal disorders of adults are osteoporosis and associated fractures, low back pain, and the arthritic disor ders. The epidemiology of these conditions is reviewed in this chapter.

TL;DR: The AIMS2-SF is a reliable and valid instrument among patients with OA and may be useful in routine evaluation of health status, in clinical research, and in predicting use of medical services among OA patients.
Abstract: Objective To validate a short-form Arthritis Impact Measurement Scales 2 (AIMS2-SF) among 147 patients with osteoarthritis (OA). Methods We used factor analysis to identify domains of functional health associated with OA. Multitrait scaling analysis was used to evaluate the reliability and validity of the AIMS2-SF. Results The results suggested that upper body limitations should be distinguished from lower body limitations in the physical function scale. The AIMS2-SF was psychometrically sound, with all 5 scales having high item-discriminant validity and Cronbach's alpha reliability above the 0.70 criterion (except 0.67 for the social function scale). Conclusion The AIMS2-SF is a reliable and valid instrument among patients with OA. Because of its simplicity and ease of application, it may be useful in routine evaluation of health status, in clinical research, and in predicting use of medical services among OA patients.

TL;DR: The hypothesis that relief from the pain of osteoarthritis as a result of THA improves sleep quality is supported and improvements in sleep contribute to the improved quality of life and day-to-day functioning seen after THA.
Abstract: Patients with osteoarthritis commonly complain of sleep disturbance that may be due to pain. Osteoarthritic hip pain is commonly alleviated by total hip arthroplasty (THA). Forty-eight patients waiting for THA completed a sleep questionnaire and were monitored using actigraphy and sleep diaries for 4 to 5 nights, 1 month before and 3 months after surgery. For the group as a whole, significant improvements were seen in subjective and objective measures of sleep after THA. In this study, 75% of participants reported that their sleep was never or rarely disturbed by hip pain after surgery. Actigraphy indicated significant reductions in time in bed and activity during sleep, more efficient sleep and less fragmented (restless) sleep. Differences between patients aged
